Title: Serious HIV Concern after Sex with CSW and ARS

I had an exposure to HIV infection on the 25th April with a CSW in Thailand and am very concerned. The risk involved:

Oral sex and vaginal intercourse both with condom. No kissing exchange of body fluids.

The condom appeared to be intact with no leakage/penis was never in contact directly with vagina.

As I had shaved I felt slight sensation on the base of my penis - worried if HIV transmission could take place from vaginal fluid onto hair follicles/shaved area.

I also have eczema on my fingers and noted that the condom had vaginal fluid whilst removing the condom. This was a very small yet possible exposure.

To the best of my knowledge I have showed no symptoms till week 13.

Thinking I was ok I had unprotected sex after about 6 weeks of exposure with my wife - always ejaculating outside. My wife now has had fever for two days (stopped), severe fatigue, sore throat (cured after medication - two days), mouth ulcer due to a cut and as of today heavy menstrual bleeding (very abnormal).

I am very concerned and preparing myself for a test, could you please help advise.

Three months after exposure whilst travelling I had Diarrhea x 4/5 days and keep feeling tightening in throat and think my right lymph node on neck is swollen.Do you think these are ARS?

Concerns lie on HIV infection through shaved skin (three hours before the encounter) and potential vaginal fluid contact with eczema cuts/healing surface.

Appreciate your assistance on the matter as I am very worried.

You did not have a risk . Vaginal fluids a woman makes while excited isn't infectious for HIV and poses no transmission risk . If present , HIV resides deeper in the vagina in a mucosal lining near the cervix . The fact you had sex with an in tack condom makes this absolutely a no risk situation , so you can relax .

 ALTHOUGH YOU DO NOT NEED HIV TESTING AT THIS TIME for this incident , anyone who is sexually active should be having a full sexual health care check-up, including but not limited to hiv testing, at least once a year and more often if unprotected intercourse occurs.

If you aren't already having regular, routine check-ups, now is the time to start. As long as you make sure condoms are being used for intercourse, you can fully expect your routine hiv tests to return with negative results.

Don't forget to always get checked for all the other sexually transmitted infections as well, because they are MUCH easier to transmit than hiv. Some of the other STIs can be present with no obvious symptoms, so the only way to know for sure is to test.

Use condoms for anal or vaginal intercourse, correctly and consistently, and you will avoid hiv infection. It really is that simple!

Hi Jeff,

Thanks for the prompt response. Unfortunately my stomach refuses to settle down, I have a bloated stomach and between 2 to 4 movements everyday. This has now gone on for 4/5 days - this has happened 13 weeks after exposure.

I also reviewed some documents available on the web and understand that some HIV cells are present in the vaginal fluids (not an expert by any means, so this is not meant to challenge your opinion).

I am now planning to go in for a test, put myself in a very sticky position and have to bear the consequences. I hope for the best, as I will be one very unlucky bloke if I do go positive.

You will not be the first person in the world to be infected from a combination of razor burn and vaginal fluids . I understand you are fearful and my advise has not alleviated your insistence you were at risk but we get countless questions that are pretty much word for word and not one person has ever tested positive for HIV ... and its because its impossible to be infected in that manner .

Do you realize that if vaginal fluids lead to HIV infections every person who ever had sex with a woman with HIV would be infected . Your ignoring the elephant in the room and that's if you use condoms correctly for vaginal and anal sex you will avoid HIV . There is no razor burn and vaginal fluid exception that exist .

I'm pretty sure Moderator Jeff meant that you WOULD be the first to be infected through a combination of razor burn and vaginal fluids.

And yes, HIV particles can be isolated in just about every bodily fluid from tears to sweat to vaginal fluids from the Bartholen Glands, which are the fluids you encounter outside the deep mucosa close to the cervix.

However, just because HIV can be isolated from tears, sweat, and vaginal fluids does not mean that viable particles can, and that those fluids are infectious. Otherwise, HIV would be classified as a contagious disease rather than an infectious one, and humanity would be in peril.

If a test will quell your nervous stomach, then by all means get tested. But from the incident as you described it, you can not possibly contract HIV.

Thanks very much for the optimism. I unfortunately can't put a finger on something and that is why I am fearing for the worst. There is something in the air or a vibe - some of it coming from my family who don't know what I am going through. I also failed to track the CSW last week and hence even more concerned - she has stopped working at her usual place.

Logically everything you say makes sense. This is the only one time, I think the condom was ok i.e. semen was still inside, I am circumcised, no kissing etc.

I never fall sick but my stomach has been bad for almost 4 to 5 days. Also stomach is the weakest area in my immune system so putting two and two together it is not looking very good. My food intake has gone down considerably.

I don't mean to waste your time by going on. I hope to have a test on Wednesday that is the earliest I can get.

Thanks for the support - this has been very difficult and if I do get out of this I will be one lucky guy.
