Question about CD4 counts....

Hello,

In April i received my positive dx, I was at 52,000 viral load and 448 cd4. After a little over a month on treatment my cd4 went to 573. That was on May 23. This week I am finally undetectable (yay!) BUT my cd4 droped to 374. Why would my cd4 be going in the wrong direction? :,( any opinions would be appreciated. It is worrying me a bit. Still waiting on reply from my doc...

everyone's does that, it fluctuates if you were to take a negative person it would do the same thing, you could of been tired, or anything really, large fluctuations happen you'll establish a baseline soon enough and you'll learn your thresholds.

Cd4's fluctuate even depending on the time of day you get your labs done.  If you have been sick, feeling sick, been under stress, things like that, can cause fluctuations also.  It's nothing to worry about, you're undetectable.

When you're not on meds, the Cd4 is the most important number; after you start treatment, the VL is the most important number.  As long as you stay undetectable, that means the meds are working.

CD4s can vary by up to 100 pts within the very same day!

cd4s are lowest in the morning (9-noon) hours and highest in the late afternoon (4-6pm). in the first 30-45 mins after exercise, cd4 counts can fall 50-100pts to regain that and more an hour afterwards. cigarette smoking can give a temporary 25-50 pt jump but the time frame is very limited before falling.

Stress, eating, relaxing, etc. everything has some sort of cd4 effect throughout the day. That's why a patient should never extrapolate anything from a single test. The best measure of cd4s is to see the trend of at least 3 tests over a time period of 4 - 12+ wks.

there's a lot of great info about cd4 counts, check out this thread "Death of the CD4 count" to learn a lot about CD4s, the quality of an immune system, and why as you reach undetectable and/or have a count over 200 that the cd4 count just doesn't mean all that much.
