Anal cancer risk increases significantly in first year of HIV infection

This information states that a persons risk for anal cancer increases by up to 70% in the first year of HIV infection.

32% develop high grade neoplasia an 35% develop low grade anal neoplasia in the first year after being infected.  Article goes on to state "intervention with early treatment and possible vaccination may be able to prevent the future development of anal cancer."

Have man people received the hpv vaccine? I asked my dr's nurse about it and she said no.  I know there are other doctors that recommend it.  I have a high rez anoscopy scheduled, this is done every year at my clinic.   I kinda want the vaccine too.

Most sexually active adults, especially gay males, already have the virus. You should be screened for it for information's sake however.

I had the three shots about 3 years ago, before my HIV diagnosis. My parent's insurances covered it. The doctor said he was suggesting it to his gay patients. I live in Montreal, so maybe the guidelines are/were different.

In order to have a risk for HPV one must be infected with it. Unlike other illnesses, HPV does tend to isolate in the areas of infection - genital warts on the penis versus anal warts, for example. I imagine straight males without any anal experience might be more at risk for certain penile cancers, though I have not found scientific corroboration.

HPV is spread via skin-to-skin contact, which is why condoms are not considered particularly useful in HPV prevention.  Straight men, women, and gay men who are not anally receptive can still get anal HPV infection from the insertion of a finger, etc.  Women in particular are subject to something called transmigration, in which HPV can spread from the vagina/cervix to the anus without penile insertion into the anus.

I am not getting that article nor this thread.  According to that article we should all be dropping dead with anal cancer by now, years after diagnosis. In that article everything is 2x 3x 4x 5x normal risk.

It is really quite simple -- the normal risk for anal cancer is extremely small.  So, even 5x a very small number will still give you a relatively small number.

While I do get my ass examined every year, I spend very little time worrying about anal cancer, as there are a whole host of other things that are more likely to be my undoing.

Agreed. There's not much reason to worry about this issue IF you're having the proper area looked at annually. Of course, having been on this board for five years I know most HIV patients don't do this, and/or don't even understand the process, nor have an HIV specialist that is pro-actively discussing and explaining it to them.

Yes, that's because we're taught from an early age to wipe front-to-back after using the toilet, for obvious reasons. It also means that we're likely to spread genital herpes to the anal area. Ouch.

Straight men with no experience of anal/penile penetration are mcuh less likely to have hpv anally, unless they've indulged in a bit of finger play.
