The real issue with poz-on-poz barebacking is the risk of picking up an STD. Having HIV can make having other STD's seem tame by comparison. Gonorrhoea and chlamydia seem pretty mild eh?
And, as you know only too well, there are other more horrible STD's such as syphilis out there. Herpes and warts are odious as well.
Now I'm not a member of the safe-sex taliban, I bareback with other pozzies meself, but it's important to be aware of this stuff. Trust me, herpes can really cramp your style. Genital warts can come with a whole range of unpleasant complications as well.
So before going pink-on-pink you need to consider these things.

You Timmy, my first STD was syphilis. Back when I was all of 15. The treatment in those days was pretty nasty and it was a full two years before I got my next dose.
Ano-rectal gonorrhoea.

I had syph at the same time as I came down with teh AIDS. That was jolly good fun. Otherwise, my hoo hoo has remained relatively trouble free.
Except for some rather rough anal tears back when I was a bottom attempting more than I really should have at the time.

I only wish I lived in such a liberated town. Now that I don't drink, I don't know where to pick up a man/woman. I do need to find out though. I know I'm behind.
As for the STDs and sexual promiscuity, I was in the doc's office when I was 14. I don't remember what it was I had, but the doc gave me a shot that knocked me on my ass, put me on two antibiotics, I had to douche twice a day, use vaginal cream twice a day and take a sitz bath twice a day. That wasn't consensual though. But after I did become promiscuous, well, I learned to turn it into a trade and make money. So, I guess you could say I was very "ecnonomical." :D No, seriously, I was 17 was when I worked in the strip club here (Kitty Kat Lounge, go figure) and learned the "tricks" of the "trade" very quickly. I was in the doc's office every week after that. Don't miss those days-well, miss the action, not the reaction. ;)

regardless, i still have safe sex when there was butt loving going on. that risky shit didn't start till last year and it only took a handful of situations to catch the hiv. and the hpv.

Hepatitis C is spread by blood-to-blood contact. There is some evidence about it being transmitted via particular sexual activities (fisting and blood-sports come to mind), there is no evidence that is a generalised STD.
For the large part in the developed world, HCV is transmitted through sharing contaminated injecting equipment.

Is there a risk? Yep. My partner is Hep C while I am not....is the risk small....yes....is the risk there? yes. I am gambling I agree (more times top than not lessens the odds but nontheless) I love being without barrier in him and on occasion him in I and I will make no apologies about it ow...or if the wrong diagnosis ever happens.

If your long-term goal is to accumulate a FULL HOUSE of STD's, I say go for it. I like unprotected sex as much as anybody, but now that I have AIDS, you couldn't pay me to go skin-to-skin with anybody. It's bad enough being treated for HIV -- I sure don't want to complicate my treatment with a case of the clap, hepatitis, syph, warts, cooties, or anything else. (Plus there's some danger -- however small -- of getting reinfected with a different strain of HIV, I believe.) When you've already got HIV, it seems sorta dumb to go out and try to catch everything else. It's like "here I am, come and get me". But it's a personal decision, and I hope I don't sound like your preacher.

My preference has been not to use condoms, which kind of limits the options.
At the moment, I am not sure how I feel, as the doctor told me that I'd had hep c a few years ago.
Though this all hypothetical, as I am in the middle of the driest period of my life. Yep, I'd even consider wearing a condom to get a bit of man on man sausage action.

For the record, if I were playing the field, I'd still use condoms even with other pozzies. I don't want a dose of ... whatever.
The guy I'm currently with is the guy I got my virus from in the first place. He's on meds and has not developed any resistances, so no worries about getting a different strain. (From what I understand, that's a rare occurance anyway.) He's also had a recent STI panel. I haven't, but my ex-partner did just before we split up in November and as I haven't been with anyone else since the last time I had an STI panel, well, no worries!

The sober and not snickering about we're having a laff answer to your question is as follows:
(1) In terms of reinfection with HIV, the lower your viral load the less the risk, which is in any case small and most likely to happen it seems in the 1st 3 years of being HIV-positive. The notable risk to your is acquiring a resistant strain of HIV, again rare, but critical if it happens since it reduces your treatment options. People who are inconsistent at taking their pills on time all the time are the greatest risk here.
(2) Not using condoms increases your risk of other STIs, because condoms are protective against these in many cases.
(3) If you are into fisting/"rough" sex, increased chance of acquiring hepatitis C.
Viral load is the key factor. If you are on treatment and have an undetectable viral load the reinfection "question" evaporates on my reading of the studies to date. Higher viral load = greater risk.

Newt, I just read an article and if I can find it I'll post it, that one can have an undetectable blood VL, but don't make the assumption that your sperm and prostrate VL are that low, because they normally aren't. I'll see if I can find the article.

