Doctor's Visit, say what?

So, I went to the Doctor today and had some interesting news.

First off, despite restricting my calories and working out like a mad man I seem to have gained weight which of course does not bode well.  The doctor said it might be the meds, but he wanted to wait 3 more months and see how it was going then before he thought about prescribing anything to potentially help.  He mentioned insulin-like HGH but said use of it was associated with the development of diabetes, so I'm going to do some research on that before I even consider it.

He's also going to check my free testosterone next week and see where it's at.

My CD4 coming up slowly is something we discussed in detail.  He mentioned potentially pulling me off of truvada in order to perhaps stimulate some CD4 growth and I believe the regimen he mentioned was Epivir + Selzentry.  Considering I just got undetectable this is not exactly something I really want to screw around with just yet.  If my CD4 is being affected by it what choice do I have though?

Anyway if anyone has any experience with IL-HGH or Selzentry + Epivir (or any other combo lacking truvada) please feel free to chime in.

I was going to ask the same thing regarding the weight gain --- is it putting you seriously overweight?  I have noticed that I have gained a lot in my stomach area since starting meds, but still at a good weight for my height - just need to try to distibute it better....

Hi, I did too gain about 8 kilos since I started med a year ago, and this with 4 times a week 90 minutes work out and watch carefully what I eat.
I did stop smoking too, that may be the reason why.
But I noticed that I gained fat around the belt (trousers easy tell) ...
If anyone has tip how to get rid of this, I'd be very happy.

Yeah ad2san - the trouser/waist/stomach area is where mine is -- for now the solution I have come up with is a) to buy bigger boxers and b) to go up a bit on dress pant size - from a 32 to 34.... If it keeps expanding after that, then I will look at other options, but for now, I could use to gain a couple of inches (and I am 43, so I have to keep that in the equation as well --- my waist size has been a 32 since I was 23 years old - so I held on pretty long).

Trey have you "blowed up" that much? i dont understand why he would mention HGH as an aid, especailly with the Diabetes risk.

and yes, Ive gotten a little rotund in the middle....although im still fitting into my 30w ok, I notice them becoming just a tad tight.

So he mentioned Selzentry along with Epivir?  This is only two drugs, do you think he means Epzicom?

There was a study that reported better CD4 recovery after 48 weeks on Selzentry. Going on that drug requires the tropism test to make sure the virus is CCR5-tropic.

My belly has also grown a bit and I've gained some weight but not a huge amount. I am exercising less and eating a bit more so it's hard to pinpoint it on the meds. I guess if I pick up my previous levels of food intake and exercise and still don't lose it then I can blame it on the meds. It seems to be subcutaneous fat and not visceral fat but is there any way to tell for sure?

Well, er yeah -- are you able to pinch a couple inches of your whale blubber?  If it's all below the surface your body will be more like a barrel instead of an inflated intertube.

I am able to pinch mine - and it is soft fat  YEAH, GO ME, I have inner tube fat and not barrel -- so, I'm thinking that should be easier to get rid of..... God forbid I have to switch to Corona Light... Ughhh.  >:(

That depends where the hair is, amount of it, etc.  as it could skew the perception of depth in regards to fat distribution....   ;)

Yeah, but just because I have some subcutaneous fat can't there also be some visceral fat? There must be some sort of test that is able to determine if there's visceral fat (in addition to whatever subcutaneous fat one is able to pinch).

Mr Hell-in-a-Handbasket, have you considered that as you're working out a lot, some of it might be muscle gain? Muscle is heavier than fat.

When I was a fisherbitchman, I was all muscle and weighed around 160 - but I looked trim. Now that I don't use my muscles anywhere near to the extent I did when I was at the fishing, I weigh a lot less but have a belly. My honest friends say that my ass has grown as well. :-\

It may have been Epzicom and not Epivir.  My memory is heavily reliant on phonics so the fact that the two sound so similar may have been me filling in after the "Ep-".  We were doing a lot of talking.  I also forgot to mention my bad cholesterol was 119 which he said was pretty high but not high enough to be alarmed just yet.  My normally decent BP of 120/77 was 135/97 because as I mentioned in another thread I was stressing hardcore.

As for the IL-HGH it will be re-evaluated after my results next visit.  I'm going to keep up my exercise and diet as it's currently going and if I'm still not losing weight or worse gaining it then that might be something to consider.  I have all this time to research so I'm going to make use of it (and you guys).  I was 245# at my heaviest, but I've usually been around 185-190# when I was hospitalized I was 162#.  I'm currently 205# and despite eating 1200-1800 calories a day and expending roughly 300-500 calories via cardio on a mostly daily basis I haven't been able to move my weight down again which is concerning to me.  This might be because I'm approaching thirty.  I'm not usually one to blame the meds or the disease for everything, but I'm beginning to wonder, especially considering this weight is being put straight onto my belly.   My limbs are still the same size as a year ago.  Anyway I have plenty of time to mull this and I'm glad you all are weighing in (no pun intended).

The med change will also be re-evaluated when I get my new CD4 results back in 3 months.  I like Atripla, but I don't want to be stuck at <100 CD4 forevermore.

