A little perplexed by my recent CD4 test

Hi guys and ladies, I am trying to make sense of the blood test I just had. It included my lowest CD4 total to date of 409 however it also had the highest CD% of 43 I have ever had. My VL went undetetable and I just started HARRT recently, this was my first test since starting HARRT. My doctor doesn't seem to be concerned as it is common to lose CD4's at first when stating HARRT. Just trying to make sense of a 27% drop in my CD4 since the last test. How is it possible to have an all time low CD4 count and yet have the highest CD% you have ever had? I've thought about it and thought that maybe the other types of WBC's that make-up the WBC total count should be lower as well. However they are about the same as they have been (3.8). If my CD4 count is lower by 27% yet I gained 3% of CD4%, would it not make sense that my WBC count should be lower by MORE than 27% as well? Just a little confused and trying to understand the numbers.

2011 Aug -- CD4=404,44% VL/WBC=8.8k/4.0
2011 Sept -- switched to Truvada & boosted Prezista. CD4 suppression, SE and adherance issues.
2011 Nov -- CD4=522,49% VL/WBC=2.3k/3.5
2012 Jan -- discontinued meds. SE and adherance issues, deciding on new regimen.
2012 Feb -- resistance testing revealed mutations: 3 NRTI (incl K65R), 4 NNRTI and 3 PI's.
2012 Apr -- CD4=599,41% VL/WBC=263/3.8
2012 May -- Shingles
2012 Oct -- CD4=493,36% VL/WBC=4.7k/4.4 (still my latest Dr. visit as of 11/01/2014)

Hi,

Your concern is understandable...

I have personally experienced raises and drops as high as 200 during monthly follow ups, at initiation of HAART, so do not sweet it out. It bounces quite a bit, and this is more 'visible' since your counts are in the higher segment.

CD4 COUNT follow up is very important for monitoring patients going out of the danger zone.

Because you are not (and has never been) in a 'red' zone, not even in an 'orange' zone, but more in a 'green' zone, per certage is more the value to look at.

It is free. Picture 3 demonstrates that the CD4 dynamics should be observed over the long range.

Some people dynamics are slower than others (age is a key factor also). This is one of the reasons why I, personally, believe in earlier treatment...

The almost exact thing happened to me.  Prior to starting meds my cd4 was at 410 at 34%.  My first blood test after I started treatment and reaching undetectable my cd4's had dropped to 350 but my percentage had increased to like 46%.   My doctor wasnt concerned at all either.  I think your fine.
