Hi guys,

Sanitex, I can comment, as I am currently under treatment for both these maladies.  I am nearing the end of the treatment for active TB, which though it caused no lung problems in my case, did effect the lymph nodes and caused me to lose about 40 pounds in a few short months.  Now I have gained about 20-25 pounds back (I don't want to gain back all forty, I was chubby!)  and I feel great - I felt all better within 1 week of beginning TB meds, appetite and energy returned, etc.

I delayed starting ARV meds until several months along in the TB treatment because when I had tried ARV before while suffering from undiagnosed TB, I had terrible side effects (apparently due to the TB, not the ARV), but now am doing very well on Sustiva/Truvada as well, at least in the sense of few side effects (dizziness at night after ingestion of meds being the only important one).  I never noticed any change in my overall 'health' or feeling of vigour or fitness related to beginning the ARV though - no benefit or detriment.

I'm wondering if your weight loss is from your TB.. are they quite sure it is 'inactive' TB?  Weight loss is one of the primary symptoms, and I do believe that the treatment is different for latent compared to active TB - the active case requires the full four drug combo and a long treatment term.. I think the inactive in some case may be only one or two drugs, so I suppose it could be dangerous to assume it is an 'inactive' case, as resistance issues to TB drugs are so important..  If your TB becomes resistant, in the case of an HIV positive person, the prognosis is not good.

I'm curious how they diagnosed your TB as in my case it was extremely difficult for the doctors to diagnose and they weren't really sure about it until recently (I think once the treatment was so immediately effective it confirmed their guess).  I think they automatically assumed that if it were TB, it was 'active' simply because of the weight loss, regardless of the lack of any effect on the lungs.  I don't mean to second guess your doctors, who are presumably well qualified, but I have been told by HIV docs that not all pulmonary docs - who specialize in TB, etc. - are fully versed in the special requirements and dangers faced by HIV positive people.  Generally my hiv docs have recommended that in the case of TB, HIVers 'err on the side of caution' - for example extending treatment for active TB to the full nine months course rather than the 'short' six month course that is recommended by most TB docs.

Yes, that four drug combo sounds right for active TB as well.  Great! (though I didn't understand most of your post)
