Very worried about Hep C
« on: October 20, 2010, 06:17:18 pm »
Hi - I posted that my partner had high Liver enzmyes and they were testing for Hep C. We get results soon but he is getting bad night sweats , bad headaches, and I can't help that it is Hep C. he has had HIV since 2001 and is getting over cancer treatment. It is just shit - I don't know how much I can take of this.
Feeling terrible and very upset.
We did have unprotected anal sex with a guy in June.

Is there a particular reason you suspect HCV infection? Specific risk factors like a history of sharing needles or syringes? Blood transfusion prior to 1990? That sort of stuff?

Great news there! HCV infection is a nasty thing, particularly when combined with HIV. :)

There is increasing evidence that HCV infection is sexually transmitted:

Very relieved matty.
And Edfu - I think there is a huge message that needs to get out there to the pos community that this is a real risk! Not just with extreme sex or fisting but unprotected anal sex.

Instead of wondering how much more YOU can take please think how your partner is feeling.I am sure he is had enough. Just be there for him. He needs you to be supportive.I have HIV and Hep C. Had Hep C 20 yrs with no treatment and no problems as of yet. Thank God.

I read through the articles and there doesn't seem to be any mention of HCV transmission through MSM oral sex so I guess it's not present in the semen?

Inch, I guess that's why doctors suggest the risk is mainly with guys who engage in more aggressive sexual behaviours like fisting, etc as it does need to be blood to blood - but my doctor said that when you are HIV + there is a highger risk of transmission with unrprotected anal sex - there was a study looking at a group of guys I think in Holland who practised unprotected sex (and I believe fisting also) and found quite a high transmission rate of the same strain of virus through the group.
I haven't done a search on this, but have had unprotected sex (passive) a couple of times in the past year and always hate myself after as it may open up the risk of Hep C infection.
(bad boy)!
BJS, yes I do realise that he needs support from me but he tends to get on with things and not want to dwell or worry as much as me - I do enough worrying for both of us!

In regard to HIV-positives being more susceptible to HCV transmission when serosorting for HIV and barebacking, see this:

http://www2.catie.ca/en/pif/spring-2011/sexual-transmission-hepatitis-c-are-hiv-positive-gay-and-bisexual-men-risk

This is the most pertinent quote:

----------

These risk factors for HCV among HIV-positive MSM (unprotected group sex, anal fisting, STIs the use of sex toys and recreational drugs) do not appear to be risk factors for HIV-negative MSM.28 We don’t know why this is the case. It may be that as a result of HIV weakening the immune system, people become more susceptible to HCV infection, though it is more likely as a result of some HIV-positive gay men engaging in more risky sex than HIV-negative men.

There are a couple of reasons why HIV-positive MSM may be participating in risky sexual behaviours. Firstly, “safer sex fatigue” may have resulted in certain groups of gay men growing tired of hearing about and practicing safer sex. There is also some evidence that since the introduction of highly active antiretroviral therapy (HAART) in the late 1990s, some groups of HIV-positive MSM have engaged in more high-risk sex, perhaps because they believe that treatment reduces the risk of transmitting HIV.

There is also evidence that some groups of HIV-positive MSM practice “serosorting,” which involves two HIV-positive men having unprotected sex, each aware of the other’s HIV status. However, it is not clear if these men are aware of the hepatitis C status of their sex partners. Some researchers argue that social circuits of HIV-positive men that do not use condoms are super-concentrating STIs, which in turn gives rise to hepatitis C outbreaks within these social networks. Finally, there is the added complication of HIV causing the HCV viral load to be higher in someone who is co-infected. This further increases the risk of transmitting HCV when two HIV-positive men have sex without condoms.

----------

The last two sentences above may be the most important.

"Mankind can never be free until the last brick in the last church falls on the head of the last priest."--Voltaire

I agree. When I was coinfected, my hcv VL was huge, 3 or 4 million something if I remember correctly - it may have been 30-40 million - it was nearly ten years ago and my memory ain't what it used to be. The doctor who managed my hcv treatment made the comment one time that the hcv viral loads in his coinfected patients were always much, much higher than his patients who only had hcv.

Wow. I am coinfected with HEP C and my viral load at the time of testing was around 30000...my doctor told me it is pretty low and he has seen worse. Strange thing is that after starting Atripla, my liver enzimes lowered to almost normal after being in the 300s. I hope it is a good sign.