If you're already infected, you can't get "reinfected". Shooting cum in your body won't "reinfect" you with something you already have. What newt says is that if you have unprotected sex with someone who has a high viral load or has become resistant due to non-adherence, then there is a possibility that you may acquire another strain, which may be resistant for you too.
This is why my doc asked me to do a new geno/pheno to verify that I didn't acquire anything new that may be resistant to Atripla.
The studies show that this "super acquisition" is very rare. Also, another strain doesn't mean a worse infection. Most strains are susceptible to a battery of available drugs, and other strains are so weak they can't replicate enough and pose no threat.

With a reg also-positive partner I would drop the condoms but a 1-off shag? hmmm
As for Rapido;s valid point re: variance in viral load (1) most (maybe all even) of these studies were done in people not on treatment (2) the variance was generally 1 log ie prob only significant in terms of transmission to an HIV-negative person, not reinfection. But tis true, you can sometimes, note a variance.

Wow! I'm surprised at how many people are encouraging the BB for poz on poz... I've been known to be a bit promiscuous myself over the years, have always tried to be safe, but obviously failed. since testing positive I refuse to have sex without protection regardless ... both for myself and my partner (whoever he might be, no long-term)....I actually fear the rumor of "super strains" and other STD's are very prevalent....and I couldn't live with myself if I thought I infected another (which is a poz on neg situation, I understand) Honestly , it just seems like the smartest thing we could all do is to practice safe sex in all of it's formats (there's a hell of a lot you can do that is safe, fun, and amazing) and I think it's funny that I sound like a bit of a prude, if you knew me you'd laugh out loud.

well, for those of you who have not "had the courage" all i can say is when i was told Dec 6, 2007 i was hiv + my first thought was 1) oh shit, who have i exposed and do those ten frat alums count? 2) holy shit, I will never get to have sex again, oh fuck, and 3) damn it, its Christmas, how will I tell my family. It took me about 10 days to have fantastic sex again, with protection with someone who I was seeing who is neg, the two people who had been exposed have now tested neg, and my family - well decided it was not something they need to know right now. having said all that, I really like skin on skin and all the rest, but the person really has to be poz for me to be kool with it. I am concerned about them giving MEEEEE something (a neg that is) more than I am about giving them something. It is my understanding that female to male transmission is more difficult for all STDs...so i say ride um coyboy. Zan

The HIV Sequence Database contains almost 100,000 entries. It's not the number of strains that you should look at, it's what are the ones that are virulent and the ones that are non-susceptible to medication.
Each entry is documented and there is a HIV database search for professionals where they can enter the HIV sequence and see how virulent or susceptible it is. When you get your phenotype done, it lists your mutations, you can always enter them in the database if you want.

You input your mutations (.e.g K13K, V35I, T2000A etc..) and it will show the subsceptibility. I have 20 mutations and all are susceptible to medication, so I'm fine, but your Phenotype report will be much easier to read.
Don't be scared of the amount of mutations, it's really what kind of mutation that is important.

When I have safe sex with a negative person, they know I'm positive, and most of the time I can feel that they're scared of the condom breaking, the cum getting on a wound or something. It's not comfy sex, so I avoid it, unless they show that they're confident and feeling good about this.

I am in, I will BB with other Poz guys. I liked it before I became poz, I like it now. I love it, I will continue to do it. I caught syphillis in Feb 07, was treated in March, but it still effects your immune system for a year or so though, so caught HIV in May from BBing and didn't even have anyone cum in me. So will not top a bottom BB. Still waiting really to play with the neggies until I know that I am undetectable and the risk is minimized, then I'll top one with a condom.

Actually you can. Reinfection simply means acquiring a 2nd infection by the same disease. And it has been shown that people can acquire a 2nd (or more) infection of HIV, even after building up an immune response to the initial infection.

I'm not convinced it's that rare. I am convinced that it goes unnoticed and is hard to detect.

So true. A LTR with another poz guy is the only circumstance where I'd consider dropping condoms.

Cliff makes a very important point here.
Studies don’t actually show that reinfection with another strain of HIV is rare. They show that identified cases that have been documented in medical journals (generally because wild-type, drug-sensitive HIV, has been replaced with drug-resistant strains of the virus) are quite rare.
Why have these few cases been identified? Generally because there has been a problem and the patient happens to have been in the right place (and at the right time) for that reinfection to be identified.
That may suggest that reinfection is rare. But that doesn’t preclude the equally plausible possibilities that reinfection is actually quite common, but not necessarily problematic .. or indeed that it is quite common and does cause problems, but is simply blamed on mutation and developing drug resistance.
We aren’t finding more cases of reinfection because there isn’t a structured program for finding them - and the ones we have documented have mostly been found by pure chance - so since few of us are ever subjected to the sort of scruitiny that would identify it, reinfection rates are almost certainly significantly higher than the documented cases suggest.
My reading of it is such that I restrict the bareback action to the one guy I trust to be totally honest about where his tackle has been (mainly because I don’t want syphilis, but thoughts about the risk of reinfection are certainly not absent).

