Hey there!
I am concerned over a an incident that happened last day. I had sex with one girl whose HIV status is unknown. I used condom which was intact for the whole session but I made a mistake every time I removed it. I touched my penis head after removing condom and also I washed it using normal water without soap. During washing it also I touched tip and around and inside of my penis head many times. 
Am I at risk for touching the penis tip and inside it as vaginal fluid from condom was into my hands and then inside my penis??

No HIV risk whatsoever.

HIV can't transmit through an intact latex or polyurethane condom. If a condom fails during the act of intercourse it's obvious to the insertive partner. There is no reason to be stressing or testing outside of standard routine as long as this obvious issue did not happen.

Here's what you need to know to avoid HIV infection:
Use condoms for anal or vaginal intercourse, correctly and consistently, every time, no exceptions. Consider starting PrEP as an additional layer of HIV protection going forward.

Keep in mind that some sexual practices which may be described as ‘safe’ in terms of HIV transmission might still pose a risk for transmission of other STI's, so please do get fully tested regularly and at least yearly for all STI's including but not limited to HIV and test more frequently if unprotected intercourse occurs

Also, note that it is possible to have an STI and show no signs or symptoms and the only way of knowing is by testing.

I am worried if her saliva had some blood and then it entered my penis when she spat over my penis. Also to add to this, I touched my penis head after sex after removing the condom, which had her vaginal fluid. Are these HIV risks? I already started taking PEP but it had a very bad impact on me. Can you please assess my risk for these incidents? Should I continue taking PEP?

Read your post three times, the concerns you posted aren't HIV risks.

Move on with your life!

Here's what you need to know to avoid HIV infection:
Use condoms for anal or vaginal intercourse correctly and consistently, with no exceptions. Consider talking to your healthcare provider about PrEP as an additional layer of protection against HIV

Keep in mind that some sexual practices described as safe in terms of acquiring HIV still pose a risk for other easier acquired STIs. So please do get tested at least yearly for STIs, including but not limited to HIV, and more frequently if condomless intercourse occurs.

Also, note that it is possible to have an STI and show no signs or symptoms, and the only way of knowing is by testing.

You did not need PEP (post-exposure prophylaxis) over the activities posted here, and, if you keep having these irrational fears, I urge you to consider two things.

1) Consider talking to a qualified therapist to help you manage these thoughts.

2) Consider talking to your healthcare provider about PrEP (Pre-exposure prophylaxis). This is an HIV medication you can take, and it would offer you an additional layer of HIV prevention, but prehaps more importantly, it may offer you some peace of mind going forward.
