I just tested positive as a result of getting sick with PCP.  I was hospitalized for 5 days, IV Bactrim, Rocephin, fluids, along with oral prednisone 20mg twice a day and fluconazole 200mg once a day.  When I entered the ER, I could barely breath, I could only make short breaths.  I was not sick prior to this, only a nagging cough that lasted a week, Saturday morning I was in bad shape.  I tested several years ago negative so this comes as a blow.  The doctor stated my CD4 count is 46.  The focus now is to rid the PCP, I am currently at home taking Bactrim, prednisone and fluconazole.  I finally have my appetite back and can breath without the assistance of oxygen.  I have acceptted this diagnosis, but still it weighs so heavily on my mind.  What is the best med for a person in my condition?  The ID Doc states she wants me to join her clinical trials when the PCP is gone, but I have researched clinical trials and with a CD4 count of 46, I am unsure I can risk such treatment.  I have excellent insurance and a good job, so free meds/labs is not really a benefit for me, along with the additional time frames required for clinical trial data. Can anyone offer advice?  I will be home for the next week recovering from the PCP and would like to start meds ASAP.  It took a while, but I found you guys......I also fear this disease will change my body and face.....thank God I am still alive.   :'(

Hi ATLpoz,
Welcome to the forum, sorry about you being sick.  I sorta had the same issue same time last year, although i was lucky enough to not get PCP, but i was sick as a dog, took me 3 weeks to get over the flu-like symptoms. i also had tested negative about 8 months prior.

I'm not sure I would do the clinical trial either, just would be a personal preference.  hope you feel better soon sounds like you are on the way to recovery, get some rest in the meantime.

Hi Mike...thanks for responding.  How soon should I start meds?  My CD4 counts are really low and I would like to start the meds very soon.  Taking off 3-4 weeks from my job to get better.  Also looking for some recommended drugs with least side effects.   ???

your welcome,  well some folks will probably have different opinions,  you're doctor will need to run a resistance test on you before you start meds.  i'm guessing that's what you're doc will do once you get you're strength back.  I'd say that since you're CD4'S are low they will probably want to start you on something soon.  Some folks like to wait for a second round of tests.

As far as meds some folks have different experiences,  There is a couple of good once daily pills you can choose from depending on your viral load count.  i started on Atripla and was lucky i didn't really have any side-effects, except the vivid dreams.

Welcome!!! The desire to start meds right away is only natural.  At least it was for me too.  You want to do something NOW.   Don't worry though, right now the PCP is more important.  Get that taken care of.  Mikey is right. First step is to take a viral load test and get a resistance test.  The resistance test will tell you if there are any meds you cant take.  Once thats out of the way there really isnt one regimen thats better per se than another.  Some work better on higher viral loads than others.  Some have this side effect, ssome that.  6 of one half a dozen of the other.

Clinical trials are not a bad thing. A lot of times they test already approved meds just in a different combination or formulation.   Look at the lessons about the different first line regimens here on aidsmeds.com .  The have a lot of info.  The US NIH also spells out what treatments they recommend and don't.  Good luck!

  Hi ATL, while I am sorry to hear of your recent diagnosis I am glad you found the forums.  I don't have anything to add by way of treatment options that hasn't already been said.  It will be a decision between you and your doctor ultimately. 

  Nasty stuff that PCP, but it sounds like you are on the road to recovery and I hope that continues going well for you.
