Long story - seeking reassurance

So I'll get straight to it. The other day I had receptive anal sex with a casual partner, they assured me they were negative, but I am perpetually sceptical/anxious.

We started as mentioned with receptive anal, he came a small amount into the intact condom. He then removed the condom and performed oral sex on him. I was quite drunk at this point, but I saw him put a condom on, presumably a new one although cant be sure and we had sex again, for a very short period 2-3 minutes, with no ejaculation.

The next day, roughly 24 hours later, I went to use the bathroom and found the condom inside me (horrible I know). From what I could see there was no sperm inside it, it was intact with some faeces trapped inside. I removed it and then 24 hours later after freaking out, went to get pep from the emergency room.

My questions are:

1. How risky was this encounter? I'm fairly sure the condom came off as I pulled away from him. It was extremely close to the opening of my anus, I realised it was there because the tip stuck out, I.e. didnt have to reach inside. Also the medic checked me and found no damage to my anus, and there was no noticeable blood or semen in my stool.

2. I started the pep roughly 50 hours post the initial incident and 26 hours after finding the condom. Is this an appropriate timeline for pep to be effective? And does pep work?

Apologies as this is a fairly graphic story, I'm extremely worried as I'm usually very careful and want to have a full understanding of my situation.

Well to start with your risks are the blowjob and the condom being left behind.   

The blowjob was a near negligible HIV concern, the condom being left behind in this case as you can't account for how it happened I would simply put this down as unprotected intercourse.

The good news is you started PEP and that it's highly effective and if what you posted is true and your only sexual exposure then I fully expect when you do test, to test negative.  So relax!  Just test at 6 weeks post finishing PEP to confirm with an approved antibodies test and if you want a conclusive result than also test to confirm at 3 months post PEP.

Here's what you need to know in order to avoid hiv infection:
Use condoms for anal or vaginal intercourse, correctly and consistently, every time, no exceptions. Consider starting PrEP as an additional layer of HIV protection going forward

Keep in mind that some sexual practices which may be described as ‘safe’ in terms of HIV transmission might still pose a risk for transmission of other STI's, so please do get fully tested regularly and at least yearly for all STI's including but not limited to HIV and test more frequently if unprotected intercourse occurs

Also note that it is possible to have an STI and show no signs or symptoms and the only way of knowing is by testing.

Thanks for the swift response. From what I remember, the 2nd intercourse was very short, maximum 1-2 minutes, probably less. The condom was left because he was going soft. Is it fair to say the shortness is a positive thing? Just still bouncing thoughts around my head.

Look I appreciate that you are looking for an answer and your mind must be playing out all the details but considering your unsure and only found the condom the next day let's simply chalk it down as worst case scenario unprotected intercourse.

Well in the worst case scenario considering you started PEP on time I fully expect when you do test, to test negative. Do make sure to test  to confirm your negative status.

It doesn't get much better than that, so continue to take the PEP & Test. Live your life as normal in the meantime
