Oral thrush... I knew I was on too good a roll
« on: March 31, 2011, 02:09:14 pm »
I've been feeling great lately but it seems my body had to go and remind me that all is not perfect. Just noticed some thrush starting at the back of my throat near my tonsils. I wouldn't mind some advice fom you guys, especially if you've had a visit from our thrush friend before.

My background is Dx May 2010 with PCP, VL stupidly high & CD4<10. Went on HAART + bactrim + fluconazole (200mg daily). Now UD, CD4 slowly climbing to 190 last month. I stopped on the daily fluconazole under dr instructions a month ago.

I don't know if I should be fairly agressive with this or just let my GP deal with it. I have fluconazole 200mg tabs still so plan on just restarting these daily and going to my GP. She's not experienced with HIV at all so I usually ask her about problems relating to HIV but always discuss with my ID dr on the next visit.

My thinking is to see if it clears up quickly first. If it doesn't, then start worrying and talking to the ID dr. I've read that thrush is more of a problem when it is in the throat and esophagus.

I would contact your ID doc for all things connected to HIV.
Diagnosed in  May of 2010 with teh AIDS.

PCP Pneumonia . CD4 8 . VL 500,000

TRIUMEQ - VALTREX -  FLUOXETINE - FENOFIBRATE - PRAVASTATIN - CIALIS

Numbers consistent since 12/2010 - VL has remained undetectable and CD4 is anywhere from 275-325

Excellent advice!

I cant speak to your situation specifically but do know that my GP had me on bactrim and fluconazole then my ID Specialist upped the dosage of fluconazole to kick it in the butt!  It worked and never came back and I had a pretty bad bout with thrush.  (March 2010).  I think I stayed on fluconazole for a couple of months, would have to check my records to give a definitive answer on the exact duration, but it has not come back.

I was pretty much in your situation in Feb of last year.My doctor said once my cd4 got up over 50 the thrush would go away, and sure enough it went away and never came back once i hit cd4 57.Its troublesome that you've been taking fluconozole for this long. You should only need to take it up to 14 days. Are you on any inhalers?Things like Advair cause thrush even in normal circumstances.

I may be wrong but I thought taking Amoxicillin could actually cause thrush instead of curing it.

I took Diflucan when I had it, and the concept that you need under 50 cd4s or thereabouts to get this is bs. I think I had 400.

On a side note everyone, I no longer have a GP only an ID doc. Here is how I arrived at this decision. My old GP knew very little about HIV/AIDS. Now, although I am healthier than I have been in years, because my dx was AIDS, I feel it's probably best to let my ID doc deal with everything. This, hopefully, prevents any confusion over drug contra-indications and effective treatment for almost any illness.

My partner, who is also HIV+, still has a GP for anything he feels is not related to HIV but I don't like that risk for me, personally. Because Thrush is a HIV opportunistic situation (or it can be) I would run this past an ID doc.

And you'd be correct about that, I was on Diflucan & Clotrimazole Lozenges 8 yrs. ago. I started this in 97
and was still on them until after 350 Tcells, and I got better, I still keep a fresh bottle of them in the medicine cabnet just incase the nasties come back.....

Drew, it preety much is an opportunistic situation that will get a lot worse if not treated correctly, good for you, on your new doctor  ;)
