Welcome to the "Do I Have HIV?" POZ forum.

This special section of the POZ forum is for individuals who have concerns about whether or not they are HIV positive. Individuals are permitted to post up to three questions or responses in this forum.

Vaginal fluids beneath condom

Hello,
I have been in continuous stress due to my risky exposure. Please help me. A couple of month ago I had sex with a sex worker, and the condom did not cover the whole length of my penis, it was not completely rolled down. When I noticed that (after a brief dipping of about 30 seconds), I rolled it down all the way. I have a small fear regarding the fact that the condom could have not been covering the entire tip of my penis when I first penetrated her. The girl seemed in good health, but I cannot be positive about her status.
I got tested 2 months after the exposure with a ECLIA test (HIV-combi test) and came back negative. Do you think I need further testing? What do you recommend I do next?

Hi Colin,

You are worrying needlessly. Your entire penis doesn't need to be covered. What is essential is that the head of  your penis is protected as the means of potential transmission is via your pee slit and foreskin if you are not circumcised. Otherwise as far as the length of the shaft is concerned, that is not important.

It's not a surprise that you tested negative since there wasn't any risk in the incident you described. There's no need for further testing this time.

Thank you for the response.The thing is I am worried that a bit of my foreskin was exposed. I am uncircumcised. The bottom of my foreskin was not covered by the condom for some seconds. Do you think I should repeat the test at 3 months time?

As Andy advised you, you didn't have a risk. You did not need the first test let alone anymore tests.

It isn't the bottom of your foreskin that matters. It's the inside of it. And as long as the head of your penis was convered then the inside of the foreskin was covered as well.

You might practice with a condom of the sort you are using to make sure they fit you properly and that you know how to put them on.

Again, you are worrying needlessly.

The reason I am worrying is that 2 weeks after the exposure I developed a strong flu, which was later on diagnosed as pneumonia. I had high fever for a few days, coughed for weeks and developed something that resembled balanitis on the head of my penis (some small red dots) . I could have sworn it is ARS. I think that i will not be properly at peace with myself until I get my 3 months test.

You were never at risk so your symptoms have nothing to do with HIV.

Nothing you are reporting is HIV-specific. But then no matter what your head is telling you, you did not have a risk.

If you need the inevitable negative test result to close down your fears than go ahead and test at 13 weeks.

That's strictly up to you.

I mean...as I said...the gland was covered, but the condom was not completely rolled down over the bottom of my foreskin. Which means that when I started penetrating, that bottom foreskin could have stretched and be exposed. The ring of the condom did not go over the base of the foreskin. Actually the base of the foreskin and the fact that the erection was full is what prevented the condom to be rolled all the way down. Immediately after beginning penetration...the erection decreased a bit and therefore I moved while penetration about 10-15 seconds. After that I looked at the condom and saw that it was only covering the head of the penis...and I rolled it all the way down.
I fear that some foreskin was exposed during my initial 10-15 seconds penetration....

Colin,

The bit of your foreskin you need to worry about is the bit that is not visible (ie inside) when your penis is flaccid (soft) and the foreskin is in its normal position over the head of your penis. When you're erect, it's the bit right next to the rim of the head of your penis. If your head was covered, this bit would have been covered as well.

You're worrying unnecessarily. Make sure you read through all three condom and lube links in my signature line so you can use them with confidence. And make sure you're using ones that are large enough for you. The condoms are a girl's best friend  link has condom sizing information. Get reading.

That is what is was saying before. That bit of foreskin right next to the rim of the penis kept me from rolling down the condom. Basically the ring of the condom was stuck between the rim of the penis and the foreskin. So...I should consider this a high risk exposure ? Get tested again?

You tested negative at two months. The vast majority of people who have actually been infected will seroconvert and test postitive by six weeks, with the average time to seroconversion being only 22 days. I really, really don't expect your results to change. You can confirm at three months if that's what you need to do for peace of mind, but I still say you're worrying unnecessarily.

I am sorry to be annoying but I am very stressed about my exposure. And by your answer I gather that the situation I described did put me in some risk of getting HIV. 
By the way...the test that was performed on me at 2 months was ECLIA (electrochemiluminiscence). I was supposed to be able to detect both antibodies and the antigen. Do you think this was a proper test? Should I get an ELISA test this time?

You were there and saw just exactly how much of your penis was exposed, I wasn't. But, with what I know about foreskins and how they work (and yes, I'm foreskin experienced), I truly do believe you're worrying over nothing. It's totally up to you whether you test again or not.

The test you used was fine.

One of the points I was trying to get across to you in my previous post is that even if you had totally unprotected intercourse, I do not expect your results to change. If you were positive, it's highly likely you would have tested positive at two months. You didn't and I don't expect your results to change if and when you test at three months.

I am continuously worried...Cannot sleep... At the end of next week it will have been 3 months..
I you were me...would you test again?
And if I test...should I search for a lab that does ELISA tests? Or should I go have the same ECLIA test? ...
I am very stressed out....

But as I said before..the ring of the condom was stuck between the rim of the penis and the foreskin...this means that my foreskin was exposed for a brief time when a began penetrating...How can I live with even the slightest doubt without testing?

You've already been told what our evaluation is of your situation. However, if you can't let go of worrying then go ahead, get tested and collect the inevitable negative result. I mean, what else is there to say to you about this?

For about 6 days now I have a rash on my forehead. It is actually composed out of several pimples which I never had before... About 10-15 small pimples on my forehead. Can this be HIV rash? I am EXTREMELY worried!

The rash you describe is NOTHING like the rash sometimes associated with primary hiv infection. It's not even in the right place.

If you insist on using this forum to give us a blow-by-blow account of your latest physical problems, you will be given a time out. We've already told you what we think of your situation - one, you didn't have a risk and two, your negative results aren't going to change.

Hello,
I am writing you with a new concern. Recently I engaged in protected intercourse with a promiscuous woman, which has the bad habit of having unprotected sex.
My concern is that some vaginal fluids reached the base of my penis, where the condom ring was and went underneath the condom, getting in contact with the skin of my penis. I do not think these fluids were able to get into contact with the head of my penis.
I immediately stopped having sex and went to wash up. What is the chance of getting infected during the washing ... if vaginal fluids came into contact with the tip of my penis alongside with the dripping water from the shower...
I am very concerned and seek an honest answer...

You haven't had a risk. It doesn't matter if vaginal fluids got on the shaft of your penis. Your skin is an excellent barrier against hiv. Getting vaginal fluids on your penis while washing is also not a risk for hiv infection. Once hiv is outside the body, small changes in temperature, and moisture and pH levels all quickly damage the virus and render it unable to infect.

Please re-read your entire thread. The woman may have been different, but you're asking essentially the same question.

Sorry for mixing the posts, it was not my intention to repeat myself.
But this is a different situation. It was not the same girl as before and I am just worried.
I cannot say for sure just how far could her vaginal fluids went underneath my condom. I hope they did not come into contact with the tip of my penis.
That is the reason for my worries.
Do you think i should test myself after this exposure?
