my new result and percentage change

Hello guys
          i appreciate your answer,pls i recieved a call from my ID doc today,from what he told me i began to doubting myself again maybe things are not going well but he told me that it's ok.  these are my past results,
            27/12/07cd4 20 vl 1000000
10/4/08 cd4 86 vl 63
1/7/08 cd4 186 vl undect
16/10/08 cd4 196 vl undect ,then during september 2008 i was diagnosed with latent TB which my ID doc said it was pulmonary tubercolsis,then i was put on nomal dose of  TB medication for two months and redeuce it after two months and continue with the same TB meds but some was reduced,almost my final doses at the month end.so my new result was cd4 248 vl 21 which means undetectable,so my question was, why my cd4 still not going up is't because of the TB medications or cause of anxiety disorder ?moreover my percentage befor was 15% and today my doc said it's 12% .so what's the problem am i not doing fine or what?pls your advice needed now .thanks

It's not unusual for your cd4 numbers and % to fluctuate when you've encountered something like TB, plus you started your diagnosis with very low numbers.  Your vl is suppressed so your meds are working.  Why don't you try to relax a bit?  If you're adhering to your medication schedule, making all of your doctor's appointments and so forth that's about all you can do.  It can take years for some people to get their immune system back in a more desirable numerical territory after all of that.

I agree. DON'T freak out over one set of test results, especially after the TB.

CD 4 counts vary somewhat, even depending on the time of day of the test. Percentages can change as well. Also, our bodies deal with the virus and the recovery after treatement differently.

Trends are more important, so as long as things are undetectable, the meds are working. You will be fine.

Believe me, I know. I tended to rely way too much on scores, including my last "blip" being more than normal (i.e., undetectable to nearly 500, but now down to 130). I'm no longer going to freak out if something is the least bit askew.

Give it another test or two and see where the trend is going. And hang in there!!

Hello,
I recieved a call from my ID doc today. From what he told me, I began to doubt whether things are not going well. He told me that it’s OK though. These are my past results:
27/12/07-CD4 20, VL 1000000
10/4/08-CD4 86 VL 63
1/7/08-CD4 186 VL undect.
16/10/08-CD4 196 VL undect.

Actually, from your message I understand that you had an increase of the CD4 count. From 196 to 248, which by every standard will be considered a fairly good increase, keeping in mind that you have started treatment with fairly low results in the beginning (20).

The problem with the CD4 count is that the lower they are when you start the ART, the more difficult usually it is to regain the proper function of your immune system. This is also one of the reasons (among some others) why some of the current guidelines say that people should start when their CD4 count is nearer 350.

As for the percentage-it is not wonderful and it may make sense to check the total lymphocytes and the white blood cells to check whether they have fluctuated, as this could explain the reduction of the %.
