Stribild - Here Goes...

I have been reading this forum on and off the last few weeks and want to say that I appreciate the information that I have learned here and thought I would share my story.  I have been positive since January of 2001.  For the first few years after I was stupid and avoided the doctor.  Luckily I never got sick.  I finally found the courage to go to the doctor in June of 2010 and my Tcells were still in the 600's and my viral load was 25,000.  We decided to postpone treatment and monitor the situation.  Well I never went back to the doctor and then this past June I started feeling really crappy and developed a bad cough.  I went to the doctor and they diagnosed me with thrush in my throat which was causing the cough.  They ran bloodwork and my Tcells were 140 with a viral load of 358,000.  Oddly enough, I was grateful it wasn't worse.  Luckily no other issues except for slightly elevated liver enzymes that we are monitoring but there was no hepatitis.  The doctor thinks I might have a bit of a fatty liver due to my diet and we are going to watch it closely.  I am making a huge attempt to eat better (fast food lover here) and drink more water.  Anyway, I was prescribed Fluconazole for the thrush which cleared it up in a couple of days and totally eliminated my cough.  I was also prescribed Bactrim as a preventative for anything bacteria including pneumonia.  I am still taking both of those medicines once a day until my Tcells get above 200.

I am seeing an HIV specialist and my regular doctor as they tell me.  I made the decision to do whatever they ask no matter how worried or scared I get.  It was recommended of course that I start treatment right away and after geno-typing my virus my options for first line treatment were Atripla or Stribild.  I was told that Atripla can cause extreme dreams and can enhance depression.  I am not depressed overall but have my bad days so I elected to go with Stribild.  Luckily my insurance company covers it and then I signed up for the co-pay assistance so I pay nothing for it - which is nice.  Well I do pay for it through my insurance premiums but it feels "free" to me.  So I just took my first Stribild about an hour ago.  I have been slightly disappointed that my head hasn't exploded as I was half expecting it too within minutes of taking the pill.  I was so nervous and felt a bit silly about how nervous I was.  I took it right after I ate lunch as it is advised to take with food.  So we will see.  So far so good.  My stomach is a little flip-floppy but I can't tell if it is the medicine or my nerves.  Plus the Bactrim makes my stomach a little quesy every now and then so as long as it doesn't get any worse than this I will consider myself lucky.  I hope to update this thread every day for the next several days and hope that I have nothing of interest to say - which would be the first time I have ever hoped for that.  :)

Well I just took my 2nd dose of Stribild this afternoon.  I kept waiting for something horrible to happen last night and nothing did.  I had some very mild feelings of nausea about 5 hours after taking the first does but nothing that I can even complain about and it only lasted about an hour-ish.  I got worse nausea with just the Bactrim I have been taking.  This morning I had some mild diarrhea (nice I know) but it was only once and nothing major at all.  Again, I have that sporadically with the Bactrim so I can't even blame anything on the Stribild.  Overall I am very happy.  I know it is only day two but I will keep on this journey just as so many of you are on.  I am not sure if Stribild takes awhile to build up in your system but if this is all I experience I will consider myself so very lucky.

Hey Slight, glad to hear you have finally taken the steps on the road towards recovery and management.  I was recently diagnosed and immediately started Stribild a month ago.  Like you, I was very nervous when I took my first pill (I take mine at 10pm every night) and was waiting for something dramatic to happen...which luckily never did.  Like you I had some really noticeable nausea (a very strange sense of it too...very different than anything I've felt before...it's not as overwhelming as say a hangover but at the same time very debilitating).  The intensity of the nausea went away after one or two weeks...some mornings I still wake up feeling a bit crappy but I'll take what I can get...it could definitely be worse.  Like you, I have had issues with the diarrhea, which for me comes shortly after waking up...but then is good after I get everything out of my system.

Now for some words of encouragement:
I just got my one month labs back today and to say the least, I was stunned.  I started with a VL of 95k and CD4's at 468 (31%).  My doc pulls up my results and my jaw drops as I see a VL of 29 (yes...29...period) and CD4's at 700 something and still at 31%. Stribild can work wonders...that said I have been keeping extremely strict with my adherence.  I have a notifications set up on my calendar that go off at 9:55 every day and every day I eat something and take my pill and have always taken it within the 9:30pm-10:30pm timeframe (it's actually quite funny how my appetite has adjusted to actually start being hungry in anticipation for my pre-pill snack).

Anyways, just keep plugging along and you would be amazed at what these pills can do.

