Diabetes, HIV & My Kidneys

I have been diabetic (type 1)  for the past 24 years with some complications associated with the condition over the years.  The worst problem has been some damage to the kidneys.  Since 2001, my kidney function has been stable with a higher than normal creatine level ranging between 1.6-1.9 mg/dL.  I started meds last Thanksgiving with Sustiva / Epzicom since that combination is kinder to the kidneys but found out I can't handle the Abacavir so was left scrounging for an alternative.  AZT made me physically ill so after a long discussion with my doc we decided to try Truvada and do monthly tests to make sure the kidneys are not affected by the tenofovit.  This was at the end of December 2006.

I just got my results back and it's interesting to note my creatine levels have improved since starting Truvada.  The latest numbers shows my levels at 1.5 mg/dL which is the high side of normal.  Also, my GFR rate has improved by 6 mL/min since December.  GFR is the rate at which the Kidneys filter the blood. 

Tenofovir is supposed to be rough on the kidneys but in my case it's helping them out.  How weird is that?

So the question then is are there others here who are diabetic (type 1) and also positive?

I have been an insulin dependent diabetic since the age of 14 (juvenile onset) and I am now 43.  Although I have yet to go on meds for HIV, it's good to know there are options.

On a side note, my doctor IS MUCH more concerned about my diabetes than my HIV.  Also, I think having diabetes and already managing a "chronic illness" has made going to the doctor regularly a little easier, but that's just me.

Going to the doctor is routine.  My endocrinologist is concerned about everything while my ID is only woriried about a single virus.  I've been blessed with a good team of docs to address this 47 year old's body.  My diabetes came about when I was 23 years old so at that time I was all doom and gloom.  Six years ago I had major issues with blood pressure and my eyesight.  Luckily those have been addressed and under control.  I now try to have "Doctor Day" so I can visit my Endocrinologist, ID and Nephrologist during the day with one set of labs instead of the three separate.

Hey Bud,

This was an interesting post you wrote.  I'm constantly paranoid about diabetes cause I have ALL the symptoms my mom has.

Although she's type 2.   Type 1 is more tricky and can be a major pain I'm sure.

You know I'm not surprised the Truvada has turned things around.  I actually feel better.   

My mom disagrees with her endocrinologist all the time.   Frankly, I'm prone to agree because he was putting her on way too many pills.   All of which have side effects.   

So, everytime I have my labs done they ck my blood sugar and the highest it has ever been was 80.   I'm suspicious I might be hypoglycemic or something.  I'll have to ck that out futher.

But it's interesting you mentioned the gloom and doom of diabetes.   The first doc. who diagnosed my mom asked her if she wanted to speak to a counselor when she was diagnosed ; )  LOL, she just looked at them and said were you expecting me to fall to pieces? ; )     We swiched her around a lot until we've found better specialists.

I believe you though.   Endocrinologists seem to look at everything and a lot of ID docs ignore everything but hiv and we get bounced around from one to the other.

Sounds like the labs are working out for your favor.   Good news to hear!

I'm happy the drugs you are taking is working. I am type 2 and right now both my doctors, primary and I.D. doctors are going crazy. My ID doc doesn't want to start me on my hiv meds due to my high sugars because he said what I would take may make my sugar go up on it's own....Atripla/Abacavir.....And my primary had increased 2 out of 3 of my pills already. He is trying his best to keep from taking shots. But this is still kinda new to me because I just found out I was diabetic about 2 years ago and it runs on both sides of the family. How do you maintain when it comes to eating and things?

Hey Queen,

That's a DAMN good question.  My mom has radically altered her diet.  She is NOT overweight, but a few extra lbs.   She's a gourmet cook and she's been altering recipies to make them fit.  It sucks for her cause everything, as you know, that was healthy now isn't for her.

I'm not overweight either, but because of the diabetes issue I've decided to loose another 15lbs cause my siblings are now borderline and both are heavy.  I'm the skinny one, but I know with type 2 you can handle it by diet. 

It's one thing my mom could relate too with HIV.   We've really gotten her shapped up and she's dropping the lbs and cooking differently etc.   Of course, she claims she has no idea what diabetic shoes are cause she's like a little Imelda Marcos ; )   On that one we agree.   Fashion hurts! ; )   But she cracks me up when she talks about all the diabetic stuff and the crap they tell her.  She'd rather die than be overweight from so MANY drugs and not have her heels ; )

I've been talking w/ her and she is considering jumping in as an activist herself.  Who would have thought?   One thing I will say is my mom has now weeded out all the horrible meds from diabetes for the most part.   At her worst she was up to about 10 a day.  Now she's down to 2 or 3.  I think she'd have some good input for diabetics and as a supporter of people with kids w/ HIV so I may help her set up a new web forum for what she's wanting to do.

But, yeah, all these damn meds got side effects.  Whether your HIV+ or diabetic or both.  The less pills the better I think.

If you can get your blood sugar under control I think you will do as well as anyone with the HIV meds.

