Husband with potential exposure
« on: November 18, 2017, 11:41:31 am »
My husband (with my full knowledge) occasionally goes to a sex club with gloryholes.  A week ago, he had an encounter at this club with a stranger.   The man encouraged my husband to put his penis through the hole, which he did, to receive oral sex along with manual masturbation.

After a few minutes of this my husband realized something suddenly felt different on the head of his penis.   He pulled away and looked through the hole to see the buttocks of the man as he stepped away.

We assume the stranger inserted my husband’s penis into his anus.   My husband feels confident that only the head was inserted, and it was very brief.    My husband immediately went to the restroom and inspected his penis for blood (there was none visible) and washed himself with hand sanitizer as well as soap and water.

The following day my husband went to urgent care and was started on PEP.   The medication was started at 20 hours post exposure.

Obviously we are very concerned and our nerves are shot.   Our questions are “would it be beneficial for him to take an early detection test at 14 or so days post exposure, or will the PEP interfere with the result”.

Also, Is There a risk of transmitting the virus to me if I perform oral sex on him but not to completion/ejaculation?   If we have intercourse we will certainly use a condom.

And he will most definitely complete all of the PEP (he is one week into the treatment now) and he will follow the recommendation he received of testing at 6 weeks, 3 months, and 6 months.

Hi

Simply test at 6 weeks and 13 weeks post completing PEP for a conculsive result and also test for easier to transmit STI's whilst you are testing for HIV. Testing at 6 months is a very outdated advice from before my time as an adult, meaning it's very outdated.

Giving a BJ to answer your other question is a near negligible risk, other conditions are needed as the mouth lacks route to infect without damage and saliva and the environment in the mouth is hostile towards HIV.

Going forward use condoms for any intercourse no exceptions.

Thank you very much for the reply.

In the reading I’ve done, it seems the transmission risk is much higher in these first few weeks after exposure, as a person is extremely contagious in this time period.    That was my concern regarding performing the oral sex and my risk of infection.  But since he’s on the PEP, would that lessen the risk?

Hi,

We always assume your partner is living with HIV when doing assessments.
So this changes nothing in what Jim has advised you , giving your husband a blow job remains a near negligible risk.

Best,

Wade