Well I agree <100 CD4s is not so good. You could get off the NNRTI combo and go on a PI or an integrase inhibitor based combo. This might buy you +50 CD4s or more.

This said, the CD4 gain game is a long one and tends to go in a pattern of slow incremental rise and sudden jumps, if it happens.

You could go veggie (or like me freetarian, ie not cook meat at home) it does wonders for weight and cholesterol

15-20 lbs over your average ain't really bad. HIV screws your fat metabolism and this may be a post-treatment normalisation. Ask yourself, what shape were/are your relatives at your age?

The key to burning fat with cardio is long, slow exercise, like 40+ minutes just over walking pace running, not hard and fast, perhaps speaking to a personal trainer would help here.

The HGH suggestion, I don't like, it's down sides tend to be more than the ups, and the effect is short term. Testosterone shot, that's a no-brainer :-)

The regimen changes were kind of brief, he made some suggestions, but said that he'd prefer to wait another 3 months to see if my CD4 stayed the same rather than change a working combo.

As for the insulin resistance Idon't know what that is.  He made the comment about IL-HGH basically almost as he was leaving the room, once again preferring to give it another 3 months before having an involved conversation about it.  This is the first time I've raised these concerns with him because previously I was attempting to gain weight as getting down to 162 had scared me.  Now that I'm putting a lot of effort into diet and exercise and not seeing any results I'm a little worried and perplexed.  I know how the equation works I've normally been able to manipulate my weight pretty easily if I put in the time.  This is like beating my head against a wall.  If this weight is where I will be staying I willl be seriously unhappy.

Anyway if anyone has any experience with IL-HGH or Selzentry + Epivir (or any other combo lacking truvada) please feel free to chime in.

Isn't epivir an old med?  Selzentry is a EI (Entry Inhibitor) ... 1 in the morning, 1 at night...  I believe dosage is 200mg, but don't quote me on that. it should work very well but only if you have a R5 (CCR5) tropic virus.  Requires a tropism test that *MUST* be done.  Failure to do this test could result in tipping your virus to (X4) CCRX4 if you have a dual tropic virus.    R5 and X4 refer to receptors on the cells that HIV uses to gain entry.  They don't really know what CCR5 receptors are used for... so do your due diligence and make sure you are comfortable.

There is another approved EI but it's requires nasty injections and is tedious to mix.  Vicriviroc was in study but appears to be languishing.  Similar to selzentry (mariviroc) but was a 30mg once daily, boosted by 200mg of Norvir which has a 5x boost.

I don't understand why he isn't pairing Selzentry with a modern PI like Isentress that has few side effects....  Epivir is a Nuke.... and if you are going to go with a 2 drug cocktail why not a PI + EI.  Anyhow, just things to ask about.

15-20 lbs over your average ain't really bad. HIV screws your fat metabolism and this may be a post-treatment normalisation. Ask yourself, what shape were/are your relatives at your age?

Better yet, La Mademoiselle of Natchez should ask himself what is weight was before his undiagnosed opportunistic infection better known as HIV wasting, before he found out about his infection.  I recall him saying he was a porker.  Sounds more like to me that the rubber band has bounced back.

As for the insulin resistance Idon't know what that is. 

Insulin resistance is where your body becomes resistant to insulin.  It basically means that you can't metabolize sugar and thus become diabetic.  I'm willing to be it's related to weight. 

Now, love you darling, so don't take this the wrong way....... but....... have you looked hard at your diet?

Weight Gain or Loss is a function of Calories - Exercise = Gain or (Loss).

So if you are taking in 1800 calories a day and still maintaining or gaining weight you need to revisit either exercise or diet.

I have the same problem, but instead of insulin, my liver is pitching a fit and getting fat too!  You can live without your pancreas..... not so with your liver...  Yes it's stressful... and btw stress causes weightloss too!.

My first enemy was soda.  I drank way too much of it... so switching to water and kool-aid (no laughing Ms. P) helps cut down on the calorie intake with Soda.  That switch has allowed me to gradually trim calories from my diet and I'm losing 2 to 4 pounds per month which in my opinion is quick.  I'd be happy with a couple pounds a month tops.... the idea is gradual, sustainable weight loss, not yo-yo moves that freak your body out and make it think you are starving or crazy diets like Atkins that piss off your kidneys.

ROFL... yes I think Fisting might cause some weight loss too....  and I'd say your bloodwork would be quite odd if you were fisting before the doc got to you.  Bet the digital exam would be easy.  You could upgrade to the 5 digit exam.   LOL ROFL.

Just because someone can't get into a size 4 bikini does not make them a porker darling.  And it's not fat if you have a 34 waist....   You know you are fat when you wear black to Sea World and children ask you to sign their Stuffed Animal as "with Love, Shamu."  Just like you know you are old when the 20 something at the restaurant calls you Sir or Maam and you realize you *are* old enough to be their parent.

245# top weight for me only once in my life and only for about a year
162# bottom weight for me and only once for about 9 months
205# current weight
185-190# normal weight for most of my life

Said this earlier, but anyway I'm right now a good solid 15-20 over my average and I'm very conscious of everything I eat and how much cardio I'm doing, thus my concern about not seeing any movement.

