Anyone on Truvada/Tivicay

Hi everyone, just started my meds for the first time, DR put me on Truvada/Tivicay and so far so good, no noticeable  side effects so far... As i go through the forum, i noticed that no one is on this regime. I am wondering if my dr is advising right since She only prescribed these two medicines even though my cd4 are below 200 and my VL is way too high...is it normal to have such low cd4 and high vl and only have these two prescriptions...also, can these medications be taken slightly at different times, lets say every day at dinner time (whatever this may be) or does time (by the hour) really matters. I have no issues taking them every day, is just the timing that bothers me a bit...thanks

Hi Turtle, welcome to the forums.

Tivicay has only recently been approved for use, so there are fewer people on it than there are some of the older meds. However, there are still quite a few members who are taking it. Put Tivicay into the forum's search engine and you'll get plenty of results.

You are actually on three meds; there are two meds in Truvada. You're on a very effective combo, and provided you take your meds every day your VL will go down and your CD4s will increase.

As for timing your doses, try to take them as close to 24 hours apart, with a lee-way of an hour or two either side, at least until such time as your VL is undetectable. Once you're undetectable the most important thing is that you take them every day. The timing is less crucial once you've been taking them for a while and are undetectable.

Hey Turtle! I have been on Truvada and Tivicay since last August. This is the best med regimen I have been on in the 5 years I have been HIV positive! Hardly any side effects at all :) I had a headache the first week or two but no other complaints. Good Luck and let us know how you are doing!

I also take both meds right before I go to bed every night around 10pm. Easy for me to remember and stays consistent!

This is same combo I am on also. I take mine in the morning. I get up pretty much the same time every day so it's for me to take my pills/ supplements.
I like morning time so I don't have to worry about being home at a certain time to take meds. That was my stress with previous regime. And we are so lucky compared to those that came before us as far as convience.

Finally it is approved in the European Union since yesterday. I am sure that there will be many new users in the near future.

But You should not use it together with supplements in the morning:

"•Tivicay interacts with certain antacids and laxatives, and with calcium and iron supplements; check with your medical provider before using these. In general, you should take Tivicay 2 hours before or 6 hours after taking these types of medicines and supplements."

Since Tivicay is new its drug interaction profile is somewhat unknown...
And they already know it interacts with iron/calcium supplements as well as other simple things like antacids.

So I take my vitamins/supplements in the morning and take the tivicay/truvada at night around 9.  Seems to be working for me.  CD4 up and VL UD.

Hi all and thanks for your responses, it is great to hear such great support on this forum... I am now on my first week of this regime and I am Loving it, no side effects at all, and it is easy to follow... We will see how my VL levels are next week when I have to do my lab work...

Another concern, I started coughing theses past three days randomly with no other side effects, I think i got it from a family member who has the flu, i am panicking cause i dont know if i have to go see the Dr, i feel fine and i i only cough once or twice each half hour, no headache or fever, i am scare is going to develop into TB or pneumonia... I am vaccinated for both and for the flu too... Should i worry or should I wait a couple of days to see if it passes... Any advice? Also i feel like my immune system is going to shatter into pieces any time... Is this a reasonable concern or can the body fight off almost to the normal levels?

Turtle, relax. You're going to get colds and flu just like anyone else, hiv poz or hiv neg. I take it you're quite recently diagnosed? It's normal to feel this way in the beginning, but you need to start letting your head overrule your heart where this type of thing is concerned.

You're on treatment, you've been vaccinated against the important stuff, and so there is absolutely no reason to think you're going to come down with anything life-threatening any time soon, if ever. 

If you are coming down with a chest cold, you only need to do what you would have done in the same situation before your diagnosis. Drink plenty of fluids, get yourself an over-the-counter cough med, and you'd only need to see your doctor about it if it persists or you develop a high fever. Don't freak yourself out about it; that will only make you feel worse.

Like I said, it's completely normal to feel like this in the beginning, all of us have been there. I remember freaking out in a bookshop when I was newly diagnosed. I was kneeling down looking at the bottom shelf of books, when a woman came along and looked at some books above me and started to cough. I ended up sprawled across the floor in my efforts to get away from her (my feet got tangled up somehow). I felt like she was trying to kill me! ;D I look back and LMFAO now, but it really upset me at the time.

So know you're not alone, but also know that there's really no reason to fear every little cough, sniffle or twinge and doing so isn't going to do you any good. You're going to be ok. Really, you are! It does get better in time, trust me.

After 9 years on Atripla, I started Tivicay/Truvada 2 months ago.  LOVE IT.  No side effects (first week had headaches).  I was averaging 600 T Cells and 32% forever.  Now I am at 992 T Cells and 40%? WTF.  Best combo ever....so far.

Wow congrats Joe, me too, i have been taking fhe combo for exactly one month and my VL went from 192000 to 42 (almost undetectable) and cd4 went up from 156 to 264 and all this in one month with minimal side effects and almost no disruption of my daily life... I did so well, i am going back to work in two weeks....feeling blessed

Man, my doctor is very happy about this combo over anything else. Even Stribild. Is there a food restriction with this one?

