Cervical Dysplasia, anyone?
« on: July 16, 2008, 12:33:00 am »
Hi, I'm new here. I'm 24, and a few months ago I had a cone biopsy to treat what was classified as moderate dysplasia. The results came back saying that my margins were clear, meaning they removed the abnormalities that were on the surface of my cervix, and that the abnormalities hadn't spread into my cervical canal. They also did a biopsy on a genital wart I have due to HPV, as well as a D&C, and from what I was told those things checked out okay. I have another appointment in August for a colposcopy and I'm hoping and praying that no abnormalities have returned, but being poz, and having HPV, I don't know how likely I am to get what I'm hoping for.

My questions to you ladies are:

1.) How many of you have been, or are in the same situation?

2.) Have any of you had surgery to treat this, and if the surgery results showed your margins were clear afterwards, has anyone had any recurrences of dysplasia, and how long did it take after your surgery for the dysplasia to return?

3.) I've read that recurrences of dysplasia are more likely to return in someone with a cd4 count either 200 or below, and that it is speculated that if a person is on a HAART regimen it may also aid in preventing the dysplasia from returning. In your opinions, or whatever knowledge you ladies may have that I do not, how credible are these things that I have read?

I was given my last counts in June my CD4 was 430 something, and he did not have my VL, but he said based on my other numbers he was pretty sure it was undetectable, and said he would call if it wasn't, and he did not call, so I'm assuming it's undetectable. Thanks for your help ladies, I'm so not ready to have my uterus yanked out at 24, and I absolutely hate general anesthesia, but for as much as I'm in denial, what will come, will come, so I guess I better get used to it.  Thanks again ladies.

Hello, welcome to the forums.   I had dysplasia back in1992 and 1997 and had cryotherapy(in 97) to take care of it. I was hiv negative at the time.  It did not come back until 2006. I opted to have a partial hysterectomy in 2007. I kept my ovaries and fallopian tubes.     Each person  has a different medical history and cancer runs in my family which is why I had a hysterectomy.                                                                             I'm not sure how HPV will  make the dysplasia progress in you but  it could be years before it comes back. I would not recommend a hysterectomy, especially if you are planning on having children,but it was a good option for me.
The other ladies might be more enlightening but also talk to your doctor about it. Hope this helps. Cristy

CJC- Thanks so much for responding with your own personal experience. I'm sorry that you had to go through this, and I hope all is well with you now. My doctor had mentioned something about a simple hysterectomy when she was briefing me on my cone biopsy surgery. So I don't know... but thanks again for your response. I feel strengthened, and thats what its about, supporting and sharing with one another. God bless, and hopefully we'll speak again in future posts.

BETTYTACY- Thanks for the welcoming and the wishes for luck, its always comforting to know that someone cares, and I could use all the comforting and support  I can get. Even though my HIV is under control I'm very depressed more often than not, mainly because I don't have anyone to talk to about my illness. I definately will join you all in the dating thread...looking forward to talking with you and other ladies... you made my day. Thank you!

Hi Sunshine-
Welcome!
I have had a cervical dysplasia since my early 20's. I am going to be 37.  I had a ton of colposcopy's , one or 2 cryo's, I am not sure.  I would just monitor it faithfully.  I just recently went for a pap smear and it came back all clear.  When I was working my company switched insurances and I had to go to a new gyn and she told me she wanted me to get a full hysterectomy.  I was diagnosed with the HIV by then, I think I must have been around 22 when I got infected and I was maybe 24 or 25 when this doc tried to get me to have the surgery.  For me personally, I think she wanted me not to have any kids because of my HIV.  I have had 2  pregnancies and 4 healthy kids since then.  I wish you the best of luck.

Snow- Thanks so much for sharing your personal experience with me. I hope all is well with you now, and its wonderful that you have 4 healthy children. I hope to have just 1, at some point, assuming all this dysplasia stuff doesn't cause me to have my uterus taken out. Also I can't believe people can be so cruel, especially doctors. Sounds to me like if you picked up on her not wanting you to have babies because of your positive status then chances are that's exactly what was on her mind. What disgusting, heart-less pigs people can be. You showed her with your 4 healthy babies. Congratulations. You're an inspiration to me. Thanks for the post.

@NothingBut Sunshine:
It's funny you brought this topic up.  I had cervical surgery (dysplasia removal) on April 29th, 2008.  The surgery was successful but my gynecologist did warn me that the dysplasia could return.   I'm 33...no children...HPV and HIV+ since 1995.

 Thankfully, I didn't have any bleeding...in fact NO bleeding/period since for me.  However, my gynecologist placed me on antibiotics for potential bacterial infections.  I'm still taking it now and it's July.  Well, apparently my urologist found bacterial growth in my urine in June.  He prescribed me the same drug that the gyne had prescribed as well.   Now, I'm very concerned about this bacterial infection.  What if doesn't go away?   I'm going to consult again with both gyne and urology.  As far as the dyplasia, I gotta make sure I go back for my pap smears.  The gyne said she needs to see me every 6 months for the paps.  MY viral load is 800 copies and my t-cells are 436.  It's a miracle because 6 months ago ...I only had 6 t-cells!  Good luck with your situation... and share any information with us.

Cheetara- It is indeed funny because I had my surgery on April 22nd , 2008. I myself, did not have any have any bleeding after the surgery. I did however, have a regular period. I've read that some women don't bleed for a while after, I think. If you don't mind me asking, did your doctor say what may have caused this bacterial growth in your urine? I really hope you're able to kick this infection.

 I've been unable to completely get rid of boils/staph for almost 3 years. It's a lot better, and sometimes there aren't any boils, now that I've started on meds for HIV but, I still get them... The HPV is a whole other thing... it sucks when you have things on you that are 100% visible. I mean screw the invisible stuff too, but at least you can't see it.  :P Well Ma'm, here's hoping our dysplasia doesn't present itself again, and if it does, here's hoping it doesn't present itself anytime soon. I've still not finished recuperating. Mentally that is... I need a break. Good luck to you as well, darlin'.

@NothingButSunshine:
I see you had your cervical surgery just days before I had mine.  My gynecologist didn't tell me that my urine could grow bacterial infections.  I'm going to ask her why she didn't mention the potential problem.  The urologist tested my urine and said that after 7 days the urine had developed bacterial growth.  So he instructed me to take the antibiotic Cephalexin...same drg that the gyne placed me on.  LOL  HPV is a pain in the ass.  I have the invisible stuff going on and it's driving me crazy.   Take care of yourself and in the meantime I will contact my doctors and get more answers!

I'm going now...to enjoy the nice weather.  Chat with you soon.

Hi Sunshine

Im so glad you brought this topic up im stressed up to my eye balls thinking what if......... i gave birth 3 months ago to a healthy baby boy, the love of my life i was all clear before my pregnancy. I just had a pap smear test  done and almost had an attack when my doctor informed me that i had grade III cervical dysplasia which makes me a high risk candidate for cervical cancer. Im having a coloscopy done next week and im crossing my fingers that it comes back clear because i had planned on having baby number 2 soon. I never knew that HPV posed such a high risk for Poz women and im glad i did that pap smear. Please ladies never skip the pap smear tests they are very important.

I am going through something similar. I just had a colposcopy last week and am waiting for the results. My pap showed "high grade" My doctor mentioned HPV but said that did not show up on the pap. I guess this is common because of the HIV...we shall see.