I think Ms. HairyPhilicia is on to something, if you were 245 prior to hiv and then went down to 165 qnd are now regaining then you really are on track to be were you were pre-Hiv.

Trey,

You might also consider if you lost muscle mass, during your weight loss and are now regaining it. Muscle weighs more than fat and since you are exercising, some of the gain may be muscle, which would be good. Given your height, I think over 200 is probably not good for you, as my weight range limit is 205 and I am 6' 1" tall. Just be grateful that you don't have visceral fat, or you would look like me: a barrel, wearing an inner tube.

My sister has thyroid issues and I'm a little bit concerned this might be it.  She has to take an injection before every meal I believe.

that weight has taken me up from the 135lbs that I've been since I graduated in 1980 to 145lbs now.

After reading this string I would only add one suggestion and of course put in my two cents. 

Suggestion:  Try the low carb diet where you only allow intake of carbohydrates during one hour each day.  NEVER put your body in a state of ketosis.  Its hard on the kidneys.  If you only eat the 'goodies' during that one hour, insulin is not produced as frequently and the extra weight should drop off without too much of a problem.  Also, knowing that you can have your treat say between 5 an 6 PM each day keeps you from having to totally do without.  This seems to work for men more than women (thanks to Mother Nature).  Dont stop the exercise.. and keep having fun on Sundays (that helps too).

My two cents:  OMG - you're almost 30!  YOU OLD FART!  You should have over a decade before you need to consider age as a factor in this dilemma.  Take it from a genuine 'old fart', my early 40s were more energetic and belly fat free than any other part of my life.  I was a fat kid, (found ice cream sandwiches in 1967 and became addicted), and chubby even in college.  Give it a try..

From your profile pic, I pictured you as someone really skinny.  My weight around 18-21 y.o. was around 170 pounds.  Then, it was around 190 for many years.  My highest weight was 231.  I think I weigh about 205 myself.  I know when I got around 30, my metabolism seemed to slow big time.  I couldn't eat whatever I wanted anymore.

I also went up in weight from about 175 to 200 after starting meds and I tried to blame it on them.  Finally, I started watching what I ate and exercised more and I'm back down to 180.  I'm 6'1" and 38.  Stupid "use more calories than you take in" rule.  Thyroid is definitely something you should get checked out, though.  It's much more common in women but since you have family history, it's not a bad idea to ask your doctor about it.

I actually don't know to be honest.  My evil plan was to have the necessary 200+ CD4 so I wouldn't have to deal with this very issue.  Unfortunately my body decided to go with Plan B.  Yakamein told me about Dapsone at least 7 months ago, but I haven't asked the Doc about it.

bactrim was the only med that I took religiously from 1996 until 2008 when I finally seemed to be staying over 200 enough to stop the med.

What does the Mepron do to you?  Does it make your stomach hurt, or does it cause you nausea?  I could barely handle either, but opted for Bactrim even though it made me feel like I was peeing crystals everytime I went to piss.  I guess that's not an option for you though.

My CD4 is 500+ but I cant come off bactrim because I get back to back infections when off of it . I was on it for 10 + years and have tried a few times to stop but cant . Miss P called me a Bactrim addict once and I think he may be right .

I hate you all.  I've been off of my PCP prophylaxis for about 6 months now and yes it's Mepron.  I've toyed with the idea of starting it again but it really is pretty vile disgusting stuff.

I thought you were restarting it about the time of the trip to New Orleans.

Have to thank the OP for the wakeup call that is this thread.

The summer I seroconverted, I weighed 70 kilos by September, when I went on HAART.  I stabilized back at my normal 80.  Well this week at the DOC, i hit 100 kilos.  I had a good look at myself and its confirmed, I am a fat pig!  How the fuck did that happen so quickly?  I was always thin.   

Oh shit I'm going to a dietician ASAP.  I thought shrink, or ID, or endrocrinologist would have some helpful ideas, yet they all stare at me blankly! 

And I don't sit on my ass all day, and I don't eat like a pig...

So what are you eating?  I'd figure that if you lived in a European large city you at least walk a lot compared to these Americans that even get their dry cleaning via a drive through window.  Having not owned a car for 20 years and walking everywhere I know that I keep weight off that way.

Nothing changed in my diet. Though last year I drank more than usual.  But still seem to be gaining this year!  I ride my bike everywhere and don't own a car.  I can't figure it out - thats why I asked all the docs.

Yeah, that's on the Lower East Side.  I had those eons ago when they only sold them at Dean & Deluca and they are incredible.

I'm very partial to a chocolate croissant if I'm going the sweets route in the morning.  A donut is just so atrociously a sugar overload.

I went to four freaking grocery stores today, and no one has these damned doughnuts.  >:( ARGH! And all four of those were in NC. Tomorrow I'll go left at the end of the road and try the 4 local grocery stores in SC. I've also put calls out to my brothers and GrandMother to be on the lookout for a store selling them. Now that I can't find them, I want them even more!

Better yet, they'll end up like the overweight man in Apartment 2F of my building who was just found dead after not being seen for a couple of weeks.  He could barely go three steps without being out of breath.
