Concerned about exposure

First, I would like to say thank you for all of your efforts in assisting others here!  I am sure that it can be difficult at times, but it is a great help to people who need help.

My partner and I occasionally will have sex with escorts (either together or separately).  Together, we do not practice safe sex, but with third parties, condoms are required and strictly adhered to.  Two weeks ago, we connected with an "escort" couple.  My partner was insertive both orally and anally (with condom) and I was the receptive partner orally only.  There was no ejaculation inside my mouth, but I can certainly recall some pre-cum albeit I tried my best to avoid it.  The escort couple we now know are HIV+.

This past Friday, my urethra started to itch heavily and I had some clear discharge.  I continue to have discharge, itching and now irritation around my rectum.  While I am not a doctor, and will most certainly make an appointment asap, I think that I have either Urethritis or more likely Chlamydia (yes. the partner knows).

My concern is that while oral sex is considered to be lower risk activity, I most likely have contracted Chlamydia orally from an HIV+ individual and would that in essence also mean that I have put myself at greater exposure for contracting HIV?  Given that I did not know about the Chlamydia in advance, I doubt that I would have been a candidate for PEP under normal circumstances.  So needless to say, I am freaking a bit.  How closely linked are the two STD's?  Thanks.

I think that transmission rates are higher in people who have a STD and have unprotected sex...ie if YOU had Chlamydia and had unprotected sex with someone with HIV. However, I'm not sure I've heard about a higher transmission rate if someone has both HIV and Chlamydia to someone who has neither as in your story. I'm not saying that there isn't a study on it, I just haven't seen it.

NB. Any advice about HIV is given in addition to your own medical advice and not intended to replace it. You should never make clinical decisions based on what anyone says on the internet but rather check with your ID doctor first. Discussions from the internet are just that - Discussions. They may give you food for thought, but they should not direct you to do anything but fuel discussion.

Yes you can contract Chlamydia from receiving oral sex but you will not contract HIV in by receiving oral sex.

The chances of your negative results changing to positive by 12 weeks would be rare. Do you have meth mouth or any other oral diseases? If not, look for a conclusive negative result when you retest at 12 weeks.

Thanks.  I have not yet tested as the incident happened two weeks ago and the first signs of Chlamydia just occured on Friday.  I will make an appointment this week with my doctor, but it would seem to early to take a HIV test now, right?  Are you suggesting a HIV along with the STD test?  Also, if Chlamydia was absent from the equation and I gave oral sex to a HIV+ person (without ejaculation but with some pre-cum) but with no overt dental issues, would PEP be in order or a HIV test?

No PEP would not be necessary if you performed oral sex an HIV positive person.

From what you report I can't see any need to test for HIV at all. If you do, you will test negative at 12 weeks.

As for chlamydia, it cannot be diagnosed by symptoms either. A large proportion of people with chlamydia (or gonorrhoea for that matter) are asymptomatic. The only way to know if you have any STD is to have the appropriate test performed at the right time.
