What's more important high CD4 or low VL?

My VL has never been und. but my CD4 is always >800. What's more important for my long term- high CD4 or low VL?

You say your virus has never been undetectable but what has been the range? There's a big difference between a VL of, say, 1,000 or even 5,000 and 50,000+

High CD4

Low CD = risk of opportunistic infections and hiv itself getting so out of control it does serious damage.

Sorry, my VL has been 4000 and sometimes been close to 10,000. But not higher. CD4 always been above 600. Should I change therapy? No problems with combo I;m on so far.

Are you on antiretroviral medications (i.e. HAART)? If so, for how long? If you are on HAART you should be undetectable after a certain amount of time.

Yes, of course you should change therapy as it's not working.  Why had your doctor not suggested doing so?
