Defined by Numbers
« on: June 06, 2013, 10:49:34 PM »
I got my latest labs back, but the doctor was out, so, I'm just picking through them. I don't know anyone who's interested in this kind of thing, so let's pretend you're my proxy for someone who gives a shit.

My white blood, red blood and hemoglobin counts are all below the minimum for "healthy", and my hematocrit is lower than the allowed range too. I think I have low testosterone, but I don't know how to read this part. That obviously concerns me more than any silly blood problems. As for HIV, I'm still undetectable, but my CD4 dropped from 1123 to 945, and my CD8 from 853 to 732 since February. In my defense, this roughly corresponds to when I had nowhere in particular guaranteed for sleeping, so any number except 0 is really OK by me.

So boo. I hoped this would be a better round. I suppose I'll have to start eating food, but I'm going to wait to see what the doctor says before I do anything rash like that. I'll let you know what happens next in September.

Well, it is good you are UD and your CD4 is still awesome.  How far below normal were the other labs?  I know hematocrit is often lower for many of us.  Mine was always a tad below normal, until recently.

Perhaps the low levels are some lab issue?  Every time my partner does his firefighter physical, many of his levels are off-- WBC, RBC, neutrophils, basophils, etc.  He will then have labs done at his doc, and they're all normal.  This has happened the last 3 years.  I think the lab they use for the fire dept has some issues.  I think they are using some discount laboratory, that may not have the best standards.  Yes, he isn't poz (knock wood), but it is interesting how one lab always shows his labs as being off.  Of course, yours may be very accurate results.

Why do you think you have low testosterone?  Low energy?  No sexual desire?  Do you wake up with morning wood?  I know that can be frustrating and has more implications than just sexual desire.  Btw, I am glad to see you posting.

Hey Oks,

I am also glad to see you posting - and do appreciate the perspective you bring to the boards.

Your comment on testosterone was of interest.  Is it just the numbers or are you having other physical symptoms?  I was at the doc today, explained that I have had some night sweats, headaches, fatigue, etc. more than usual and his first comment was that I may be low on testosterone.  I will go in next week for the test. Even if I am low, I don't think I will supplement unless I really have to.

This round doesn't look too bad, but I wish you the better on the next round.

Your numbers (the one's you posted) are fine.

Numbers go up and down all the time,  it's normal.  None of the numbers that you posted should be of any concern.  Easy for me to say,  right?  ;)

Any lab test is just a snapshot of a particular moment.   Changes from one moment to another don't necessarily mean anything.

My white and red blood cell counts are frequently out of the normal range.  I worry myself senselessly for awhile and then for no apparent reason they go back into the normal range.

What are the other numbers that you are concerned with?

Once you're > 500, your CD4 can fluctuate a lot, and this change is clinically (and statistically) not important.
Mine keep going up and down in the 750-1350 range ;)
It's normal, especially if your CD4% is stable.

Thanks for your responses, folks.

Yes, I'm not too worried about my CD4 unless it drops next time too. My complaining about that is sort of like 'first world problems' at this point. I was "almost anaemic" last time. I'm not too surprised that I crossed the threshold.

As for testosterone, maybe it's overrated.

;)

PS : Oh, and my CD4% is stable across all three labs ever I've had done since diagnosis. Thanks for reminding me.

Oh, I forgot to respond to anyone. Sheesh. :)

Well, it is good you are UD and your CD4 is still awesome.  How far below normal were the other labs?  I know hematocrit is often lower for many of us.  Mine was always a tad below normal, until recently.

Why do you think you have low testosterone?  Low energy?  No sexual desire?  Do you wake up with morning wood?  I know that can be frustrating and has more implications than just sexual desire.  Btw, I am glad to see you posting.

WBC was 4.3 K/µL ( normal is 4.5 - 10.8 )
RBC was 3.91 M/µL ( normal is 4.5 - 5.9 )
HGB was 12.3 g/dL ( normal is 13.5 - 17.5 )

LYMPH was 45.8 (normal is 18 - 44)
MONO was 3.5 (normal is 4.7 - 12)

I also have low cholesterol (HDL and LDL), which I hadn't noticed until just now. Lord. Ha. :)

Testosterone: yes, don't care, no.

Good luck with your test! Agreed, I'd rather have low testosterone than artificially high. My understanding is that high testosterone puts you at risk for all kinds of guy cancers. I don't notice any real symptoms, but I do feel weakish a lot. I think I mentioned in my other post that my pedometer tells me I walk at least 50,000 steps per week, which isn't too bad. {Note: if anyone has an iPhone, try the Moves app. It's not a pedometer per se, but there's one built in. It's free and pretty cool to see everywhere you've been all da/week/month. Just keep an eye on your battery until you know how much it uses.}

My white and red blood cell counts are frequently out of the normal range.  I worry myself senselessly for awhile and then for no apparent reason they go back into the normal range.

