This is about a dental issue, but it's Living With because it's all tangled up in that whole ADAP / Ryan White / Grady stuff.

Here's the deal:  About a year and a half ago, I went into the IDC dental office for a cleaning, and during that visit, it was discovered that I was just a hair away from serious OUCH and in immediate need of a root canal.  I had a funnel shape hole vertically centered through the tooth and almost to the nerve.  I went straight into the chair for the procedure, and they did what was explained to me as a temporary job that would do until I could be scheduled for the real deal.

This section of the IDC is notoriously backlogged, and it can take months and months to get a chair - and it did.  Many, many months...like 10 of them altogether.  I went in first after six months to get an, "Oh...did you not get a message from us?  We had to reschedule."  When the 10 month appointment came up, I had to cancel on THEM because it ended up being a time of major crunch at work, and I just couldn't do it.  Less than a month later, I got let go as a recession casualty.

So now here we are at a year and a half, and the back half of that tooth completely cracked off.  It looks like a "C" from up top and a "V" from the profile facing my tongue.  I am supposed to have an appointment coming up in February, but as of last night the temporary stuff they stuck down in the tooth has also popped out.  So far, I haven't been able to get anyone on the phone.

If anyone on here knows about this sort of thing, my question is:

Is this a "just ride it out for a few weeks til your appointment" situation, or is this a "uh...you need to be getting down there in person before..." type of thing?

The nerve, of course, was removed during the temp job, so it does not hurt.  But I'm assuming the temp goo stuff was put in there to protect gum stuff underneath from food and bacteria and all that.  So now that the goo has popped out, am I a ticking time bomb for any serious shit with infection?  Or can I be calm about it for the next little bit til they see me?

Any advice?

My advice would be to wait it out till FEB, or if you can afford it, just go and have the rest of it done, I had that happen to me (what your talking about) but I didn't have to wait that long (10 months is way too long)
so what they did to fix the broken temp tooth was to make a Crown, I had to wear a Temp Crown until the real one was made, but this only took about 1 week 7 to 10 days, not 10 months like you described, if you had to wait that long, your Ryan White Dental Program leaves much to be desired  ???

I'm not worried about the actual tooth or the appearance so much as any potential for a major infection or that type of thing.

Get a load of this mess: 

I just reached the desk gal at the dental office.  I asked for confirmation of my next existing appointement (2/17) and then quickly stated my issue and that I wished to speak to my tooth guy to find out if this way any sort of emergency...and she tells me, "Well, if this is an emergency, it will have to wait until March because our next emergency appointment is March such & such".

So February 17th it is...unless it's critical.  Then we can see you sometime in March.  WTF?

Anyway...I did convince her to let me leave a voicemail for my tooth guy, but it's a group voicemail.  HE is dependable, but someone in that bunch isn't because it is widely known that messages often drift off into the void without return.

We'll see.

Good gawd, sounds like a bureaucratic nightmare!

From your description, the tooth is still in place beneath the gumline.  If that's true, and only the top is broken off, even with the interior exposed (and the nerve removed), you should be OK for a few weeks if you don't have pain.  I did the exact same think with a molar (and WITH the nerve), and waited a few weeks to get it repaired.  It's when you have a hole into the gums, etc. that it could be an infection problem.  You sound pretty fastidious (hell, some people wouldn't even care about a broken tooth), so I'll presume you don't have any other dental issues that could be exacerbated....?

This probably isn't going to calms your nerves (pardon the pun) but my sister just went through a nasty bout of infections related to a root canal.   In her case, she ended up with an abscess, which required multiple rounds of nasty antibiotics.  So I think you are right to be concerned.  But my sister had the infection before she had the root canal.  And you've already had the root and pulp removed from your tooth, which I suspect lowers the risk of infection a lot.  Until your appt on the 17th, I'd recommend that you keep the tooth very clean, floss and gargle frequently, and generally keep an eye on the tooth and gums to make sure nothing gets worse (without the nerve, it might take longer for you to notice any pain from infection).

