Welcome to the "Do I Have HIV?" POZ forum.

This special section of the POZ forum is for individuals who have concerns about whether or not they are HIV positive. Individuals are permitted to post up to three questions or responses in this forum.

Ongoing participation in the "Do I Have HIV?" forum (posting more than three questions or responses) requires a paid subscription, with secure payments made via PayPal.

Broken condom with Thai CSW

Hi all,

Firstly thank you for accepting my request.

Now, for the incident. I'll be as detailed as possible.

I was in Patong and picked up a bar girl (I paid 3600 baht if I remember correctly). We got to the room and she went into the shower and washed herself. We both stripped down, I put on a condom and we began having sex in missionary position. I'm fairly well endowed girth wise so it was quite a tight fit (apologies for the graphicness). I pulled out and the condom was still intact, we then began having sex in the doggy style position. I was intoxicated so I can't remember the exact amount of time but I'd guess around 3-5 minutes. When I pulled out, to my horror I noticed the condom had snapped completely. She didn't seem particularly worried and gave me unprotected oral sex. After we finished in questioned her about her HIV status and she just laughed it off as if I was stupid for asking, and showed me some condoms she keeps in her bag.

I've had an antibody test done at 7 and 12 days, both negative and have tested negative for chlamydia. I have no symptoms of gonorrhoea. I'm awaiting the results of a PCR DNA test I paid for from a clinic in patong, they claim it is 99 percent accurate. Anyway, that aside I am beside myself with worry. The doctor I saw said in his 2 years of working in patong he's had around 30 guys like me come in and they have all turned out negative. He said he expects the same for me. I just can't stop trawling Google and keep seeing things like subtype E being more easily transmitted etc. The girl was fairly dry, and I had no cuts or sores on my penis shaft. There was no blood and it was a single exposure, I don't know the HIV status of the girl but may try and take her for a test. Can somebody offer any dort of relief from my worry? Are my chances low?

So I understand it was a broken condom incident.

The good news is that until the condom broke it would have been protecting you, also as you were the insertive partner it would be less risk than being the receiving partner as example.  The oral sex you mentioned was not a risk at all just to be clear.

Now saying all that you did have a risk from the unprotected intercourse and so testing would be advisable. Guidelines are to test at 6 weeks past any incident and then at 13 weeks to confirm the result. You can read more about Testing here: https://www.poz.com/basics/hiv-basics/hiv-testing

I agree with your doctor and expect you will be fine however testing following this incident in line with the guidelines is the only way to make sure. You can post 3 times for free on this forum so please keep this in mind when/if responding.

You do not need to ask the girl to go test, it's kinda pointless. Instead you should simple focus on simply knowing your own HIV status and continue practicing safer sex. Note as you are sexually active, it is also highly recommended that you get a full sexual health check-up / screening at least annually including but not limited to hiv testing.

Here's what you need to know in order to avoid hiv infection:
You need to be using condoms for anal or vaginal intercourse, every time, no exceptions until such time as you are in a securely monogamous relationship where you have both tested for all (STI's) sexually transmitted infections together.

Remember that some sexual practices which may be described as ‘safe’ in terms of HIV transmission might still pose a risk for transmission of other STI's. So please do get fully tested regularly for all STI's including HIV and more frequently if unprotected intercourse occurs. Also note that it is possible to have an STI and show no signs or symptoms and the only way of knowing is by testing.

Finally use condoms for anal or vaginal intercourse, correctly and consistently, to avoid sexually transmitted hiv infection.

Just to update, I manged to track the girl down and had a long chat with her. Turns out she has only been a CSW for one month and she seemed very careful. I took her for a rapid HIV test just to be sure and it was negative. I feel much more confident now after speaking with her as she was actually very upset that I felt the need to ask her to test. I am still going to continue with my testing to make sure.

So firstly for the sake of the assessment we always presume the other person is living with HIV.

I had explained getting the girl tested is kinda pointless, it does not change your need to get tested and the reason is even if the result is negative from the girl today you do not know her risks were leading up to your contact and she may well be in the window period herself and during that time a person can be infected with HIV and be infectious but show a negative HIV result.

Yes, I can imagine she was. Her status is not your concern, it also not your business how long she has been working etc. Now please leave the girl alone and focus on your own business.

I agree with Jim … all you did was upset a woman who probably has other issues to deal with. Getting her tested means she didn’t have HIV 3 months ago so you upset her for no good reason. If you had a risk thats on you and for you to test for and no one else.

Please leave her alone .
