Undergoing surgery on meds????

As many of you who have read my previous posts know, I'm a transguy (ftm transsexual). Well, today I had the great news of scheduling a date for my hysterectomy! The issue I have is that when I first met with the surgeon to discuss it, I wasn't on meds yet, but recently started HAART. I'm wondering if this will have any effect on my ability to undergo surgery? Has anyone here undergone surgery while on HAART. Just for reference, my regimen is Truvada/Reyataz/Norvir all once a day with food. I just started less than one week ago. I really hope this won't stop me from having the operation, because I've been wanting this done since I was a teenager! Any advice, thoughts, experiences, and/or knowledge would be greatly appreciated!!!

Hi Odyssey,

Your meds should not stop your surgery. However, it is imperative that both the doctor/surgeon and - importantly - the anesthesiologist know what meds you're on so they can avoid giving you any additional medications that may have interactions with what you're taking.

You may also want to take your own supply of your meds into hospital with you. Although I'm not on meds myself, I have heard stories from others who have been in hospital and had a difficult time 1. obtaining their meds at the correct time and 2. obtaining the meds at all. Not all hospitals will stock all hiv meds, and nurses are often too busy to make sure you've got your meds when you normally take them.

Good luck with the surgery! I bet you're really excited to be finally having this done. Hope it all goes well and you recover quickly.

Thanks for the well wishes. I'm absolutely thrilled that this day is finallly coming! I'll definitely take your advice and bring my meds with me to the hospital, as well as be sure everyone involved knows what I'm on to avoid interactions. Thanks!

Congratulations on moving towards your goal!

As for meds... be sure the doctors are aware of the Norvir.  Norvir and Reyataz both block certain liver enzymes.  I have been on both of them for a little over 15 months.  Norvir is normally a booster because it slows the breakdown of other meds.  If they give you something that is metabolized by your liver they may need to adjust the dose. 

For your own safety make sure that everyone is aware of your current meds.  It's also not a bad idea to specifically ask your pharmacist to check for drug interactions.  It is the pharmacist's job to check these things and they are normally happy to do so.
Don't obsess over the wrong things.  Life isn't about your numbers, it isn't about this forum, it isn't about someone's opinion.  It's about getting out there and enjoying it.   I am a person with HIV - not the other way around.

Thanks hotpuppy! Somehow I have a feeling I'm going to be telling about a milllion and one people what meds I take just to get through this operation, lol. But yeah, I'm going to be extra cautious, don't want to have anything horrible happen to me or whatever. I think I'll make photocopies of my med list and hand them to every single person who asks, that way they can be sure to know what I really said.?? Good idea maybe? Who knows?

Congratulations!

Nothing wrong at all with being proactive about your care.  My default response is typically "Make copies of your medication list with dosing/dosage information and make sure your surgeon and treating physicians see what medications you are on.  Discuss all medical issues with your physicians and nurses."  I think that's fairly good advice about any medical procedure--as well as medical concern you might have.  We may be able to tell you anecdotal information about our experieinces and some of us my have a bit of information here and there--some of us are just wing nuts :)  --but have the discussions with medical professionals please.

Well, my surgery date got postponed until December 14th, so now I have an entire two more weeks to be a nervous wreck about it. Hopefully they won't cancel it altogether after I overdosed on muscle relaxantants last week because I was so blantantly terrified and overwhelmed with feelings. I know I'm ready for this surgery, its just this whole HIV thing stilll has me feeling  a lot of strong stuff and sometimes I don't know what to do with those feelings. Uggghh, being poz sucks. I'm sooo angry this happened to me. My life was finallly achieving a state of balance and then this whole HIV thing happened. Oh well, at least this surgery will help me feel more of the man I know  I am.

Well, my surgery is a little more than a week away, and I'm soo excited to finally be having this surgery. Emotionally, I'm in a much better state. My boyfriend and I broke up, but we're still talking, so I guess we're just friends for now. I was wondering if anyone had any tips dealling with the excitement/anxiety going into this final week before surgery, which is a week from Monday?

I know Norvir can cause problems with some drugs--like versed.  They usually give ya that when they knock ya out for surgeries, colonoscopies, etc.

This Monday I spoke with a preop nurse and we reviewed the extensive list of medications that I take. I also used the computer to make a list of the drugs I take to make a list and print it out to bring with me to show every doctor/nurse/etc I see on the big day. At this point I'm fairly confident they'll know what they'll be doing medication wise and won't be dumb enough to give me something that has a known interaction after we've discussed my meds about half a dozen times. hopefully, that is. I still can't believe its getting so close. I'm so nervous, yet excited. This is something I've wanted for almost ten years, and to finally have it done, just, wow. EEEk! Does anyone else get what I'm feeling or I'm a bit nuts?

In the link from madbrain the doctor recommends to "get the viral load under control," which I assume means undetectable. Since you started recently, do you know what your viral load is?

The only other thing I'd mention is that there's one specific med used in anesthesia that isn't recommended when on certain HIV meds, there was a thread about it recently. Maybe not all anesthesiologists are aware of it. I think it was with Sustiva, though, so it might not affect you. I don't remember the specifics, maybe someone else knows? Doesn't hurt to double check.

I'm pretty sure its midazolam, common name Versed, that can interact. And I would hope any anesthesiologist worth his salt would at least bother to look up any potential interactions before he or she just goes injecting meds all hokey pokey. That's what they're paid for after all, to keep us asleep but alive! And thanks for the well wishes, they are very much appreciated!

Yeah, that's it . . . the reason I mentioned that maybe not all anesthesiologists know about it is because in a recent thread someone said that he had to bring it to the attention of those doing a colonoscopy on him because they weren't aware of the contraindication.

I'll make sure to say: "Are you aware of any potential interactions, such as that with midazolam?" to the anesthesiologist before the procedure. Hopefully that'll trigger them to either tell me which ones they know that they already found or to go look them up. Anesthesiologists are the most highly paid specialists of all doctors, and I doubt its because their job is easy. But again, thanks for the well wishes!

I just wanted to let everyone know that my surgery went off monday without  a hitch, I stayed overnight until this evening, when I came home. I'm feeling pretty welll right now, nothing too sore or anything wild. So far, I'm really glad I had this done! Thanks for everyone who thought about and/or prayed for the success of my operation.