I used to take Atripla, have been since becoming POZ 4 years ago.  I switched to Stribild back in December 2012, and honestly I'm thankful for it.  No more feeling like I'm drunk after an hour of taking the pill (like Atripla).  I don't have to worry about getting to a bed after taking Atripla.  So far no bad side effects that I've seen.  Which is why I switched in the 1st place.  Good luck!

Hi Kevin & everyone that commented on this thread.  I would love to hear other people's experiences regarding Stribild.  I know it is not only a year old yet but any information and experiences are good to hear.

Today marks my 8th day on Stribild.  So far I haven't gotten any "real" side effects.  I say it like that because I have had some minor upset stomachs and this afternoon I was nauseated.  Today was probably the worst issue I have had but even then it was relatively minor.  Plus I didn't get nauseated or have an upset stomach today until after I had chicken tequila pasta for lunch and I think it just disagreed with me.  Now I feel fine.  Plus I am still on Bactrim and Fluconazole so I am sure some of my issues are from that.  Anyway, I couldn't be happier with the outcome so far

I started taking Stribild last Friday at 2:30pm then realized that time wasn't going to work for me every day.  So I have pushed it back an hour for a couple of days and now take it at 6:30pm every night with an early dinner.  I can't say I look forward to taking it but I don't dread it any more like I did the first couple of days.

Hi Slightlyremoved,

I started Striblid on July 3rd & I take it around 7 am everyday, and I am very happy to report there are no major side effects for me. Although I was the one pushing to start meds to begin with I did experience a lot of anxiety in regards to possible side effects. My only side effect has been bloating and that was extreme in the beginning but has lessen after a couple of weeks, now it may last an hour or two after taking my pill.

I will say my initial vision/expectations were tainted by nightmare experiences I witnessed of my friend who has been on meds for a couple of decades.

I can say I only can see good things for us! We are blessed to have such great options with these news meds! Best Wishes  :D

I take Stribild at 10pm every night, which is when I took my Truvada/Norvir/Prezista before I switched.  I had some dizziness when I woke up for the first few days, but since then I haven't noticed anything different at all. 

The only thing I'm concerned about is the effects it'll have on my kidneys.  I have been doing clinical at an HIV clinic for school and I'm astounded at the amount of people that have renal disease requiring dialysis.

I am on stribild 2 weeks now.  The fist day after the first pill was a little nauseating, then that symptom went away.  I am hoping for good numbers at my next checkup. I have a long way to go. 

I definitely have been having different and vivid dreams with stribild. Last night I found my first car under a pile of leaves and was still able to drive it. The dream was so vivid. Does anyone else experience this?

Hey all...

I'm brand new to this site entirely but wanted to find somewhere I could get some support for what I'm going through.  I started taking Stribild about a month ago and the side effects I'm experiencing are definitely the nausea (I throw up nearly every morning when I wake up) and diarrhea (not nearly as often).  Due to the nausea, I'm running at around 119 lbs. at 6'0" tall.  My partner is HIV- and has a great body :) but we eat the same foods and while I feel better than I have in awhile, I still feel frail and weak.  Additionally, I have noticed what I can only call "spider bites" all over my body which have begun showing up after I started taking Stribild.  Currently, I'm out of town for work and have a doctor's appointment for when I get back but these bites are driving me CRAZY :o :o

I hope someone else has the same weird symptom and may have some sort of suggestion which has worked for them I would be so thankful!!

The nausea only lasted a while for me, I found that eating a substantial amount of food with the pill makes a huge difference, that and when i wake up, just FORCING myself to eat usually got rid of any remaining nausea.  As far as the "spider bites"...I haven't heard anything similar to that on these forums...would def talk to your doctor about it though.

Edit* I take my medication at 10PM every night...I don't eat nearly as much as I used to (maybe a granola bar or a cookie) and I have zero side effects...maybe I'm just lucky?

Hi there Slight,

I am on my 4th day of stribild. I too experienced the anxiety of taking it.. But so far so good. I'm getting intense headaches but I think that may be coming from my anxiety issues (as in I'm being medicated for it) and I don't think I'm getting enough nutrients due to loss of appetite. But overall I'm impressed with it. But like you, I'm enjoying reading everyone's experiences with it. Good luck!

I started working night shift recently, but I still take the Stribild at 10pm every night.  I noticed that I have some brief episodes of dizziness at work a few hours after taking.

