ADAP/HIV Care in Remote Areas

 :)  How does one go about receiving HIV care in very remote areas.   If I were to live in Tonopah, Goldfield, or Austin, NV how do I recieve care.  There is, at least not at this time, a waiting list in Nevada, so where would the care come from in those areas.   They can't mean to tell me that I would have to drive 4+ hours to Reno or Las Vegas for care.  Do the health departments in someplace like Tonopah or Hawthorne handle care for patients..??  There has to be some sort of other solution than making long treks.  Even California, if I were to live in Bishop or Mammoth Lakes, health care to Fresno or Bakersfield or Needles, health care is a hike.  Where does someone go?

I've certainly heard of people in isolated areas of the interior Mountain West region driving insane hours for their routine HIV care, yes. However, if one is quite stable with their numbers it's something you're only going to do twice annually.  Other than that it's quite impossible for most of us to offer you specifics on certain locations like you are suggesting. I suppose that's also for someone insisting on being cared for by an infectious disease specialist. There's no reason that you can't just see a doctor of Internal Medicine. If they don't treat much in the way of HIV they should have an infectious disease specialist to confer with over the phone, and who if you were to ever get in a pinch still be able to travel multiple hours to see. You're basically going to have to do your own homework with everything as I'm not sure what kind of case management you're going to find.

Certainly if one is looking for employment in such areas these are considerations to make. Is there some specific reason you have to live in such a place?

I fly. (No, not my broom - in an airplane.  ???  ) Sometimes I end up sitting around for hours waiting for a delayed flight. It happened yesterday - there was fog at John Lennon airport so we were stuck. After five hours of waiting, they announced they were going to fly us into Manchester instead and taxi everyone into Liverpool.

Why they didn't do this first thing in the morning is a mystery. They know that the early morning Liverpool flight is mainly made up of people travelling to hospital appointments. (58 of us yesterday.) Infuriating. My bf and I both had to cancel our appointments because there was no way we'd be on time. (A direct flight at that time would have just got us there before the clinic shut at 1pm.)

BTW, the airline is called FlyBe - but most people here call them FlyMAYbe. They're notorious for delayed and/or cancelled flights but if you want to get to Liverpool, it's either them or the ferry. >:(

On clinic days I have to get up around 4am (so early to give my gut time to settle down, have a shower etc) and leave the house around 6am. I don't get back home until about 9pm. It's a long day, but worth it. I often get the chance to do some shopping in Liverpool as well, which makes it doubly worth it as everything is more expensive on the Rock due to shipping costs. But I wouldn't want to live anywhere else - aside from maybe Hawaii or some other tropical paradise. Travelling for my healthcare is a small price to pay to live in such a great place.

Anyway, the moral of my rant story is that if you want to live somewhere remote or without good hiv care and end up having to travel elsewhere, that's a choice and you just have to get on with the consequences. Travelling to receive good hiv care is an inconvenience for sure, but worth it if you want to maintain your health. It's worth it for me anyway, you'll have to decide for yourself how you feel about it.

No sweetie, I wouldn't be living in Liverpool. If I ever win the lottery I'd have a second home there, but I won't leave the Rock. It will be my (primary) home until I kick the bucket.

When I say the travel is worth it, I mean the time and effort it takes, not the money. It's a very time consuming thing when your clinic appointment takes you away from home for a fifteen hour day. And I'm usually exhausted the next day as well, so it's two days gone out of my life.

That's not my point. If your flights to the HIV doc weren't paid for by the government, what would you do?

I'd see the extremely inferior hiv doc they fly in from Manchester to the Rock once a month - and raise absolute hell that they didn't have a better doctor for us. And I wouldn't shut up until they either got a new doc, or paid for me to go to Liverpool to see a real doc.

Meh, it takes five minutes from my office on campus to go visit Dr. Margaret Fischl, aka the Queen of AZT.  She is lovely.

Well I have to take a 4 1/2 hour bus ride into the city to see my doctor, and I usually crash on a friends couch overnight so I can take the next bus home in the morning. There's no way to do a same-day round trip, which sucks. If I could afford to live closer I would. I can't afford a car, either, so I'm bus bound for now. A lot of ASOs can help arrange transportation, but living in the boonies just means you have to travel for specialist care.

I can't comment on ADAP since I don't know anything about how it works in other states.

In New Mexico there is enough care scattered around the state between Albuquerque, Las Cruces, Roswell, Clovis, Taos, Farmington, and Silver city.  I believe that there is even on in Gallup.  Nevada is a whole different story, it's Reno or Las Vegas.