   It really shouldn't be that big of a deal.  The infection from rotten mouth only affects the valves of the heart.  You'll be fine, really.

Are you sure that's the only Place that accepts Ryan White Dental? most Cities ( like where I live) they have at least 3 or 4 Dental Offices that will take Ryan White Dental, if that's the only place you have in your area, then maybe you need to seek out another place somewhere else,  that meets or exceeds your States requirement for Ryan White Dental, I would have gone somewhere else, if that option was open to me  :)

Thanks for the posts, folks.  To answer some of the above...

This is the first real dental issue I've had (aside from the wisdom teeth thing); however, there has been some sort of infection thing regarding that tooth since the temp canal.  Back in the fall (September), I got a very sudden swelling in the gums right beneath that tooth.  It didn't hurt, but I could feel the swell as like a numbness.  You could see it some if you just looked at my face.  It died down some, but stayed big enough for me to keep checking on it for about 6-8 weeks.  That infection hasn't returned since.

Right now, if I inhale through my mouth, I can feel a slight sensation there - not really pain, just more sensitive.

Trust that I'm going to be extra mindful of oral care til the appointment.

And I live in the sticks.  I drive down to Atlanta for this.  I seriously doubt there are many (if any) other places closer, but good suggestion.

A great dental rinse (and I speak from experience ... not a spammer here) is Peroxyl.  Its not cheap but it will keep mouth infections at bay.

I doubt it.  It's unusual to come on as a new patient for most any doc or dentist and get your first appointment right away.  You'd usually have to wait a bit anyway.  I've got 17 days on this route. 

If I did get an infection meantime, I suspect the regular med team downtown would slip me a scrip for antibiotics and pain meds, and then I'd have to wait for that to go away before the dentists would do anything anyway.

With my luck, I'll get a problem about....oh...day 15 or thereabouts.

That's great news ... and Wow I have a dental restoration appointment on the 17Th as well ... that makes us tooth fairy's or something .

I have eight of these procedures....(mouth from hell) and i am glad you are going to get this done soon.  You do have a lower risk of abscess but you will know it if it happens.  The temp crown is the important next step.  You can keep those on for up to six months before the permanent one, although most of us have them for two or three weeks tops.  You will notice a dramatic improvement once the temp crown goes on.

I had this procedure done just yesterday, went in for a drill & fill, so they did that, but when they tired to fill it
it broke in half, dentist was LIVID , got out his phone called Ryan White, and told them, I need a crown Approval, after a few seconds, and told me ( "we got an approval for your new crown") so now were gonna do the crown-prep and the temp, I was in that chair to 2 hrs. but they got the job done, and I'll get my crown next week, now that is what I call service, I love my Ryan White Dental, here in New Mexico, they are FAST and have people that know what they're doing  ;)

How inspiring to see that bureaucracy can turn on a dime if the need arises.  I am fortunate to have private dental insurance (although like most...it only covers half the cost of a crown and root canal and has a $1000/yr max..one procedure pretty much uses all of that.)

Ryan White has a limit of 1,500 a yr. if you use that all up, you will have to pay outta pocket, ( I've had to do this before, I made monthly payments before when the State funding just wasn't available)  it only works, if the State funding is available ( sometimes it's not always) yesterday, I got LUCKY, but it doesn't work that way all the time, I've waited for about 4 to 5 months to get some things done before, so, I know how the OP feels, but I'm glad he got in to see someone, and if it's an emergency they'll help him out, funding availability that is  :)

About root canals--  If you can afford the crown, you should get it.  I learned that the hard way.  I went through all the trouble of 2 root canals only to have the teeth break and had to be pulled.  I didn't realize how important the crown was.  Without it, the tooth often becomes brittle and breaks.  My partner's tooth did this as well.  It is a shame that most dental insurance or programs will not pay for crowns, as they see them as cosmetic.  They are not just cosmetic.