I'm wondering if I should switch to taking it before going to sleep, but I'm not sure how without "missing" a dose.  Instead of taking it at 10pm, I'd wait until I got home from work and take it around 9am and go to sleep.

Do I wait the extra 11 hours before taking the dose or take two doses within 12 hours of each other?

I personally would wait the extra 11 hours.  The goal for viral suppression is 95% adherence to medication.  So if you miss one dose, you're not going to build resistance, or risk viral replication.   

Of course this is just my personal opinion, I'm sure you'd be fine either way.

When I've needed to move the time, which has happened a lot in my now 9 months on Stribild (mostly to keep it as far away from other drugs as possible), I've done it in 2 hour increments per day, so it takes a few days to get it to the new time. It's tedious, though, so BT probably has the right idea - like ripping off a Band-Aid(tm) brand adhesive bandage.

When I spoke to my ID about switching my dose of Stirbild from the morning to the afternoon (from 9AM to 3PM) he told me just to start taking at the new time and there was no need to gradually move the time of day in which I took my dose.

Oh, and for what it's worth, I used to get bizarre dizziness (up and down and left and right played musical chairs) sometimes after I took Stribild during the first month or two, but it never happens anymore. Hopefully yours will pass too even if you decide not to move the dose time.

I have been with stribild for nine month and no side effects so far. Best cd4 count ever  :)

I'm switching from Atripla to Stribild next week...as soon as the pills come in the mail. Great feedback....I was very reluctant to switch since Atripla got the job done. I am looking more forward to this now!

I notice some have/are switching to Stribild from other regimens.  When I was going over the pamphlet for both Stribild and Complera, they both say they are for those who have never taken HIV meds before. 

What's the deal with that?  I was going to ask whether my short course of Sustiva in the hospital would apply to this, but it seems many are switching to this, who are not treatment naive.

I was on HIV meds since almost the beginning of being diagnosed, over 20 years ago, and I've been on Stribild for a few months now.  Not just for treatment naïve.

I've also switched from complera to Stribild. It's so much more better for me. I don't have any issues with bloating and gas like I had with complera that's the only reason I switched. I haven't had my blood work for CD4 OR VL since starting Stribild in June.

Ive also noticed it says for treatment naive patients but doc said it was fine and I trust her so I left it alone.

Not sure why the pamphlets say that when doctors are prescribing it for treatment experienced.  I'll have to ask my doctor next time I see him.

My doctors would not prescribe Stribild for me because I carried a high viral load at one point in my infection . I think Stribild has a higher treatment failure risk for people with a history of high viral loads .

Our forum page on HIV drugs had this to say ... Stribild was developed by Gilead Sciences. It was approved by the U.S. Food and Drug Administration on August 27, 2012 for people starting HIV treatment for the first time.

Some one will correct me if I'm wrong about this but this is why my doctors would not prescribe it for me . I am decent at the prevention and risk assessment end of HIV but keeping up with the science behind the meds has always done my head in and I defer to Ann or Newt about such things .

Am I confusing Stribild for another drug that isn't indicated for those that had a high viral load in the past ? I was almost certain that was the reason they gave me but I also concede I shouldn't way in if I have the slightest doubt .

You're thinking of Edurant, which is part of Complera: Compared with patients who took the NNRTI Sustiva (efavirenz) plus Truvada (tenofovir/emtricitabine)—frequently used together as Atripla—in clinical trials, people with high pre-treatment viral loads (greater than 100,000 copies) who used Edurant plus Truvada were more likely to see their viral loads rebound while on treatment. Patients using Edurant plus Truvada were also more likely to develop drug resistance to other NNRTIs, including the second-line drug Intelence (etravirine), and/or Truvada, compared with those using Sustiva plus Truvada.

I was on reyataz, norvir and truvada for 3 years before stribild and since i switch my cd count have been the higher ever

12/08 start treatment ( norvir, truvada,reyataz)
07/12 und/cd 575/42%
12/12 start Stribild

I was on Stribild for 4 months and while its very powerful (made me undetectable in a month), I did not see impressive CD4 increase on it. Your CD4 does not depend on the drug combo so much than on what you eat/exercise.

Because of the booster Cobicistat, Stribuild has wide potential of interactions. I remember after working out it was making me feeling high for a few hours. Stribuild also made me more talkative.

On the other hand, I could not fall asleep AT ALL on Stribild, even when taking half the pill.

So my advice to you people is "if it's not broken, don't fix it". If your combo works, no need to jump on the latest hiv drug bandwagon. I know it's tempting but you may encounter new side effects if you do.
