I just travelled to Greece and Turkey a few months ago.  I'm not sure what "problems" you're expecting.  A guest (American) at the hotel in Athens where I stayed tripped down some stairs and had to be taken to the hospital for treatment... she didn't pay a cent.  As far as Turkey, Istanbul is as cosmopolitan as anywhere and is huge.  I have no direct knowledge of what would happen if you suddenly started to keel over with PCP.

However, I would say if I was in the O.I. range <200 cd4's I would be cautious of traveling overseas, certainly to areas where there is no HIV infrastructure of note.  I didn't start going on overseas jaunts after diagnosis until I got up in the mid-300's.  It's just an issue of being practical.  There are risks to any travel, regardless of HIV.  If you're in a large city, you'll find medical help.  If you're out in the highlands of Ethiopia you may have some problems.

As far as your question about airport/ticket scheduling, I would think if you encountered a severe medical emergency that your airline would tend to work with you in getting you back to where you needed to be, though I'm sure it's not as easy as snapping your fingers.
