It doesn't really mean anything specific to people with HIV.  It does mean something about HCR.

This was the first federal appellate to rule on the constitutionality of the healthcare reform law that passed a few years ago, and of course they ruled that it is valid.

It is not the end of the road, though.  There are 2 more appellate cases yet to be decided, and they will all certainly be taken to the Supreme Court, maybe as soon as next year.

The Supreme Court gets to chose which cases it hears.  If all the appellate courts rule that the law is constitutional, the Supreme Court probably will not take up the cases.  If one of the courts rules against the law, the Supreme Court probably would have to take up the case to resolve the split in opinions.  If the Supreme Court chooses not to hear any or all of the cases, then the appellate court decisions would stand.  In that case, this decision would be the law.
