Exposure - I have herpes, he's undetectable
« on: July 11, 2014, 10:30:10 pm »
I'll keep it to the point, with detail.

I'm male, have herpes, and take Valtrex daily. He's positive with an undetectable viral load but slightly low CD4 - which of course makes me more harm to him than he is to me.

We had sex, and he was the passive partner. It was all quite well and good for about 10 minutes, 15 max, until I pulled out and saw that the condom had essentially torn (he used Vaseline...  >:( if I had only seen beforehand) leaving most of my upper shaft exposed (if it helps, I'm uncircumcised and my skin doesn't pull back by itself when erect - when I have outbreaks, they're usually under the foreskin). Naturally we both freaked out a bit.

I went to my PCP about 12 hours later, took a test and it came back negative so that's a start. He didn't prescribe me anything. I called later when I got worried again and asked about PEP and it seems that no one there knows what it is - yet another sign that I need to find a new PCP. It's now been about 42 hours.

My question is not only "Am I infected" but might he be infected with my herpes? What would this mean for us in the future?
Should I bother calling the emergency room about PEP? I know having herpes makes HIV transmission more likely. How much would it cost with insurance?

The risk of HIV infection is greater for the bottom that for the top and only you can decide on whether you want to take PEP.  Being that he is UD, your risk would be on the very low side, but again, only you can decide on what you want to do.  I would not consider PEP required in this instance.

As for the herpes, you should check with your doctor.  There are many factors to consider and he needs to examine you physically to assess any issues.

The negative test result you received means nothing, because you could not possibly seroconvert in a matter of hours.

I really think you are going to be fine.

Joe
