VL <40 but can it go lower?
« on: May 22, 2019, 11:23:58 am »
My last ID doc appointment a few weeks ago, my VL was below 40 copies, which it has consistently been since the treatment which I am on approx 1 year now fully kicked in.

In my hospital ID Doc does not consider <40 to be totally undetectable as per his definition of it. Now I know anything under 200 is Untransmittable so no real worries about this.  Doc said that maybe with more treatment time under my belt that my VL might go to "undetectable" as they measure it which I'm assuming is <20.

I'm curious has anyone else heard of this or being able to squeeze in a marginal improvement in VL count with more time under treatment?

Logically I know it doesn't really make much of a difference to me day to day, as I know in other clinics/countries <40 is considered undetectable. However if I can gain <20 in time as opposed to the <40, that would be nice too.

Any thoughts if more time under treatment might bring this?

You could measure the VL to 1 copy/ml

There is no practical or clinical, or studied difference between <20 or <40 or <50 for that matter.  Could it get lower with a switch, sure it could. Seen it in people who have VL's in multiple 100's and can't get the last bit suppressed

Personally if it was me and, i was UD by all treatment & clinically recognized standards I would not be switching if my meds i was on are working well for me. If it was in the 100's than yes, but ultimately do whatever you feel is right for you. ;)

Best, Jim
