Nice that these are beginning again. I hope with this more structured and clinical approach, we will attract a broader audience. As for the drug chart, I know my number will be quite low, but I’ve had 9 of those medications.

Really amazing presentation- Thank you Michael. I can honestly say, that after 13 years, you’ve taught me some new things. I found the different medications, and how each class approaches treating HIV, the most interesting.  Also I learned that my never having taken my medication at the same time daily, hasn’t really affected my overall treatment!

Thanks! I was really glad that I could present this information about the HIV life cycle, medications and what they do, and what drug resistance is and how to not have that problem (take your daily meds!)

Jim will be posting a link to a pdf version of this presentation. If anyone would like to know more about how HIV meds work against HIV make sure to grab it and a take a look! If you have any questions, please feel free to PM me or post your question here....or make plans to attend our peer support group next month. You're going to be on ARVs for the rest of your life, you might as well know what they're doing for you!  ;) :D

Speaking of Jim, a big thanks :-* to him for giving such great information in the Q&A session afterwards. While I've learned a lot about HIV in my 40 yrs. living with it, and I've taken a lot of the medications, I can't even begin to rattle off all those generic drug names and regimens like Jim does. Dude! You're brilliant! :D

My notes will be posted tomorrow instead, sorry all.  Without going into details it's been a dramatic day and I need sleep.

Best, Jim.

;D Thanks, you make me blush, I wish I could take credit but it stems from my first ID that said to survive  you need to learn more than the doctors. Times have changed thankfully.

ojo.          @JimAllen, my friend I forgot the ask you that send ne an invite, I can’t wait to see what the expert Michael have shared with you guys… please, Jim, I want to participate in the peer support group. Although next month, I will have something better to do. I would be suffering on the beautiful wishes of Cancun, MEXICO, but maybe for September I will be able to attend, obviously if you invite me.

Just to remember all those years when all of us were learning about the pandemic makes me sad. What I remember the most is when people were dying alone and the doctors and nurses had to wear like astronaut suit. Just like people with Covid hat to endure. As remember, my father telling his girlfriend via video camera that he felt so lonely because nobody was able to visit him. He died depressed because he didn’t want to be tubed… anyway, it is what it is. Times have changed for better speaking of HIV treatment so call me let’s stop playing the victims let’s be VICTORS because we can still make plans for the future and for those who want to have babies, they can have them HIV free… Hugs

Micheal's full presentation

My notes:
  • What HIV Does
  • What HIV Medications Do
  • Antiretrovial Medications
  • Antiretroviral Regimens
  • HIV replication (life) cycle
Each stage can be a target for HIV drugs.

Cell Entry
Attachment Inhibitors prevent the virus from attaching to the CD4 receptor
capsid inhibitors interrupt the opening & unpacking of the capsid, preventing HIV infection of a cell.

Reverse Transcription
(NNRTIs) attaches itself to reverse transcriptase and prevents the enzyme from converting RNA to DNA.
(NRTIs) contain faulty nucleotides and when reverse transcriptase uses these faulty building blocks, the new DNA cannot be built correctly.

Integration
(INSTI) Integrase inhibitors target a protein in HIV called integrase and block the integration

Transcription & Translation
Assembly, Budding and Maturation

PIs block the activity of the protease enzyme & prevent the cell from producing new viruses.
HIV capsid inhibitor, interferes with viral assembly.

Other drug classes
Post-attachment inhibitors.
CCR5 antagonists.
Fusion inhibitors.
Boosters
Maturation inhibitors

Resistance
  • Pre-treatment HIV drug resistance
  • Transmitted HIV drug resistance
  • Acquired HIV drug resistance

Treatment

Currently, the standard treatment combines three or more antiretroviral medicines and more recently the increase of two drug therapy options.First-line treatment recommendations are an integrase inhibitor plus one or two nucleoside/nucleotide reverse transcriptase inhibitors (NRTIs) (US mainly Bictavy). In low or middle-income countries WHO recommendations are for TLD (tenofovir disoproxil, lamivudine, dolutegravir)

Injectables, currently cabenuva. Although overall suppression rates in studies are similar to current treatments, concerns have been raised that when suppression failure occurs (virological failure), the acquired resistance % is very high with some publications showing 25-100%. *

Doctors should be cautious when prescribing long-acting injectable cabotegravir and rilpivirine to patients with long and/or complex treatment histories. In addition, people with low CD4 cell counts or previous experience of AIDS were excluded from clinical trials, and might also be at risk of treatment failure.
Particularly older adults, women, POC are under-represented. Some evidence indicates treatment failure & developing resistance seems more common in HIV-1 subtypes A6 & A1 and also under those with a high BMI. More study is needed.

ojo.          Hello Jim!!…I bet lots of newbies didn’t even heard about all these class of medication and how different they attack the virus, this is the reason of the so called meds “ cocktail” that I guess we don’t use it anymore. Everything is just in one pill… I guess it is important to mention that, even though it’s been 40 years off medication research, we are still juicing some of all those old medication’s specially, I think they all this one that I can think of, lamivudina (3Tc), tenofovir TDF and ritonavir as booster for other antiretrovirus…good job Michael and Jim… keep up the good work… hugs
