Welcome to the "Do I Have HIV?" POZ forum.

This special section of the POZ forum is for individuals who have concerns about whether or not they are HIV positive. Individuals are permitted to post up to three questions or responses in this forum.

Ongoing participation in the "Do I Have HIV?" forum (posting more than three questions or responses) requires a paid subscription, with secure payments made via PayPal.

Question to those on this forum,

have you or anyone you known gotten HIV from only precum?
As in, you never let anyone cum inside you and you still got it?

Any information you have would be amazing because I am sort of freaking out.

Well, there seems to be an outlier population that might have gotten HIV from precum. It's not been thoroughly quantified, and I always assert that the plural of anecdote is not data.

Might I ask what, specifically, was your risk? We do risk assessment here, based on science.

i was the bottom in unprotected sex with someone with HIV two or three times
(used a condom the other times, very stupid in the heated moment decision that i hugely regret, and can't stop thinking about).

there was no blood involved, and he never came inside me.

i know you can still get it through tears in your anal tissue though, but I wasn't sure the risk of just precum.


it hasn't been a month yet so it's useless to get tested so i was just wondering. really curious to know if anyone here never let someone cum in them (or shared needles etc) and still got it.

Well, unprotected receptive anal sex is one of the highest risks. Of course, a great deal depends on whether or not the insertive partner was on meds and had an undetectable viral load. Though having an undetectable viral load as measured in the blood is not always an accurate indicator of seminal viral load.

In addition, if your partner had any other STDs, the risk of a spike in viral load is more significant. Same goes for you and STDs, as inflammation in the rectal area can lead to a greater risk of tearing and a higher risk. If the insertive partner WAS on meds and had an undetectable viral load in the blood, then the risk is rather low. Not zero, but low.

I guess some additional details to answer your questions:

He is otherwise STD free,
I am STD free,
his viral load is low but not undetectable, he isn't on meds yet and only recently diagnosed but past the period where the viral load spikes. (he had an appt the monday before we had sex and this is what the nurse told him)


I understand my actions were high risk but I'm still curious about precum... I haven't found any firsthand accounts of becoming infected by precum, but it's obviously possible. Thanks a ton for your reply, anything additional you could talk to me about would be incredible...

Possible, certainly. But not necessarily probable. I have yet to read confirmed documentation outside patient report (which is notoriously unreliable) which makes such a claim. Sadly, I have also yet to find serodiscordant couples studies to confirm or deny this.

I am not even sure such a study would be ethical.

i certainly wish I could be more precise, but the data seems murky on the topic.

Sadly, very few studies exist post-HAART that follow gay male serodiscordant couples. Hopefully in the future this will change. But at the rather dizzying speed at which we find ourselves barreling towards a functional cure (treatment as prevention, PrEP, sustained undetectable viral loads through long-term treatment options, et al) I rather suspect we will close the barn door long after these horses have returned to the stable.

I certainly wish more research had been done on the population which, in the western world, continues to lead in HIV cases. Sadly, HIV science inexplicably omits long term studies of serodiscordant homosexual male couples - even as major funding sources prevent the existence of them.

Trust me, it's frustrating not to have a better answer. And by now, we should.

There have been scattered people who enter these forums and present as having been infected through precum. Not many, but a few. Unfortunately, many of these people were not HIV positive at all, but simply worried wells who refused to believe their test results. Others reluctantly (or in some cases, unwittingly) admitted to far more risky acts than they claimed.

Thing is, anecdote =/= data. So it is really difficult to tell.

Again, studies in vitro say it's entirely possible, yet not likely. But that would be cold comfort to the rare outlier.

All you can do here is wait six weeks and test - then test again at three months to be certain. I wish there were another legitimate, definite way.

I will say that, with your partner's low viral load, I would be really surprised if you tested positive over this event.
