I saw this the other day, and it's that type of conclusion I find hard to work out.

Not saying there isn't a correlation with death and alcohol related health issues for PLWH but isn't drinking, along with a variety of other things like drug use and depression, far more common amongst this population - and that could surely skew the stats?

But I'm also curious about the statement that the effects of alcohol happen quicker in PLWH. With an immune system in normal range and a person who is UD, how would this happen, medically speaking? I guess there could be interaction between alcohol and specific medications, but that in itself doesn't mean that it would apply to all people with HIV. A similar effect could be found with a variety of other conditions which are 'medicated' surely?

I'm probably just being thick here, but it does seem a bit of a stab in the dark.

Very few details indeed.  No theory on why this may be?  Unless it would not load all the way for me, I didn't even see details on the physiologic harm and what exactly that meant.

I didn't even see details on the physiologic harm and what exactly that meant.
I didn't see any details about what "physiologic harm" consists of either. Nor about "mortality". What exactly were the situations when PLWH died from alcohol?!? That surely entails more than just social drinking and would probably only include people with alcohol addictions who already had serious harm resulting from years of drinking.

Quote
Conclusions
Despite antiretroviral therapy, HIV+ individuals experienced increased mortality and physiologic injury at lower levels of alcohol use compared with uninfected individuals. Alcohol consumption limits should be lower among HIV+ individuals.

Results
The sample included 18,145 HIV+ and 42,228 uninfected individuals. Among HIV+ individuals, 76% had undetectable HIV-1 RNA (<500 copies/ml). The threshold for an association of alcohol use with mortality and physiologic injury differed by HIV status. Among HIV+ individuals, AUDIT-C score ≥4 (hazard ratio (HR) 1.25, 95% CI 1.09–1.44) and ≥30 drinks per month (HR, 1.30, 95% CI 1.14–1.50) were associated with increased risk of mortality. Among uninfected individuals, AUDIT-C score ≥5 (HR, 1.19, 95% CI 1.07–1.32) and ≥70 drinks per month (HR 1.13, 95% CI 1.00–1.28) were associated with increased risk. Similarly, AUDIT-C threshold scores of 5–7 were associated with physiologic injury among HIV+ individuals (beta 0.47, 95% CI 0.22, 0.73) and a score of 8 or more was associated with injury in uninfected (beta 0.29, 95% CI 0.16, 0.42) individuals.
http://www.drugandalcoholdependence.com/article/S0376-8716(16)00045-4/abstract

Dying or harm from alcohol happened in HIV+ persons drinking 30 or more drinks a month while it was 70 or more drinks a month for HIV- individuals. That sounds like a fair amount of drinking every month to me and not something described as "lower levels of alcohol use". I also wonder just what kind of alcohol was consumed. Beer, wine, and liquor produce different effects in people (the wine/alcohol has beneficial side effects while an equal amount of beer/alcohol would include a lot more calories. I would imagine there is a lot of difference in the alcohol content and other side effects from drinking 5 6-packs of Bud a month to having a shot of tequila every night for a month to a glass of red wine every night with dinner) which also play into a person's health - like the mortality and physiologic harm this study was measuring.

since we're talking about alcohol and totally opposite this report :_) .....
recently my partner and I stumbled upon the "best damn root beer" (http://www.bestdamn.com/) and I have to say it's delicious. It tastes just like a good A&W root beer with a hint of hops/beer as an aftertaste. We just bought some ice cream so we can try it as a root beer float.

Even though almost everyone is virally suppressed on haart,  that doesn't tell the whole story.  Ongoing inflammation continues for most of us.  And that seems to be a major contributor to future health issues that pozzies have - suppressed or not.

Alcohol causes inflammation.  Just a guess, but it stands to reason that exacerbating already existing inflammation would,  over time, lead to higher mortality.

That said,  I had a delicious merlot with dinner last night.  It was my 11 year anniversary and I made an exception for the special occasion.  I gave up regular drinking several years ago.  I feel much better.  But I really miss it.

Haven't tried that one, but I have tried "Not Your Fathers Root Beer"-- still have 5 left if anyone is traveling through the Houston area