What are the other numbers that you are concerned with?

Do they go up and down or is it always one extreme? My blood counts are consistently low, and I shouldn't be surprised with my eating/sleeping schedule what it is. These are the lowest I've seen, but I think the doctor would've called if they were horribly alarming. She's pregnant and out just at the moment, but she's a great doctor and wouldn't leave me hanging over anything serious.

Really it's just that and the testosterone numbers. They're all within range, but at the low extremes. If this is illuminating:

SEX HORM BIND GLOB 44.5 nmol/L ( normal 13 - 71 )
TST TOTAL 535 ng/dL ( normal 262 - 1593 )
TST, FREE 1.8% ( normal 1.7 - 4.1)
TST, FREE 98 pg/mL ( normal 50 - 350 )

Once you're > 500, your CD4 can fluctuate a lot, and this change is clinically (and statistically) not important.
Mine keep going up and down in the 750-1350 range ;)
It's normal, especially if your CD4% is stable.

Yes! CD4% is always 48. I forgot that this matters.

Nice to see you posting. Sorry its about worry about misc blood numbers.  When can you ask a doc about this, and will that doc be any good? And do you have a doc who follows you, or different docs all the time?
Considering your recent lodging and employment nightmare, your worries are hardly "first world problem" frivolous. You've had a LOT on the plate.

Is the housing still holding?

Anyhoo, send you best wishes....

Hey, thanks for asking. Gosh, I'm not worried-worried - just melodramatic and adjusting to reading labs on a regular basis.

I know my numbers are (mostly) fine. I think I mentioned once that before HIV I hadn't been to the doctor except the health department for HIV tests in, like, 10 years. I'm not used to having so much information to pick through. Haha. I just posted because it's good to hear what other people think of as "normal" and "un-usual". That's why I started reading these forums in the first place, really.

If the people in "Am I infected?" would read a few posts instead of raising the panic straight to 11, they'd probably be able to sleep easier. There's not much you can think of that someone else hasn't already dealt with, and somehow the world keeps turning. Unfortunately, people tend to not show up here until they're already panicked. I prefer to panic early and alone so you don't actually panic when anyone's around with a mini-cam.

A few of my numbers are good warning signs, though. I doubt it's healthy to stay anaemic for a long time, and cholesterol has a minimum for a reason (not that I've looked that up [yet]). The testosterone is just a me-Tarzan, vanity thing, like working biceps at the gym instead of exercising lower body which is probably an overall healthier thing to do.

I have a really great doctor. I go to the place recommended by Miss P a long time ago: Center for Comprehensive Care. It was through GMHC that I linked up with them originally, though, and they do everything (dental, HIV, nutrition etc.).

I honestly see nothing all that "abnormal" in your lab results.

Your CBC (WBC, RBC, HGB, etc) is nothing that I would worry about -- and I worked in a Hematology lab for many years.  Folks do stress over results that fall outside reference ranges (I don't call them "normal ranges" because they aren't based on YOUR normal), but when they are just a bit out there is usually little to worry about.

So -- don't be defined by your numbers -- but if you must -- choose the great VL and CD4 results!!

Ive had similar thoughts about my labs as well.

A few days back I cleaned out a drawer and found some old medical notes and test results, (including one HIV neg test, for what its worth now...  ::))
But I also had a CBC test in that drawer from approx 2 years back, so I sat down starting to compare my CBC test results HIV + and -, turns out I had MORE out of range results on the test when I was Neg, the HIV test was taken about a year after the CBC so I was not poz at that time.

Just shows that we shouldn't read TOO MUCH into these numbers, be observant yes but don't obsess over it.

I have no idea what you're talking about. If you're trying to make me feel bad for posting a discussion on a discussion board, go try somewhere else.  :)

In the beginning the amount of information that you're given about your health can be information overload.  It's not at all unusual to be concerned.

But looking at your numbers there doesn't seem to be any reason to be overly concerned.  The numbers that are out of "normal" range are barely so.  So I wouldn't worry.  You should always discuss your concerns with your doctor,  and never feel apprehensive about bringing your concerns up here.  No matter what anyone says.

My red and white blood cell counts were out the normal range (and dropping) for years.  I was very concerned but my doctor wasn't.  He was right,  I wasn't.  They eventually returned to "normal"  without any intervention on my part -  aside from a great deal of unnecessary worrying.   ;)

I also once had a total testosterone count of 90.   My doctor said "you have a testosterone count of an 85 year old.........woman".   That eventually normalized too.  Go figure.

BTW,  your CD4 count and % is stellar.

oksikoko - care to elaborate on what thoughts led to this thread title?  Your status weighing heavy, or what?  Your responses seem reasonable and informed... Who / what is defining you?

(Far as I can tell, the word "fizz" and "frizz" are used to make assorted portmanteaux on Twitter.  That's all I got.... )
