Monthly Injectable Treats HIV as Well as Daily Meds

From the March 8, 2019, edition of POZ online magazine:

(Lede paragraph)  Treating HIV with monthly combination injections of long-acting formulations of ViiV Healthcare’s cabotegravir and Janssen’s Edurant (rilpivirine) suppresses the virus as effectively as a standard daily oral antiretroviral (ARV) regimen. Given the success of the experimental regimen in a pair of advanced trials, the long-acting combination of cabotegravir and Edurant is poised to achieve approval from heath regulators, including the Food and Drug Administration (FDA)....
This would mark the first time an approved regimen does not require taking daily oral medications.

Nota Bene: The FDA's approval decision could come early 2020.  If approved, the injection would need to be given in a clinic and would not be self-administered.

Selfishly, I want the approval delayed so I can keep getting it for free, but I think more people will come around to it once it hits the market.
75% of us experienced injections site pain, yet 97% of us prefer this over daily pills. Once a month vs 30 x per month. No bottles with your name and drug names on them. Meds sit in your hip.

This is gonna be a game changer.

With all new combo's & treatments it's great to see more options developing, as people will have more choices and, this monthly injectable will surly suit some people.

As i've said before I think there are draw backs like having to preform monthly clinic visits, that will be a barrier for some people towards this Injectable but who knows maybe the next generation of this in the future could be 3 or 6 months.

They are working on having pharmacists do it and also arrange for home visits.
Im hoping that they're able to get it approved for bi monthly injections. The data so far looks good for those getting injections e every other month.

I think there should be more tailored treatments. And if someone can go 3 months, that should be available to them.

Pharmacy option if that happens it would certainly open it up to more people, and maybe one day once it reaches 3-6 months and self injection it would open up to an even wider market.

In the meantime we will have to wait, see how things go.

what's a "tailored treatment"? You could pay for "therapeutic drug monitoring" to have monthly blood work gone to determine the best dosage for you; but that's incredibly expensive for a small change from the recommended dosage.

Options aren't just "made available" (unless you're dying of leukemia and you might want to try stem cell/bone marrow transplant). Ethically, it will require several years worth of trials to determine if anyone can remain virally suppressed for 3 months, who might fit that same profile and who doesn't, what side effects there might be, and what restrictions/limitations might need to be considered.

What do you mean monthly therapeutic blood monitoring? I didnt mean everyone goes to a compounding pharmacy, although they should be available for those who can afford it.

What I mean is there should be more dosage strengths. Not everyone metabolizes 50 mg of Tivicay the same way. Some may get by on 25 mg, or what have you.

I dont think you need blood monitoring to see that based on someone's weight, height, age, and other health factors that they may be able to survive on less than the standard dose.

while weight, height age, etc. are factors influencing metabolism rate; however metabolism rate is more about digestive absorption rate, the rate of distribution, and rate of elimination; but I get what you're talking about.

I've always had a really fast metabolism. Maybe it's from the amount of Coke I drink (over 2-liters a day) or maybe it's why I can drink that much a day and still weight 145 lbs. Whatever the reason (I'm guessing genetic), meds of any type have always been hard on me. For most meds I take about half the dosage and have had successful treatment for whatever ails me. Clearly there's a dosage that is smaller than the dosage in the pills that works for me but without therapeutic drug monitoring there is no way to know the exact dosage is, so I don't not suggest or propose anyone not taking 100% of their meds as prescribed by their doctor.
(however, if you don't all of your meds and your treatment is still successful, well, I understand that ;) )

ViiV Applies for Approval of First Long-Acting Injectable HIV Regimen

Dosed every four weeks, the injection, which requires a clinic visit, includes long-acting rilpivirine and cabotegravir.

I remember I heard about this from a secretary (from my hiv unit) that dropped me off (i had missed the bus 😑) and he said that there was a trial about injectable treatment.
I hope these become available to everyone.
Do you think that if people use the injectable treatment more,the pills will become more affordable for the people who can't afford treatment ?

the simply answer: no. Until the US government regulates pharmaceutical prices/profits, medications will remain expensive. (have you seen the dust-up about the CDC patent on Truvada that Gilead has used to rake in millions of profits? 7 Senators have started working towards the CDC reclaiming those patent profits and/or reducing the price of Truvada.)

and why expect that people will switch to the injectable? It'll probably be terribly expensive. many insurance plans and state Ryan White programs won't be covering the cost. Right now, many plans don't even cover Biktarvy. If the injectable isn't on the formulary, it's price will put it out of range most people. Even if the injectable does get onto formularies quickly, the other meds will stay and all this stuff will remain high priced. With unbridled capitalism, there is never an incentive to reduce the price; only regulations can make that kind of change.

Since most of the drugs we take come from American companies,once the US does what you said,all the world will probably have affordable treatment.
I just don't know how people can be that awful.
Even though it's "free" Here (greece),the meds are still paid by our taxes. I hope something changes in the near future.

Meh. I wouldn't count on that happening any time soon. You have to inject them into your glutes and with the amount of fluid, I don't think that will happen.
