Hi zgood:
Wow, alot of questions. I'm on Sustiva and though I didn't experience the rash, I know quite a few people do. As far as waking up with the horrible mood swings, maybe you could cut back on the sleeping medication. It just seems like taking a high dose of that and an antihistamine might be a little over-medicating. Of course, I'm not a doctor, therefore I would discuss this with your doctor. Tell him/her about your concerns etc. and see what his/her opinion is.
As far as going back to work, I don't know what your counts are, how long you've been diagnosed positive, what your source of income is etc. There are a lot of factors that are missing. Maybe you could fill those in. But again, as far as going back to work, that is a personal and medical choice. Oh, and welcome to the forums!
Peace-

Zgood,
The mood swings are most likely a side effect of the Sustiva and should subside in another couple of weeks to a month. If they persist you should discuss this with your doctor.
Sleeplessness is also a side effect of Sustiva and it maybe that the anti-histamine is sufficient to make you drowsy, without the sleeping pill.

hi,so i should continue thet and stop the sleeping pills?

Well, you should discuss it with your doctor. If you're feeling completely bombed out all day (the Sustiva can add to that as well) then it might be an idea to take one of the drowsy-making medications out of the mix. Anti-histamines are often used in over the counter preparations to aid with sleep.
But do discuss it with your doctor first.