Years ago my partner got a root canal and had issues with it for a year afterward.
Before we headed out of the US, he went to several specialists about the pain he had.
Both were respected in their fields and also friends, but neither found the problem.
We moved to this 3rd world country and he went to our regular dentist for cleaning.
The dentist saw the x-ray and immediately got us an appointment with a specialist.
Within the first 5 minutes she discovered our US dentist had drilled through the root leaving a hole that was allowing bacteria into the tooth and destroying the jaw bone. Three dentists in the states fucked up and my partner nearly lost his jaw over a root canal..... It took three surgeries to fix the issue with a cost of $900.00 and she apologized for the high cost!  :o
My thought is if I ever need such a thing, I will just pull the tooth.
There are fewer issues when the tooth is gone.

Got a cleaning on Friday, and they re- Xrayed  my choppers.  The short version:  they feel that the chipped part is too large to do much of anything with it as far as a crown goes.  They think they are now gonna have to do a bridge...which will mean that they are gonna yank the tooth.

I'd be lying if I said that there isn't a part of me that feels that maybe this could have been avoided if they could get it together a little better with the dental department & having to wait soooooo long to get stuff done there.  On the other hand, where would this fella be without it, eh?  And in the grand scope of AIDSy issues, I still have plenty of reason to smile.

I just kinda sorta wish it would be a front one extracted so it could be seen while I wait for the gums to heal before the bridge.  Where I live, that sort of thing actually gives you street cred (or gravel road cred, such as it is).

I apparently got my dates scrambled, but I just got a call informing me that my appointment is today.  I'm expecting some bullshit, particularly regarding pain meds.  It's just a gut feeling.  They'll either once again try to give me a scrip for something I can't tolerate, or I'll hear that the scrip can't be filled til the next day or...something.  Maybe I'll be wrong, but that's my hunch.

If I'm left in pain, I'm going to be one raging son of a bitch.

They didn't give me ANYTHING for pain when I had one of my bottom teeth pulled out, they just said to gargle w/ slat water, it healed in about 3 to 4 days, and in about a month the empty hole will close up, don't worry, you won't feel any pain  (when they pull it out) just a lot of pressure, at least I didn't feel any ............good luck  :D

And get this:  I was right about some pain med bullshit.  As part of the Grady / ADAP "cost cutting measures", they no longer issue client cards.  They encourage people to keep their old cards so they can keep track of their appointments easier.  I brought in my old card for this, and the lady at the window wouldn't process my scrip because my "card had expired".  I mumbled to her that of course it had expired because they don't give new ones anymore.  Still, she wouldn't continue until she had confirmation from two people that I was legit (and why this information wasn't on the computer screen in front of her, I don't know).  This took a little while, but eventually they said OK...and that the presciption would be ready in about 45 minutes.

They also told me that due to my allergies and stuff to other meds, the only options they had to offer me was Tylenol with a pinch of codiene or 600 mg pills of Ibuprofin.

I place my fist on my cocked hip and said, "Barbara, please.  I can score better drugs right outside the door - and faster.  But I'm not leaving without something - and make it the codiene on principle."

This little queer is glad he planned in advance because the pain started to appear during the wait.  I went out to car and crushed some Xanax and snorted the line...with a cop parked about two cars down.

And I also just drove through a major storm with a tornado warning.  I guess Jesus must have taken the wheel because - be it OZ or home - I wasn't about to stop for anything.

Some people just don't have a very good pain threshold, I've had several teeth pulled thur the yrs. (currently have a lower partial) and was NEVER given anything, but was told to take some aspirin, go home and suck it up, but for me there was nothing to suck up but blood, and the pain you are describing, I never felt any of that  perhaps maybe, you should do the same ::)

Hunter,

While staying in town at our friend's B&B, we saw a hillbilly with only 3 teeth getting dental work.
Those 3 teeth looked like hell, but those three were his GOOD teeth the dentist left in place..... :o
A week later when we saw him, he had a full set of implants and they looked great!
He was still an ugly hillbilly, but had a beautiful smile for a total of $13,000.
I am always amazed how affordable our medical services are here.

