Been on my new regimen (Raltegravir/Norvir/Truvada/Prezista) for a month or so.
Viral Load down from 96,000 to 496.
CD4 up from 12% to 14% (it's like in the 200s)
Oh yeah. :)
I keep forgetting to take the morning dose, but with numbers like these who gives a shit?

I'm interested in your combination, as I'm taking the same drugs but without the Raltegravir. Do you know what sort of level of benefit there is adding Raltegravir into the mix, and did you and the doctor discuss this combination without that particular drug?

Oh honey I don't really know.
I was meant to be on this combo without the Truvada but Alain or possibly RAB suggested I needed to have it added. So I kvetched at my doctor to have it added.
And to shut me up, she agreed.
Frankly my eyes glaze over when Frau Eva discusses the drugs, so I'm not sure about the fucking details.
But to answer your question, yes I think raltegravir was always part of it.

Interesting. Although I'm not sure I'd want to add a fourth pill to my regime and the one I'm on at the moment means I can take them all at once in the morning and then forget about it. Still, I might ask my doc about the benefits of Raltegravir with this particular combination when I have my next consultation.

3-drug HAART regimens are standard for initial treatment patients, or those that change regimens due to side effects. A 4-drug regimen is often used for those with more advanced HIV, resistance issues and I guess *cough* adherence issues like the OP.

happy for you bud! I to am on the norvir/Prezista combo and I miss 20% of the dose the past month.. My t-4s are over 1,000 and unD viral load. My Dr. even prescribed one norvir per day (Instead of 2 per day) and reduced the Prezista. He is sure that my cholesteral will improve by this new dose! Your on to something good.
I'm happy for us both. Keep up the great work.

You offer much sagacious advice on these forums. You can joke around about it all you want, but I’m surprised that with all your knowledge on matters HIV you still think adherence isn’t important. I’m sure a brilliant mind like yours can come up with some way to remind yourself to take that morning dose. How about when you take the evening dose, put out the dose for the next morning in a place where you’ll be sure to see it in the morning.

It's the main dose I miss as well. I've resorted to taking it at different hours to keep from missing it all together, but for some reason still manage to miss at least one dose a week on average. Shits hard man...fuck.

Oh man, taking these pills is like dealing with an Ikea flat pack. There's always a handful of piddling little things left over.
I mean seriously, after a girl has washed down her bran flakes with a litre of Irish Cream what's she meant to do? One side of the dosette box looks much like the other. :)
So I take the fucking pills. But since I don't have the sainted Dr Gallant to guide me along the way I make the occasional mistake. ;)

Come on guys, get with the program. Get yourself a pill box, that breaks down by day and/or time and then you can fill it and forget it. I have one that splits each day into four sections, but I only take pills twice daily, so that gives me two full weeks of pills in one case. If I think I missed a pill, a quick check will tell me. I have had drug regimens that baffled the human mind and without pill cases, I would not still be here.
I can't buy this missed dose talk, as if it were unimportant. Get a grip, get organized and get compliant.

Pretty impressive numbers for a month in - well done!
As for adherence - I can't berate you for that as I still miss doses and not through forgetting either, but just because I choose not to take the odd day or two every now and again!
Do what works for you until the numbers start telling you otherwise.

The pharmokinetics of all drugs is good enough. Specifically, raltegravir has a long half-life, and is being tested as once-a-day right now, with good first results. Darunavir is approved once a day for treatment-naive people, so the PK works, and truthfully if you are on the 600mg dose the difference between 1 and 2 times a day in terms of effectiveness is unlikely to mean anything in practice, plus it is considerably better than missing a dose from a 2 x daily schedule.

Amazing. You must be relieved. As Newt noted the current study of once-a-day Raltegravir caused me to ask a couple ofdocs about it (as that's what I'll restart in another month). They affirm and one said while they wouldn't "agree to it" they concede patients having been using it off-label at once a day with great results.
With your clever graphics abilities, couldn't you doll up your pillbox to make it an attractive place to visit twice a day?
