What is my risk and was I correctly given PEP?
« on: July 27, 2017, 10:45:28 am »
Hey thanks in advance for taking the time to read my post.

40 days ago I had sex a transgender (MTF) escort. We had protected anal (both top and bottom) and I received unprotected oral.

6 days ago I had sex with another transgender (MTF) escort whilst in Ibiza. The sex consisted of unprotected receptive oral, protected anal (I was top), I kissed and licked a bit the penis but didnt insert into my mouth. I also fingered her and may have after 5 minutes put that same finger into my own mouth. Sorry for the very exact details but I just wanted to give the full picture so you can assess my situation!

5 days ago, so less than 24 hours after my second encounter i began to freak out about potential HIV exposure so I went to the doctors and described my situation. When asked if the condom remained in tact I said I couldn't be 100% sure although I am actually 99.99% sure it didn't break, both times. The 0.01% of doubt is due to the fact that I didn't closely inspect it once removing, so worried there could have been a tiny tear?
The doctor prescribed me a course of PEP which I am currently 5 days into. Was I at risk enough to warrant this? I think the fact I got given PEP has spiked my anxiety over the exposure more than anything.

When I went in to get the PEP they did a quick test for HIV which came back negative. Would this be a pretty good indication that I didn't contract HIV from the first encounter 40 days ago? I know 3 months in conclusive but negative after 40 days.. can I begin to relax a bit more about that?

And with regards to my most recent encounter. What are my risk factors for the intercourse I had? I know giving oral sex is low risk and even lower if I was just kissing and licking but I do have bleeding gums (mild gum disease) when I brush my teeth? would this put me at higher risk?

2 days ago I started having hot flushes and post nasal drip. Now my throat is really dry and I am getting concerned that these could be symptoms of ARS?

Any answers on this would be so gratefully received. I'm beginning to freak out a bit.

Thanks

Hi

Firstly HIV isn’t transmitted By hugging, shaking hands, sharing toilets, sharing dishes, kissing, through saliva, tears, or sweat. Also HIV is sexually transmitted inside the human body, as in never leaving the confides so the finger to your mouth was never a HIV risk.

The licking and kissing was simply not a risk - Oral such a crap way to word a sexual act. Unless you were giving a BJ, you simply had no risk and even if you did give a BJ ? ? ? ? than its nothing to panic about as the risk is near none as long as you don't have gaping wounds in your mouth as your month lacks the cells for HIV to infect or the route, and no, the brushing your teeth and bleeding gums made no difference.

Getting your penis sucked is also not a HIV risk, saliva is not HIV infectious, nobody has ever been infected this was and the same can be said for fingering, its not a risk and has never happened and you will not be the worlds first.

Regarding PEP well guidelines are changing in lots of the health departments and hospitals to no longer give PEP when the only reason is for mental/emotional relief.
Instead focus on counselling the patient and educating them and if warranted from that session providing further support. I know some Doc's still do give PEP even when no risk is present just to give the patient a fix (peace of mind) however this does not address the route of the issue/concern.

Clearly it has not worked for you in this sense of reducing your stress about this no risk scenario, as you are still in an emotional sate, and posting online in forums (Not only this one i noticed)

From what you have posted you simply had no HIV risk.

Here's what you need to know in order to avoid hiv infection:
Use condoms for anal or vaginal intercourse, correctly and consistently, every time, no exceptions.

Keep in mind that some sexual practices which may be described as ‘safe’ in terms of HIV transmission might still pose a risk for transmission of other STI's, so please do get fully tested regularly and at least yearly for all STI's including but not limited to HIV and test more frequently if unprotected intercourse occurs

Also note that it is possible to have an STI and show no signs or symptoms and the only way of knowing is by testing.

More information on HIV Basics, PEP, TaSP and Transmission can be found through the links in my signature to our POZ pages, this includes information on HIV Testing

Kind regards

Jim
