My partner just had cartoid artery surgery Doc is looking into meds that don't elevate cholestrol
He is on Crixivan and Combiver. Been on cholstrol lowerig meds Any suggestion would be helpful

My partner had cartoid artery surgery due to high cholestrol. He does have a family history but the vascular surgeon thinks med did contribute to the problem. ID doc notified and is looking into this. He has been on this combo since 1997. Viral load <48 CD4 638. Any ideas which med may not raise level of chol so high

Something from the nuke class apart from AZT which is in Combivir ie Truvada (tenofovir + FTC)

plus

an NNRTI like Sustiva or Viramune or perhaps the integrase inhibitor Isentress.

Isentress has no metabolic interactions with lipids so perhaps the best bet here.

Crix plus Combivir is truly an ancient combo these days...

Isentress and Truvada has easily been the most side effect friendly combo for me.

I'm on Viramune and Truvada and have experienced no negative side-effects at all. Some small percentage of people develop a rash early i treatment, but I didn't. Have been very happy with my combo.

Norvir is causing me endless blood lipid problems. The statins don't do a thing at normal doses for the Norvir cholesterol issues.

I don't know about any other combo; I've been on Norvir, Invirase and Truvada my whole time.

I recommend not using Norvir if you are concerned about blood lipid issues.

I started Isentress and Truvada about a month and a half ago and I am not experiencing any side effects at all. In fact I was getting bad headaches before starting meds and since I have started I have not had any headaches at all.

For the long term I'm concerned a little about Truvada and it's possible effect on the kidneys. I've decided not to take any NSAIDs at all (aspirin, Advil, Alleve) and make sure to drink lots of water, in the hopes of minimizing any possible kidney toxicity.

I'm glad I started meds while my numbers were still decent (390 CD4s and 9k VL) and I was not feeling sick (other than the headaches), since the whole thing has been a seamless transition.

I take Norvir, Truvada and Reyataz.

I have no side-effects except for a benign effect of Reyataz; which raises the bilirubin in my blood to a level six times higher than normal, and causes my eye-balls to have a yellowish tint.

Because I've had bad side-effects with every other drug combo I have tried, I am very pleased with my current meds. I have no vanity issues; so the slightly jaundiced tint in my eyes is not problem for me.

My cd4 count is in the 500 range. My viral load is undetectable.

Do I understand that you take one Truvada a day and Isentress twice a day (three pills total)? That would be an improvement on my regimen.

Exactly. Also, neither have with/without food restrictions.

(I also have Prezista added in but that's because of my resistance profile -- the "normal" regimen would be as I described above)

Have you talked to your Dr. about other combos that don't require Norvir? Isentress/Truvada is excellent but there are others. Assuming you don't have abacavir hypersensitivity and no resistance issues, Rayataz/Epzicom is very good.

Switching meds doesn't make you resistant if it's done the right way.

Might be worth mentioning that Reyataz uses the least Norvir of any PI-based regimen (1 capsule per day). Sometimes it is possible to use Reyataz without Norvir but it this is not recommended if you've used other PIs. Reyataz does not cause the increases in cholesterol seen with other PIs.
