plzz Help... iam resistant to atripla

Iam 21 years old and Hivpoz infected in 2012   :'( I was very sad and Tried to commit suicide twice  ;D  :D  sorry guys I was very scared
my VL was 280000 and my cd4 was 280... so I met DR and he recommend I start MED
and I started it was ( truvada,norvir+prezista)  but It was sooo bad I had pain in My stomach and Nausea and Diarrhea  all the time I was sad and  :'( every day I went to get test and I was vl 10,200 and my cd4 was 300 ... I was not happy with this MED and the Doc say that he will change it to truvada+intelence  :D and it was nice nooo pain att all in my stomach and i was happy but i most take this MED with foot.. so this year 2013 in January I went back to Dr. for blood test !! i went back to Dr to see the result  he say that my vl is 11,800 and cd4 is 280  he ask me if i miss doses my answer was NEVER  :o and i asked he if he can take test nooooow and see it was just 10 days or less than the first one ... he do it  and he ask my to come back after 8 days or 10 days  >:( he say that my VL is 500  and  my cd4 was 320  >:(  i asked him how it will be like this and I dont understand .. he blame my Hiv-1 mabye is unique he cant explain more ....
so I chang it  to atripla  :'( and  I test my self in mars and it was Vl 90  and cd4 is 360 or somthing  than I went back to my country for 4 monthes and when i come back to Europe  where i live !!  so i test my self in Sep 13 so in around  27
i get the answer  and my VL was 93  and cd4 400.. than the Dr ask my to come back after 3 weeks or less than that  :'(  i went back to him to make a test it was in OKT 2013 11 ... I get the answer 21 or 22 OkT five days ago  :'( he say  that iam resistant to atripla  :'( and I swear to God that I never forget any tablet from atripla  :'( but I'm taking every morning  natural herbs  :'( thy say its good Against hiv and u can have it with atripla ... it is my fault .... the Dr he talk about genewhatever and he will tell mee when my virus was resistant ... and he say it all the time  its noooot GOOOOD ... its NOOOOOT GOOOOD .... Its NOOOOOT GOOOOD
 :'( :'( :'( :'( :'(  Does this mean I am going to die  :'( :'( :'( 
iam resistant to atripla  :'( and why its nooot gooood ???? i will die ???
 I would love to have any one day pill ... plzzz anyone can help meee

Hi,

First time I get to talk to you , so, let's start with a greetings: welcome !

Now, This is scary and you are very young. I guess it is not easy to handle the situation.

Yet, the chalenge for your doctor is to get the viral replication suppressed.

Nowadays, it is very uncommon that this is not reached.

My doc told me, that HIV treatment decision where not as difficult as one might think, but, there a few cases where it can be tricky.

You have given a fairly precise account of your meds history.

You also have to know that, while you will see many people reporting that they have achieved UD (undetectable), there are a few where VL hovers slightly above 50, but less than 400, and, well... they are doing fine, somehow.

Have you told your doc about the Herbs you take in the morning ?

Recently I found one that was very good for me, but contained St John's Wort (you may have to google to find the exact name in your native language). You should know that St John's Wort (a very common herb to relax) is a NO-NO when taking Atripla. Sometimes, it is difficult to get through the entire composition to find what is exactly in the herbs you are taking.

Anyway, with the CD4 level you are at and since you have access to meds and professionnal doctors, you shoud bear this in mind: there are many options to choose from. Ultimatly , they will find out something for you

I told the Nurse (she take blood test) and she told the Dr  but noooo one told to meee stoooop taken it  :(

Hi Zoob, welcome to the forums.

It sounds like you need to find a new doctor. He should have run resistance testing on you before putting you on meds. This way, your resistance issue could have been identified before he put you on a med to which you are resistant.

You are most likely resistant to the Sustiva (aka efavirenz) component of the Atripla. Atripla consists of three drugs: efavirenz, tenofovir and emtricitabine. Tenofovir and emtricitabine are also the two drugs that make up Truvada.

If your doctor thought you were having problems with the combo that included Intelence (aka etravirine), he never should have put you on a combo that included efavirenz. They are in the same class of meds (NNRTI) and resistance usually carries across the class.

Some people with resistance to efavirenz can take another med from the NNRTI class called Viramune (aka nevirapine), but it depends on your exact resistance profile and you need a doctor who knows what he's doing where this is concerned.

By the way, it wasn't necessarily anything you did - or didn't do - that cause this resistance. The person from whom you acquired your hiv infection was very likely resistant to efavirenz him or herself. Resistance to efavirenz is one of the most common.

You DO have other options for a new combo, so don't despair. For example, you can take Isentress (aka raltegravir) plus Truvada.

