Most recent labs
« on: October 19, 2017, 08:26:45 pm »
Hello everyone, hope you all are  doing well
Had labs on on 9-27, here are my results:
% CD4 (HELPER CELLS)     35
ABSOLUTE CD4+ CELLS     496
% CD8 (SUPPRESSOR T CELLS)  39
ABSOLUTE CD8+ CELLS   543
HELPER/SUPPRESSOR RATIO  0.91
ABSOLUTE LYMPHOCYTES  1406
I got my results from a different lab report than the one my doctor gives me and on my Docs report always says I have been undetectable and this is what this report says??:
HIV 1 RNA, QN PCR  <20 DETECTED
HIV 1 RNA, QN PCR  <1.30 DETECTED
This patient had an HIV-1 RNA viral load below the lower
Limit of Quantitation for this assay (20 copies/mL), but
above the lower Limit of Detection for the test. Because
of the low viral load, we were unable to determine the
actual level of the virus in this patient's sample.
Repeat testing may be warranted, when clinically
indicated.

In different parts of the world there are different tests and measures of viral load. Because of expense and infrastructure.

Undetectable does not mean there is no viral load, simply than there is less than the lowest reliable limit of the test used.

20 copies per ml is the limit of quantification (countability, often referred to as detectability) used by the laboratory that was used this time (it's also the pretty much a gold standard). "<" means "less than". So your test result says that you have less than 20 copies, ie less than the threshold of being able to be reliably counted.

Your result for VL would be reported as either:
HIV 1 RNA, QN PCR  <20 DETECTED
    or
HIV 1 RNA, QN PCR  <20 NOT DETECTED

The difference between the two is that if the assay detected any viral particles (but less than the 20 that is required based on the assay's sensitivity), you get the first result.  If it does NOT detect any viral particles at all, you get the second.

The reality is that both are considered "Undetectable", even if not wholly true.

Your results are good, so there is no need for worry.  This confusion is a great example why it's important to work with your doctor to interpret lab reports.