Hey there, Wesley. I think that's great how your Mom is coming along. At the moment, I am on 3 different diabetic meds with 2 of them being increased. But I will admit, Pepsi is my downfall. I can't live without it. I try to make up by drinking a few bottles of water.
 
I think it's great that your Mom is becoming an activist. I wish I had the balls to do it, maybe one day. It could happen...lol

Oh, you are so bad ; )   Switch to diet!   So, the chemicals are not so healthy, but you will drastically cut your blood sugar and depending on your weight situation will likely be thinner yet.   Those Pepsi's loaded w/ pure sugar are gona screw up your glucose levels and you know it ; )  Why not have a Snickers too? ; ) 

I actually can't stand the regular sodas now.  Way too much sugar and I love that Equal stuff.   It's a preservative and will help keep you young : )

You ain't never lied. The only diet soda I could tolerate was diet dr. pepper but bought a can recently and now that tastes like crap. I do drink Crsytal Lite at times. I don't eat a lot of sweets even when I have the munchies, I tend to eat more veggies. But will admit to giving in to my craving of cookie dough ice cream.

Hey Wesley -
On the Type 2 Diabetes, there are two possible causes of it.  One is that your pancreas is not putting out enough insulin for your body.  This is typical of overweight people but the other side of Type 2 is that the body is able to produce sufficient quantities of insulin yet the cells are unable to proces the insulin properly.  Type 1 is an autoimmune disorder where the body produces antibodies to attack the pancreatic cells producing insulin.

The 80 mg/dl is normal for fasting.  My sugars are running 65-75 in the morning usually but my A1C is 5.0 which is on the low side even for people who aren't diabetic.  I wouldn't worry about it unless you find yourself getting super shakey a couple of hours after meals.  My brother was hypoglycemic and he would see his blood sugar plunge after a meal because his body produced too much.

Queen -

The Pepsi is probably the biggest problem in your struggle to get your sugars down.  I drink a real coke if my sugars crash and it only takes about two sips and my sugars will go up 100 mg/dL.  It's pure sugar and it plays hell on you.  I have been drinking that stuff for so long now that I think real anything tastes funny. 

Have you tried some of the diet ice teas?  The Arizona Diet Green Teas are awesome.   It's my new fetish other than Coke Zero with a slice of lime in the glass.  The thing about being diabetic is you don't keep yourself from the things you love, just moderate it.  Also, exercise (a four letter word for some) will help your muscles get hungry for glucose.  A workout and increased muscle mass will help tremendously.  It revs your metablism resulting is some impressive numbers. 

I am not sure what your doc has done for you but I would find a nutritionist and explain you addition to Pepsi and other foods.  If she is good, she'll have create a meal plan which incorporates what you need.

I have had complications with my eyes about seven years ago so I know the downside of letting control get away from you.

Bud~~

When I first tried to stop drinking Pepsi, I had started drinking Lipton Green Tea. I was proud of myself til my doctor told me that wasn't really good for me either because of the caffeine and that it would dehydrate me faster. That's when I went back to the Pepsi.
 
I do have a mutritionist but she only comes in so many months from Pittsburgh, I think. I am good about not eating a bunch of sweets even though I do treat myself. I'm more into veggies but does occasionally enjoy a good steak or roast, which I am gonna cook today.

I will admit that I do need to exercise but needs a workout partner who is motivated to go. I am considering getting a 6 month pass to the Y.

One thing my nutritionist taught me was that everything turns into glucose.  Some are easier to process into glucose (like sweets) but meat, vegetables, etc. will be processed into glucose so the body can use it.  The ideal thing to do is eat lots of veggies but avoid the starchy ones like potatoes since they actually aren't that good for people watching their sugar.  For me I am allowed 15 fries and that's it but it's fun (i'm a nerd at tiems) to see how fast the glucose reading shoot up from that. 

The caffiene in tea and Pepsi is about the same from what I have seen on the internet unless you are drinking the caffiene free pepsi.  The question is how much you're drinking.  I have discovered I really like the flavored waters but that can be expensive considering what they're charging for them.  Also beware of the juices since they're also laden with fructose and corn sweetners.  The juice is just an afterthought.

My big weakness is breads especially the loafs of sourdough fresh from the oven.  I never can eat a single slice so I just avoid it as much as I can but I also take a diabetic holiday sometimes where I enjoy a nice meal provided I have done well all week.  It's the small victories that makes it durable.

My doctor declared me an Insulin-dependent type II diabetic about seven years ago.  Actually it was caused from a feeding I was getting in a tube that had high sugar content in it and my pancreas couldn't handle it.  But now it's under good control.  I agree-Pepsi is sure to only progress diabetes along.  I never did control my diet for the first four years and paid heavily.  I have problems with my eyes also due to diabetes, but they're actually improved from what they were.  Nevertheless, I do have the glasses clause on my license.  But diet control is so worth it.  I never did feel good when my sugar was way high.  It's just something else to deal with..... :-\
