Switching from Atripla to Reyataz/Epzicom/Norvir

Hello all-

I've been on my first HIV meds, Atripla (Sustiva/Truvada) for 5 months and the side effects are not tolerable.  I'm having pretty serious anxiety and difficulty concentrating.

My Doc says there are so many options available to me that I should not put up with those side effects and suggested trying different meds.

We are looking at Reyataz/Epzicom with the Norvir "booster."

The lab has drawn blood to check for the genetic marker for a reaction (the test is the HLA-B0512... ???), so if I don't have it, we will probably go with that new combo.

After doing some reading, I see Epzicom has some of the same side effects as Sustiva.  I also read that Reyataz and Zantac don't really get along (I love my Zantac for heartburn).  To top it all off, I also occasionally take 25 mg Viagra when I'm interested in some all-night-long you-know-what... apparently not a good idea.

QUESTIONS:

Do any of you have experience you can share with regard to anxiety with this combo?

Do any of you take Zantac and/or Viagra with this combo?

Any other thoughts/suggestions?

Hey Cruisin,

I take Reyataz/Norvir, without any problems. I substituted my Prilosec with Pepcid, taken either 10 hours before or at least four to five hours after the Reyataz/Norvir.

So far, so good.

I have no experience with Epzicom, so I can't offer any experiences.

I am sure others will chime in though.

I've been on that combo for about 5 months
I do get headaches but besides that its working great for me     
after many years of cd4 under a hundred this 8/28th I should  break 100 (fingers crossed)

Viagra and Norvir have an interation. The recommendation is to use less ie 25mg (ie half a 50mg tablet or 1/4 a 100mg tablet) no more than once every 48 hrs. Norvir boosts the level of Viagra and similar drugs and makes them last longer in the body.

Hello all-

Thanks for your input/advice.  I've also been reading other postings concerning Sustiva.  Glad I'm not alone, but so sorry others are having issues too.

I had a little bit of a "meltdown" earlier today, but coming out of it.  If the huzband doesn't kill me first  :P, I'll probably start the new casserole next week (pending test results).

According to treatment guidelines listed above, Norvir is not required when taking Reyataz with Epzicom. You do not need to boost with Norvir, which help eliminate potential side effects and another costly med.

This is true.

This report, among others, shows better virological outcomes but worse cholesterol outcomes at 96 weeks for people using boosted Reyataz v uboosted - something to bear in mind when choosing perhaps.

Hello-

I've been cleared to start the Reyataz/Epzicom with Norvir booster.  Since Zantac (and other medications for reflux/stomach acid) are not compatable within 12 hours, I decided to take the new combo in the morning and the zantac in the evening since my heartburn bothers me more at night.

We'll see.  I'll start the new ones tomorrow.  I'm looking forward to being myself again.  The Sustiva should work it's way out of my system in 1-2 weeks, right?

BTW-I did ask my Doc about not using the Norvir booster.  He's insistent about it, so here goes.  He has not steered me wrong yet!

I take Reyatz just for a few days and noticed heartburn.  Used to take Prevacid, doc said that's no good, that I should try Pepcid and space 12 hours from Medicine.  Doc said one of the side effects of Rayataz is heartburn.  If I continue to have heartburn will take medicine in morning. Pepcid at night.   Good luck.

I decided to take the zantac at night because that's when my heartburn bothers me most. 

I took my first dose of the new combo this morning.  Right now, there's a bit of nausea (right after eating dinner), but not too bad.  Other than that, I had no strange feelings/side effects today.