Technically no... Some people find it better when Truvada is taken on food (a subtential meal) something to do with gas ... Eric

I have been taking my new meds (Tivicay and Kalexa) for almost 10 days. I had trouble sleeping by having some dreams....
I am now feeling better

Hey. Also on Truvada and Tivicay. Newly diagnosed in May 2014 and started these as first treatment in June. My CD4 starting was 285 and VL at 254,000. My first test at 30 days on this treatment was CD4 at 587 and VL at 77. I have had zero side effects. They told me about vitamin supplement interaction with Tivicay so I take those in the morning and  the Truvada Tivicay with dinner. So far I feel good but hoping for an undetectable viral load.

Thank you for posting this.  When I read up on it, I read that supplemental calcium was the issue and not multivitamins.   However going to the viiv website, that seems to be inaccurate.

"Calcium supplements, iron supplements or multivitamins should be taken well separated in  time from the administration of dolutegravir (minimum 2 hours after or 6 hours before)."

 I specifically looked up information because I wanted to get off twice-daily Isentress because of massive headaches/insomnia and I sometimes miss the nighttime dose (it's the only night medication I take) and taking the Tivicay with my multi and yogurt was my goal.  I'll move the vitamins to night (yogurt doesn't have enough calcium I guess to be a problem.)  I start in about a week. 
Dates are blood draw dates:
3/12/15: CD4 941, 36.4%, VL UD
9/4/14: CD4 948, 37.9%, VL 150
5/23/14: CD4 895 --.-% VL UD - Truvada/Isentress
09/21/09: CD4 898 27.0% VL 120 - back on track, same meds.High level enzymes, but less so
06/15/09: CD4 478 21.8% VL 1150 - high liver enzymes... looks like I may not be resistant
05/22/09: Fixed insurance, resumed medicine
04/17/09: Ran out of medicine, could not resolve insurance problems
04/01/09: CD4 773 28% VL 120 - high liver enzymes
12/01/08: CD4 514 23% VL 630
10/17/08 started Reyataz, Norvir and Truvada. -- possibly minor neuropathy, but otherwise okay.
9/10/08: CD4 345 17%, VL > 78K

Are you gaining body fat or muscle?  If you gained 17-18 pounds of muscle that's a good thing.

I think it's somewhat common when starting meds, nothing specifically to do with Truvada or Tivicay.  I switched to this combo maybe 18 months ago.

I believe leatherman describes it as your body is no longer spending lots of resources combating the infection once the meds knock the virus down.

I thought this combo was ok at first, however, the medicinal sort of feeling I found on other regimens with Truvada (such as Norvir/Prezista) was lingering with this regimen as well.

My doctor started me with Truvada/Tivicay. As of this posting, I am 22 days into my treatment. No issues with side effects. The first (3) days, I had very vivid dreams. That has since dissipated. I take everyday at 6:00 p.m. Dr. said to take with or without food. I have found that with food is best for me.

Being how I'm new to treatment. I asked the doctor in her opinion was this the best option to start with and she concurred. I can't wait to go back on 11/05 to see how my body is responding.

Hi everyone. Thought I'd chip back in.

After a full week I can tell you: I HAVE HAD NO SIDE EFFECTS. None. Zero. Zippo. Zilch.

On the second day I sweated a lot: then we had a massive thunderstorm that afternoon: humidity.

I am grateful that this combination is so tolerable. Like, I was prepared for anything. Seriously, I still have nothing to blame on these meds.

My first time ever posting...

Anywho, I've been on truvada/tivicay for five days now. My doctor switched me from Atripla. I had severe insomnia the first night. The insomnia decreased in intensity every following day and I'm excited to say that I slept like a baby last night. Hopefully I'm over that hump. I work in the construction field so sleep is essential for me if I want to not drop dead while I'm on the clock.

Sleep is essential for sanity! I am soon to be switching off of Atripla and am seriously considering Truvada+Tivicay. I've never been on an Integrase inhibitor regimen. The insomnia people talk about concerns me a bit but overall sounds like people are doing well with it!

Truvada was poison for me and I won't take another nuke ever again. I had been on it since it's introduction and before that my experience with nukes went back to the AZT days.

Virtually every organ system in me was adversely affected-opthalmic, muscular, skeletal, venus/BP, nervous, skin, you name it. Hopefully others won't have the same side effects but I don't see how they couldn't and remember it's effects are cumulative, i.e. over time more bone loss and decreased kidney function. By the time the idiots catch it you've suffered needlessly.

Yes Truvada has dramatically reduced our communities viral burden and reduced transmission but at a horrendous cost to the individual and one that many in the  medical community are oblivious to.
2016 CD4 25% UD (less than 20). 30+ years positive. Dolutegravir, Acyclovir, Clonazepam, Lisinopril, Quetiapine, Sumatriptan/Naproxen, Restasis, Latanoprost, Asprin, Levothyroxine, Restasis, Triamcinolone.

Since this thread came back up, I thought I would discuss something I just read Dr. Gallant say.  On his forum, he said when he asks his colleagues what they would take if they were poz, he said it's almost unanimous that they would choose the best of both classes-- Tivicay and Truvada and switch to the new Truvada, when it becomes available early next year. 

The downside is having to pay 2 copays, but that isn't an issue for everyone.

This is a resonnable strategy, especially since the SWORD1 clinical trial is on going and should lead to a one pill (Tivicay+Rilpivirine). This would be a simpler way to ditch the Truvada

I started trivivay/truvada 5 days ago. The only thing I've noticed is nausea. Very glad I saw this post. Great info.

nausea, tierd ,headache, and I feel I want to go to the bathroom and always whistling in the ears ... And just two days  truvada +tivicay