I have to wonder how long you gave your first combo (Truvada, Prezista, Norvir). The stomach and intestinal upsets will usually calm down within a month. I'm on this combo and had a problem with diarrhea in the first few weeks, but I rode it out and I went back to my "normal" by the end of three weeks.

This combo is usually one of the easiest to tolerate, although the first weeks can be a bit difficult. If you only took this combo for a week or two before giving up on it, you might consider trying it again.

I would again urge you to try to find a new doctor, making sure he or she has experience with hiv positive patients. Your current doctor doesn't sound very experienced or knowledgeable, going by what you've written so far.

You would also be wise to start learning as much as you can about hiv yourself. The Lessons section of this website is a very good place to start. The lessons are written in easy-to-understand English and you are most welcome to come back into the forums to ask any questions that may arise while you read.

You might notice that hiv meds always have more than one name. When the name starts with a Capital letter (eg Sustiva), that is the brand name. When it starts with a lower case letter (eg efavirenz), that is the generic name. In Europe, most doctors use the generic name of a med, except when talking about pills with more than one med in it, such as Truvada or Atripla. It can be confusing, but you'll get used to it eventually.

Isentress and truvada - for most people its smooth sailing as far as side effects.  Don't need to take it with food. Do need to take the two pills of isentress - each pill about 12 hours apart.
Stick to it. Get an HIV doc where the communication is good - doesn't have to be the most famous or "best" doc, just the right doc for you.
Do you have a GP - general practitioner?  Ask your GP or your HIV doc about your mental health.  Two suicide attempts - probably a good idea to follow this up and get some professional help - some sort of mental health counselling..  Maybe there are support groups where you are living now.  Meet some more young people, or any age really, with HIV.  Learn how its manageable etc etc.  Seems like you have an awful lot of anxiety about some unnecessary fears - like you are going to die from an HIV infection.  Assuming you have access to good care and can afford the treatment, HIV will be very manageable.  Time to get your mind around that...

I was born before you and before the epidemic. So I can remember 'the dark ages'. ii can sympathize with how scared you are.

This is not a matter to take lightly, but, I have always been suprised that my Doc gives at lot of attention to my health (on one hand, as he is both my GP and HIV Specialist), but, he always shrugged at my scare for Opportunistic infections, etc. He takes it lightly because HE KNOWS; but the patient, me, does not.

I have never been in low CD4s, but he sure said to me that people in the 400+ are very unlikely to have AIDS related problem.

You have HIV.
It causes AIDS 
BUT, You do not have AIDS

Back to the resistance test...

You are getting health care in Europe. That is good...
If you are in a part of Europe where European standards (aka guidelines)  apply, then the resistance should have been done. A resistance test takes one month and must be done while VL still above , say 500 at least. It costs little less than a 1000 Eu or $ 1200 (depending where you live, of course, you don't buy them on amazon...)

One day pill: I am on a once daily regimen. It was 3 pills (at same time), now it is 2 pills (same time)
Whether it is 5, 4, 3, 2 or 1 makes no difference if you are well organized

I used to be on a twice daily regimen before moving to once daily.
That makes a real difference in life.

Yet, as I said, I was on twice daily for a while (6 months). I prefer once daily but twice daily was not THAT bad. You might be better off with a twice daily that works for you, rather than chase a once daily at all cost, because most (if not all) twice daily will, one day or another be manufactured in such a way that they are autorized as once daily. Please keep faith in technical progress (and competion between suppliers)

Altogether, you should realize that, since care is available, and you have started treatment early enough, it is very unlikely that you will ever have an AIDS event.

As a side note, I side by Mecch: please address the suicidal mood seriously

Suicide will cure HIV. But, if you die because you are scared of a virus, you let the virus win

the Dr call me and he say that Iam not resistant to atripla :D and he think that i should keep taken atripla !! I do not understand how the virus rose to 500 ???
I had the same before and the dr say that he cant explain it  :( >:(

Im sorry to hear that news about your family.  Its going to be a burden continuing in life without your family there for you.  But a person can go on without a family!  You will need to rely on a bigger mind and heart than your family is displaying.  You will need to develop friendships and other mean of support.   

I don't think there is ANY reason for a family to expel some, excommunicate, reject.  Maybe if the person is dangerous to the family, some kind of maniac or out of control criminal. 

I don't know if you feel like talking more about that in the public forum, but feel free.  Or, you can always PM me if you want to talk but don't want it to be public in the forum...

What country are you living in now...?  Are you living where you have some contacts from growing up?  Also, what sort of services - medical health, mental health - are there in your country?
