Are the odds against me?

So, I'm a 26 y/o male and about 5 days ago I hooked up with a guy i met through tinder. We kissed for a while and subsequently had unprotected sex. I bottomed for him and it was pretty rough, the pain was almost unbearable. It was the first time I've ever bottomed or had unprotected sex. I started freaking out as soon as I realized my mistake and asked him if he was HIV negative. He reassured me that he was and also said he's on Prep, on a 2-1-1 schedule, and so I shouldn't worry as chances of an infection are "zero". The fact that he is a med student did help to calm me down a bit, although he is clearly a lot more sexually active than me.

Two days after my encounter I started feeling intense pain in the lower part of my anus. The pain got so strong that I could barely walk. Defecating is absolutely excruciating. The pain has been persistent and shows no signs of improvement.

On the fourth day after my encounter, I started having a mild fever, and so became completely paranoid about the possibility of it being HIV. I have OCD, severe social anxiety and Asperger's and am having an extremely hard time being functional or even sleeping. I've been laying in my bed for days, completely devastated and horrified about what I have done.

Yes, I know what I did was extremely stupid. I'd just like some input on how big of a threat this really is. How likely is it that my fever is related to ARS? I also have a mildly sore throat. How high would you say the risks are for me?

Your left testicle could have dropped off and it would have nothing to do with HIV from an encounter less than four days ago.

Totally irrelevant. I could claim to be a flying elephant but it doesn't make it true and even if they tested today or used some prep all it means is they didn't have HIV some months ago, hence totally irrelevant to you.

How high would you say the risks are for me?

I can't calculate your odds and anyone who claims they can is full of shit but it's about 1 - 25 %  presuming as we should that the person you had sex with are living with untreated HIV.

End of the day, you had condomless intercourse, and it's an HIV risk and warrants testing to confirm your HIV status.

Test at six weeks post your last potential exposure with a blood-draw (lab) HIV antibody test. The result at this time will rarely change, and retesting at three months generally isn't needed.

Here's what you need to know to reduce your HIV risks:
Use condoms for anal or vaginal intercourse correctly and consistently, with no exceptions. Consider talking to your healthcare provider about PrEP as an additional layer of protection against HIV

Keep in mind that some sexual practices described as safe in terms of acquiring HIV still pose a risk for other easier-acquired STIs. So please do get tested at least yearly for STIs, including but not limited to HIV, and more frequently if condomless intercourse occurs.

Also, note that it is possible to have an STI and show no signs or symptoms; testing is the only way to know.