If you want to get any implants for missing teeth, the dentists do them for $500.00 a tooth.
We have a guest house that is always empty and we love company.

I've been going to the dentist for the last 40 yrs. of my life, OH Please, nothing they do hurts that bad you sound like child that's never been to one, maybe you should go more often, and you may not have those problems with your teeth, this is no body's fault but your own, also you might want to start flossing
in-between-meals, and brushing twice-a-day, poor oral health breeds bad health, I've even gone to the dentist when Ryan white didn't have the funding , so, I still went and paid outta pocket, so what's your
excuse  ::)

Den, sounds like you've been lucky where dentists are concerned. Not everyone has. Last time I had to have a tooth pulled (it cracked, through no fault of my own, thankyouverymuch) it was horrific.

My molars have claw-shaped roots, which means the tooth has to be cut into pieces before it can be removed. The dentist who pulled my tooth last year wouldn't listen to me and proceeded to try and pull it whole. At one point he had his knee on my chest, despite my protestations. He also wouldn't stop to let me spit and I started throwing up due to the pool of blood at the back of my throat before he let up.

The pain was unbelievable, even though the Novocaine had taken effect - and that was due in large part because he'd only numbed around the tooth in question, but he kept slipping off the tooth with his pliers and banging the roof of my mouth, under my tongue and other places. At one point he hit a tooth on the other side of my mouth so hard I was sure he loosened it - and he did. It was like torture. 

And the bastard had the cheek to show me the bits of my tooth (after he finally gave up and cut it up) and say "gee, guess you were right about your roots". He also refused to give me antibiotics and I ended up with a nasty infection. Good thing my GP is only over the road from where I live - I was a mess and don't think I could have rode in a car or bus without throwing up due to the pain and infection.

He (my GP) took one look at the damage that was done to my mouth (had deep nicks and cuts all through my mouth) and asked me if I wanted him to write a letter of complaint. I didn't, but only because I'll have to use that same dental clinic again and don't want any hassle. I did tell another dentist there about my experience though, because he wanted to know why I refused to be seen by the other guy.

Tim, I hope you're feeling better now. Getting a tooth pulled is no fun, even when it goes smoothly.

edit I've never, ever been afraid, or even wary or nervous about seeing a dentist, until this guy got a hold of me. Last time I saw a dentist, I broke out in a cold sweat as soon as I sat down in the chair, even though it wasn't the same person. He's ruined me for dental procedures, the bastard. >:(

Yeah, Ann, I have been lucky, when I used to work 15 yrs ago, I had very good dental Insurance, and always went to one even if i couldn't afford it, they do let you make monthly payments, at least over here in the States they do.......@ thunter34 , I'm glad your on the mend, you already sound better, feel better, you will in about a week or so  :-* did they say what they are gonna do to replace that missing tooth? maybe a filpper/parital or something, if they do, get this, and I hope you get it approved thur Ryan White, so good
luck with that, sometimes it takes a bout a month or so, but don't give up, soon you'll have a HOLLYWOOD

Well, at least I have radiant smile, and no teeth missing  :-* missing teeth is not hot, and not sexy at all  :-*
it doesn't matter if they are dentures, partials are even crowns, and when I open my month I still have them
,that is all that really matters to me, so I'm  truly grateful for this  :D

@thunter34... and @ Dachsund  :) the reasons why you think I have a lower partial isn't the reason you think it is ,when I was a child, I suffered for childhood gum-disease, so before the age of 13 I had a lower -partial, so you see, I've always had a partial all of my life, I'm on my 5th one @ 55 yr old  ;)
