Disulfiram / Waking the Reservoir

So I went to UC San Francisco / SF Gen Hospital today to participate in a study.   They wanted about a cup of blood to do reservoir research.  I asked about other things that were happening in the field, including disulfiram (Dr. Deeks at UCSF is working on that).  I was told that it worked somewhat, if you take an average of all the study participants.  But for some, it had a significantly greater effect in purging HIV from the reservoirs.

It turns out that the level of disulfiram in the blood determines how effective it is and that this level can vary greatly from individual to individual.  So, what causes this variation?  They aren't yet sure and this is going to be the focus of the next trial.

That's all I know... thought it was interesting and encouraging so I'm posting it.

How do they research reservoir in blood? What study did you go for? I always imagined research in regards to the latent cells to be tissue based since the virus producing cells arent free flowing in the blood. Or are they?

Hey C_G,

The blood they took was to be sent to Hopkins -- Dr. Siliciano there has an assay that measures how effective drugs are getting HIV expressed from memory T-Cells.  There are other tests as well and this study is to test the efficacy of his test vs. others.

Living in SF I guess there must be quite some opportunities to join trials. I would love to be a part of some trials even if it came with some risk, problem is since I am not a citizen here I cant join any trials.  :-\

I agree with you CG,  there are a lot of trials in SF and LA (at least the interesting one's). 

My clinic runs a lot of trials but they are mostly HIV related cancer studies as my clinic is a research site for the AIDS malignancy consortium and I would rather hope Not to get cancer.

If this drug like the other cancer drug can force the HIV out of its hiding place then what is stopping them from combining it with antiviral drugs to probably achieve a cure? It's so annoying reading about all these possible cures and not seeing any aggressive effort being made to try them. If I had access to a doctor that can give me prescription for these drugs I will pop the pills and damn the consequences, good or bad.

It's weird that many ''waking'' drugs are effective against parasites.
Disulfiram is active against protozoa, and another drug which ''wakes'' the reservoir (miltefosine) is a drug used in therapy of leishmaniasis.
