Really worried shall I start PEP?

Hello,

2 days ago I had an encounter with a guy and I wanted to understand my risks and have been anxious.

I gave him oral and I have had gingivitus and bleeding gums a lot in the past. I understand it’s a minute risk for oral but wanted to know that if this completely raises the risk of infection as in very worried. He did not ejaculate into my mouth but I was worried about pre cum

We also rubbed dicks together and I wanted to know if it’s possible to get infected with pre cum?

Worst case scenario the risk is minute and it does not warrant testing outside of standard yearly routine.

Here's what you need to know to avoid HIV infection:
Use condoms for anal or vaginal intercourse, correctly and consistently, every time, no exceptions. Consider talking to your health care provider about taking PrEP going forward as an additional layer of HIV protection.

Keep in mind that some sexual practices which may be described as ‘safe’ in terms of HIV transmission might still pose a risk for transmission of other STI's, so please do get fully tested regularly and at least yearly for all STI's including but not limited to HIV and test more frequently if unprotected intercourse occurs

Also, note that it is possible to have an STI and show no signs or symptoms and the only way of knowing is by testing.

The next day I returned and had unprotected anal sex as I was drunk and now I’m worried sick. I have started prep just under 24 hours from the unprotected anal and it was about 50-60 hours from the oral.

She is an escort and I have asked since if she is clean and she is ignoring me which makes me worried and she insisted on it being without condom.

I know I am in the highest risk category, but could you help me understand my likelihood from these two exposures and the other factors. And also help me understand how highly effective pep would be if I finish the course? I’m not sure numbers will help as I fully expect to be infected but I did hear someone say it’s rare from 1 exposure.

Don't worry, I am sure she washed her penis before engaging in intercourse with you, but it's just irrelevant as HIV doesn't make you dirty; you don't get it from being unwashed, and even if her penis were unwashed, your HIV risk would remain the same...

Anyhow, you are overthinking the blowjob, its a minute risk at best that doesn't even warrant testing outside of the yearly routine screening you should be getting, let alone PEP.

As for condomless intercourse, that was an HIV risk. However, I fully expect you will be fine. PEP is highly effective. To confirm your HIV status six weeks after finishing PEP with a blood draw (lab) HIV antibody test. A non-reactive result at that time would rarely change, and retesting at three months is generally unnecessary.

Sorry, I didn’t mean like clean as in dirty or any offense, clean is a slang term where I’m from as in to say you do not have any STDs. So to say I’m clean is to say I have no STD’s. Very sorry to sound like anything else or to sound like I meant hiv is dirty. That wasn’t what I meant.

So last night I made a big mistake and had receptive intercourse with two transgender escorts.

I did get them to use a condom even though one didn’t want too and it seemed it was used ok and for duration other than it looked like it slipped slightly.

You didn't make a mistake. You required condoms for penetrative sex. That's great!

As far as the big finish, teeth often bleed mildly after brushing, but that is NOT a risk for HIV. Wounds are designed to bleed outwards and the receptors that HIV use to infect are corroded by changes in temp, air, etc when outside of the body.

You do not need PEP; but you do need to continue making sure to use condoms just like you did. You might want to consider PrEP to give yourself more peace of mind over these kinds of incidents.

Reducing Your HIV risks:
With no exceptions, use condoms correctly and consistently for anal or vaginal intercourse
Talk to a healthcare provider about PrEP as another layer of protection

Get tested yearly for HIV and other STIs.
If you don’t use condoms and/or PrEP, test more frequently

Some sexual practices described as safe in terms of acquiring HIV still pose a risk for other more easily acquired STIs. It is possible to show no signs or symptoms from an STI so testing is the only way to know.

Get tested at least yearly for STIs, including but not limited to HIV, and more frequently if condomless intercourse occurs.

What’s the ONLY way to know if you’ve been infected by HIV or an STI?
Get tested.