Which is the definition of reinfection (or superinfection).
Reinfection simply means a new or secondary infection by a virus that has already infected a person. With most viral diseases it is true to say that reinfection doesn’t normally occur - because once the immune system conquers the original viral infection, it creates immunity against that virus. With HIV (and indeed the common cold and the flu), because many versions of the virus are substantially different from that already present, reinfection does occur (and with HIV, there is of course the added complication that the immune system never actually conquers the virus).
Where I think a lot of the confusion stems from is that most of us seem to be quite happy to assume that reinfection has only occurred where there has been a rapid and unexplained progression to AIDS. That is, unfortunately (or should that be fortunately?), not the case.

I can't deny that sex without condoms is more enjoyable but I obtained the delights of syphilis at the same time as HIV and once was enough for me thankyou. They are not coming near me with those penicillin injections ever again!

Well someone knowledgeable would have to confirm. Being infected with another strain doesn't make the infection "super". It can be super, but usually it's equal to or lower to what you already have. Maybe we should use "primary infection", first time infected, "secondary infection", getting other strains that what you already have, and "super infection", where one of those secondary strains is uncontrollable with the meds you already take.

Reinfection and superinfection are exactly the same thing. One term is just a little more dramatic than the other and creates a sense of that rapid progression to AIDS.
Even using your definition of ‘superinfection’, all superinfections would be the result of reinfection.

I know BB isn't safe, but when I was hiding my status and using condoms I just didn't have many playmates. So many guys would just walk away if I put the condom on. Reinfection and superinfection are exactly the same thing. One term is just a little more dramatic than the other and creates a sense of that rapid progression to AIDS.
Even using your definition of ‘superinfection’, all superinfections would be the result of reinfection.

Hmmm... over a decade of high, unsuppresable viral load, multiple drug resistance issues, one case of gonorrhea from a LA porn star (very Studio54 of me!), copious bouts of anal warts from Carlos at the TomKat Bookstore, and a grade 4 external hemorrhoid -- all in the span of 18 months... well, it made me a more prudent individual.

This is more myth than reality and Matty the Damned says "Feh!" to it.
It's possible but extremely unlikely. In all the world the total number of poz-bodz who've acquired a nastier resistant "strain" of Iris the Virus through bareback fucking number less than two dozen. As someone noted earlier, you're more likely to manufacture your own resistant strain somewhere than catch one from someone somewhere along the line.
It's other STD's such as syphilis which are of more concern.

Not to rain on your, um, parade, but Poz On Poz unprotected is just as dangerous because of the risk of contracting med-resistant strains or more virulent strains of the disease. You may not be that sick now, but you sure don't want it to get worse.
And don't want to preach, but you are intending this in a monogamous setting, right?

I can understand why people feel this way, but things aren't this simple. We've discussed this at length many times and we always end up back at the same place. Superinfection doesn't warrant the fuss that's made over it. Contracting an additional, nastier strain from unprotected sex is vanishingly rare.
When we make superinfection the focus of the bareback debate we ignore the far greater risk (I'm talking greater by orders of magnitude here) of contracting another STD.
As has been noted previously STD's like syphilis and hepatitis B are a real problem especially for HIV positive people.

I was diagnosed in March 2006 after ending up in hospital with PCP and Cytomegalovirus and lucky to be here.
At every appointment my consultant raises the issue of contracting a second strain of HIV and in particular one that is drug resistent. I never went BB and probably still wouldn't. I just don't have sex now. Guess I'm just paranoid that if there's anything out there, I'm going to catch it.

I still work with this man's son as both his parents are now dead. I don't spend any time thinking about how I got this virus. I don't blame anyone. I have it and I cant get rid of it. I need to use all my energy to live with it and get to tomorrow.

I am only more fearfull of anonymous sex because I don't know someones sexual history and also the fact that when I was having Lott's of anonymous sex I was drinking and drugging allot ...so for me at the time it was self destructive. I think ADD makes it hard for me to really be afraid of future consequences i am so stuck in right now. So um yeah I know all the risks and I only BB and only have slept well had sex with other POZs since i was infected. i sure hope i dont get a super infection, syph etc but in reality i have zero fear about the future till iit comes <no pun intended>. My Dr. redid the genotype once and then I read to get a genotype you need over 1000 viral load so what was the point? I know it's a bad choice to BB. I know with my labs and how well my body contained the intial infection a reinfection with a strain that was different might bring that house of cards down but it doesn't change my behavior. In a way it's insane but it's how I got to here in the first place. I guess it's like being a smoker for me. I know it's not a good choice but I want it soooo bad! The fact I know iis a step on the road to change but i even think about safe sex and I feel like a whiney chiild who is having a toy taken away. A therapist, a Dr, a case worker all talk to me about it and I am like Sam i am.
