Hi,

First of all, thank you for all the support that you’ve been providing to this community. I’m a new member, and it’s great to see all the unwavering support and education around the HIV topic.

I’m reaching out to get an educated opinion on my HIV risk exposure pertaining to some of my recent sexual experiences. I’ve had the opportunity to explore the website, and it appears my risk is low, but I would still love to get an educated take on it.

Here it goes. The following are the facts. In sum, I had sexual encounters with three sex workers during a recent trip to Colombia.

Day 1: first sex worker, met her at a strip club, age and status unknown.

Activity: Protected vaginal sex (once) and protected oral sex (one BJ). No other sexual acts. No visible blood at any point and no condom failure. Unfortunately, I can’t confirm whether the condom for vaginal and oral sex was latex given that she provided the condom. However, this occurred at the most high-end strip club in the city I was in, so chances are that the condom was latex, but I can’t confirm it. Poor lapse of judgement; I should have checked the packaging.

Day 2: second sex worker, met her on the streets, age 20 and status unknown.

Activity: Protected vaginal sex (twice with latex condoms) and unprotected oral sex (two BJs). No other sexual acts. No visible blood at any point and no condom failures. The first BJ lasted about 30-45 seconds and the second one about 10 seconds. Another lapse of judgement. I should have used a condom for oral sex.

Day 3: third sex worker, met her on the streets, age 18 and status unknown.

Activity: Protected vaginal sex (once with latex condom) and unprotected oral sex (one BJ). No other sexual acts. No visible blood at any point and no condom failure. The BJ lasted about 1 minute or so. However, there’s a twist to the “protected” vaginal sex. After I ejaculated, she stayed in the cowgirl position while we talked for about 2 or 3 minutes. As we were talking, I was getting soft. When she finally got off the condom slipped off and stayed inside her vagina. She was able to pull it out quickly from the base (it was not stuck high up near her cervix). It appeared that my penis was covered in my own semen, but I can’t 100% confirm whether it was mixed with any vaginal fluids. I was really worried the next morning, so I investigated the condom. The condom had a decent amount of semen inside the tip. That being said, I think we can assume the condom protected my urethra up and until I ejaculated. I just don’t know if my urethra was ever exposed from the time I was getting soft to the point she got off – WHICH IS KILLING ME AND ADDING TO THE ANXIETY. I would like to think that given her body weight, gravity, her ability to pull it out quickly, and the fact that semen was inside the condom, the base of the condom did not roll up and I was protected until she got off, but I can’t confirm it :(. Another lapse in judgment; I should have grabbed the base of the condom and asked her to get off immediately.

Day 4-6: Symptoms

Mild swollen groin (comes and goes)
Tight neck
Burning hand and feet sensations (comes and goes)

Day 6-8: Symptoms

Mild swollen groin with armpits (comes and goes)
Tight neck
Burning hand and feet sensations (comes and goes)
Mild headaches
Mild sore throat

I’m currently on day 9 starting from my first sexual experience at the time of this writing.

Given the numerous unprotected oral sex incidents and the condom slippage, my symptoms, and coupled with the fact that it involved sex workers, I’m really worried about my risk exposure.

How would you evaluate my risk? Realistically speaking, how concerned should I be? Did my experience call for PEP? Any advice would be much appreciated. Thank you!

Read your post three times, you received blowjobs, not an HIV risk to you and had sex with condoms, and they remained intact during the act of intercourse and, during the last encounter, you slipped out whilst withdrawing; it's not an HIV concern.

Your issues have nothing to do with HIV from the encounters posted here. Move on with your life, continue to use condoms for any intercourse, and as you are sexually active, get an STI & HIV screening yearly out of standard routine.

Here's what you need to know to reduce your HIV risks:
Use condoms for anal or vaginal intercourse correctly and consistently, with no exceptions. Consider talking to your healthcare provider about PrEP as an additional layer of protection against HIV

Keep in mind that some sexual practices described as safe in terms of acquiring HIV still pose a risk for other easier-acquired STIs. So please do get tested at least yearly for STIs, including but not limited to HIV, and more frequently if condomless intercourse occurs.

Also, note that it is possible to have an STI and show no signs or symptoms; testing is the only way to know.

Kind regards

Jim
