Welcome to the "Do I Have HIV?" POZ forum.

This special section of the POZ forum is for individuals who have concerns about whether or not they are HIV positive. Individuals are permitted to post up to three questions or responses in this forum.

Ongoing participation in the "Do I Have HIV?" forum (posting more than three questions or responses) requires a paid subscription, with secure payments made via PayPal.

A seven-day subscription is $9.99, a 30-day subscription is $14.99 and a 90-day subscription is $24.99.

Real Risk

My bf of three years is positive. He is not on meds. We were having vaginal sex last night, protected of course. We finished, but we kept fooling around. He then put on another condom and we started again. Must have been having sex for less than 20 seconds when he noticed the condom broke. He doesnt know if he pulled out right away when he felt it pop. Chances are he most likely did. He thinks he did, but isnt sure. He noticed it because we were going to change position. I'm still in the time frame to seek out help.What should I do? Is PEP something I should seek as soon as possible?

Hi Bettie,

Yes, that was a risk but what I consider to be a relatively low level one. It was a single incident and from what you have described a brief one. And you were protected until the condom broke.

Figuring into what you decide I'll also mention that HIV is easier to transmit from male to female than the other way around. 

You are still within the window period for PEP. It needs to be started within 48-72 hours after a risk, and the earlier the better. You can go to your doctor or any emergency room where they can get you started.

Is your bf's health being monitored? Do you know his viral load numbers? If he has very low or undetectable VL, that makes it less of a risk.

Thank you for the reply. If he immediately withdrew his penis right after the condom broke, would you still recommend me to seek out PEP?

It's certainly what I call a very low level risk. A brief insertion, condom breaks and no ejaculation. Yah, low level risk.

But the decision has to be yours of course.

Bettie,

I agree with Andy that this is a very low level risk. It sounds as though he realised right away - and importantly, he didn't ejaculate in you. It's up to you whether or not you take PEP - but I really don't think it's warranted in this situation.

Keep in mind that if you do take it, you will be taking some very powerful meds. It will also extend your testing window period by one month for a conclusive negative result.

Regardless of what action you take in this instance, I would recommend you both sit down with his doctor and discuss what should be done if something like this happens in future. Your boyfriend may also consider going on meds, as this will greatly reduce his ability to transmit the virus.
