Title: 1 case of unprotected insertive anal sex

With a woman who is HIV positive, with an undetectable viral load. I am circumsized.

It happened about 5 hours ago. Should I get PEP? (Also, I am in America, and have some health insurance, how much would it cost).

PEP is expensive and, should you choose to pursue that option, I've no doubt the cost will be discussed with you. If you decide to commence PEP, you need to do so within 72 hours of the sexual encounter.

You have had a risk and you need to be tested, The window period for HIV infection is 13 weeks. You should also consider being screened for other sexually transmissible infections too.

There are things working in your favour here. As the insertive partner you are at less risk than a receptive partner and the woman's viral load being undetectable also reduces the likelihood of you being infected.
