Not sure if this should go here or in the nutrition area.
My question is what are the issues if I take my medication without taking it with at least 400 calories? Will it be less effective in some way?

Yes. drugs with the requirement to be taken with food, absorb differently and less effectively without being accompanied by food. Too maintain the correct level of meds in your system complera must be taken with food. as to how many calories, you would have to look at the information on the package insert to find out that.

a little googling ("complera package insert") and I found this in the Pharmacokinetics section:
total calorie content of the meal was approximately 400 kcal with approximately 13 grams of fat
Single-dose administration of Complera tablet to healthy subjects under fasted conditions provided approximately 25% higher exposure of rilpivirine
so the answer to what happens w/o food is that the level of rilpivirine/Edurant is about 25% higher than it would be with 13 grams of fat in a 400 kcal meal
the package insert (http://www.gilead.com/pdf/complera_pi.pdf) does provide this caveat also:
Effects of Food on Oral Absorption
Take COMPLERA with a meal. A food effect trial was not conducted for COMPLERA.
Therefore, the specific effect of food with COMPLERA tablets on rilpivirine, emtricitabine and tenofovir exposure has not been established. The recommendation to administer COMPLERA with a meal is based on the increased exposure that was observed when rilpivirine tablets were administered under fed conditions.

One question I have is : is it better to take complera without a meal or wait to take it with a meal?
Today, I missed me standard "time" that I take my meds. Being new to this I had a meal and forgot to take them. I realized this 30 min later and wasn't sure if I should take it immediately or wait for my next meal? I chose to eat another 500 cal meal within a few hours to play it safe as I've heard the life of complera in your system is around 36 hours.
Any thoughts?

that is exactly why i said no thanks to this drug when it came out. too much work to have to take with a meal. ha
ill wait for quad pill.
also the drama that happens if you take it with acid reflux meds too.
way to many rules for a pill in this day and age.

that's funny. LOL
I pop a Reyataz/Norvir/Truvada with dinner every night (because a person's gotta eat sometime during the day, right?) and think nothing of it. Taking pills with a meal is one of the easiest regimens out there. To be honest, it's just as easy as taking a pill at any other time of the day. (quite a few meds have restrictions about antacids because, just like food, that affects absorption)

30 mins later? I would have taken the pill.
An hour and a half later or more? I would have taken it with the next meal, or had a snack later on.

ONE pill a day, with food -- my my folks, that is just so damn difficult. It's not like we all eat every day... something.

To be blunt I have found if you are not at 400 it gives me a massive case of the runs.....

The only problem I've had with it is the nausea. When I wake up the next morning I am nauseous for probably an hour or so. When I eat it seems to go away.
