Switching - Stocrin to Viramune

All,

my ID has suggested switching me from stocrin to viramune due to my sleep disturbances... I was wondering if any of you could give me your insights...

I'm concerned about the following

1. Viramune is not as effective... I keep reading that it has to be twice a day but my ID says i can take it once as my vl is under 50 copies for 6 months... basically stocrin has done all the work.. I'm not a morning person and quite often forget to take my vitamins first thing of the morning
2. resistance due to the switch
3. how long can i stay on viramune if i am adherant.... So far i've never missed a dose of stocrin

Hey Parteboy,

I had to switch from Sustiva (Stocrin) because it really did a number on me. I have never taken Viramune. I instead went back to a PI-based regimen.

Viramune is an effective medication and one which passes easily into the brain, a definite plus.

I will defer the dosing to your doctor's expertise, since he/she is, after all, a doctor. Twice a day isn't that tough to do. One way is to use a pill box. It really helps me.

An across the board switch should not lead to any resistance, especially since both Stocrin and Viramune are non-nukes and you have not shown any resistance to that class of meds.

You could stay on Viramune for years as long as you don't develop resistance. I stayed on one regimen for 11 years and only switched when long-term side effects started showing up.

These are generalities, of course. Nobody can predict how you may react to any of the meds, or how long they may work for you.

Oh, one thing. Since you aren't a morning person, has your doc ever mentioned taking a once-a-day regimen such as Truvada and boosted Reyataz. I know this is a totally different class of drug, but it may be something to think about if the thought of taking Viramune really doesn't set well with you.

Viramune is just as effective as Efavirenz, and without the psychiatric side effects.

I mean how undetectable do you wanna be?  (joke meant for reassurance purposes only).

Lots of people take Viramune once a day without any problems.

Check with your doc.

Viramune is as effective as Stocrin without the mindfuck.  I have friends who have been on it faithfully in combination with other meds for over 8 years while maintaining an undetectable viral load.

Resistance arises naturally when people take medications to fight off a virus that is sloppy about copying itself.  As long as there is virus aline, virus is being produced with little imperfections, and eventually one or more of those imperfections provides resistance to the drug.  "Perfect" virii get killed off, but the copies with the defect live and reproduce.  Your chances of developing resistance to NNRTIs should be no worse on Viramune.

Hi all (or should I say "g'day cobbers").  I've just been introduced to this site and posted a message asking about the Stocrin(Sustiva) / Viramune comparison.  My doctor(s) want to start me on Stocrin + Kivexa, and I'm very worried about the side-effects of Stocrin. Everything I've read so far seems to steer me away from Efavirenz and towards Nevirapine - unless there's some new information to the contrary (?)

I'd really appreciate it if anyone with personal  experience and up-to-date information could reply to my posting. I found this conversation pretty reassuring.

I'm taking Viramune, and love it.  Was very leary about taking Sustiva due to psychiatric side effects.  I've had absolutely no side effects on Viramune, and it seems to be doing a good job with the virus.  Having to take a pill twice a day is no problem for me.

Well,

After spending a year on stocrin I've finally switched to viramune... PTL (Praise the Lord) why didn't i do this before???? DOH....... After 12 months of living with the sleepless nights I can finally sleep..... OMG... I have to also say i can actually think without feeling like my brain is in a frying pan.....

I've had a few side effects from viramune, nausea, and a few mouth ulcers but I can live with that.... The nausea has just about gone... Its strange its worse in the afternoon than the morning... And I have to pee at 7:00am exactly every morning.... What a great alarm clock......

I just got my LFT results back and they are normal which is great.... The hardest thing is that its spring in oz at the moment so i can't tell if its a side effect or my hay fever playing up.... So i do panic a little....

I just got my latest blood results back and my CD4 are 750 and vl undetectable after 12 months on stocrin... Yippee

I am glad to hear that things are going much better for you on Viramune. In another 45 days or so, it will be 5 years since I was started on Viramune, as part of my first regemin, And I really didn't have that much of a problem.

It's great that your able to get a good nights sleep again !

 Tested positive in 1985,.. In October of 2003, My t-cell count was 16, Viral load was over 500,000, Percentage at that time was 5%. I started on  HAART on October 24th, 2003.
