Research Reveals Oral Sex May Be Leading Cause of Mouth, Throat Cancers

Mouth and throat cancer, called oropharynx cancer, used to be a disease seen most commonly in elderly persons. Tobacco smoking and alcohol use were known to be the leading causes of oropharyngeal cancer. Today, oral sex is listed as the leading cause of cancer of the mouth and throat.
New research, published in the Journal of Clinical Oncology, demonstrates that human papillomavirus, HPV, is the leading cause of cancer of the oropharnyx in the U.S. The number of people diagnosed with HPV-related oral cancers in the U.S. tripled from 1998 to 2004.
Currently, as NPR reports, almost 10,000 new cases of oral and throat cancer are diagnosed each year, with a 28 percent increase in incidence since 1988. Interestingly, the majority of those who are being diagnosed with the HPV-related oropharyngeal cancers are men. This leads researchers to wonder if the vaccine against HPV that is recommended for teenage girls may be affording that gender with protection against oropharyngeal cancer, as well as the currently known protection against cervical cancer.
Researchers have noted a marked drop in non-HPV-related mouth and throat cancers since the American population has begun to turn away from tobacco smoking.
The oropharyngeal cancer most commonly affects the tonsils, palate, base of the tongue and the upper throat. Whereas this type of cancer was previously seen most often in aged patients, it is now more common at younger ages, including baby boomers and their juniors, reports Bloomberg News.
HPV is the most commonly transmitted sexual disease, but as Dr. Gregory Masters, an oncologist at the Helen Graham Cancer Center in Delaware reminds us, research is not yet clear that oral sex is the main or only transmission factor in this cancer,
Dr. Maura Gillison, author of the research published in the Journal of Clinical Oncology states that persons who have had six or more partners on whom they've practiced unprotected oral sex are eight times more likely than those who have not had oral sex to develop the HPV-related mouth and throat cancers, reports CBS News.
Merck, the pharmaceutical company that manufactures the HPV vaccine, has issued a statement that there are no current plans to research the usefulness of the vaccine against oropharyngeal cancers.

Rising rates of oropharyngeal squamous cell carcinoma may stem from a new epidemic of sexually-transmitted human papillomavirus (HPV), according to a review.

The incidence of oral cancer has been on the rise over recent decades in the U.S. and some northern European countries, noted TorbjŲrn Ramqvist, of the Karolinska Institute in Stockholm, and Tina Dalianis, of the Swedish Institute for Infectious Disease Control in Solna, Sweden.

The corresponding increase in the percentage of those tumors positive for HPV over the past 10 years isn't simply because of more sensitive testing, they wrote in the November issue of Emerging Infectious Diseases.

For example, the Swedish Cancer Registry documented a 2.8-fold increase in tonsillar cancer around Stockholm from 1970 to 2002. Examination of all available tonsillar cancer samples indicated a rise in HPV-positive tumors over this period from 23% to 68%, with a further jump to 93% by 2007.

Similar patterns were seen for tumors at the base of the tongue, the second most common type of oropharyngeal squamous cell carcinoma.

"We suggest that this increase is caused by a slow epidemic of HPV infectionĖinduced oropharyngeal squamous cell carcinoma," Ramqvist and Dalianis wrote in the paper.

Changes in sexual patterns are likely to blame, "such as increased oral sex or increasing numbers of sex partners," they added.

The HPV-positive rate is much higher -- at 45% to 100% -- in oropharyngeal squamous cell carcinomas than for other types of head and neck squamous cell carcinomas, in which HPV DNA has shown up in only about one-quarter of tumors over the past decades, the reviewers noted.

Studies have shown that HPV present in these tumors is integrated into the genome and actively involved in their development with gene expression similar to that seen in HPV-positive cervical and vulvar cancer.

The International Agency for Research against Cancer now recognizes HPV infection as a risk factor for oropharyngeal squamous cell carcinoma, particularly the high-risk HPV type 16 (which accounts for about 90% of HPV in these tumors).

Other known risk factors -- smoking and drinking -- are often absent in patients with HPV-positive oropharyngeal squamous cell carcinoma.

One study recently linked open-mouth kissing to development of oral HPV infection, which "could play a major role in timing of prophylactic vaccination of children," according to the review.

Vaccines directed again HPV16 (Cervarix and Gardasil) are recommended for girls at age 11 or 12, before becoming sexually active, but can be given to girls as young as 9, depending on circumstances.

"Although it will likely take several decades before the effects of HPV vaccination on cancer incidence will be detected, it is crucial to monitor the effects of the present HPV vaccination, not only on the incidence of cervical cancer but also on the incidence of oropharyngeal squamous cell carcinoma," the reviewers wrote.

The research was supported in part by the Swedish Research Council, the Swedish Cancer Foundation, the Stockholm Cancer Society, the Stockholm City Council, and the Karolinska Institute.

 In October of 2003, My t-cell count was 16, Viral load was over 500,000, Percentage at that time was 5%. I started my first  HAART regimen  on October 24th,03.

 As of 6/4/14,  t-cells are at 423, Viral load <40

 Current % is at 13%

I thought it was approved for boys and young men (ages 9-26) in 2009 (against genital warts. but that wouldn't stop a doctor from using it to prevent other sexually transmitted diseases as well)

So you're saying a colorectal doctor can't visually observe anal dysplasia unless the patient has had warts previously?

Which is why everyone should have an anoscopy -- have you had one (silly question of me to ask, I know). Regardless of if you "think" you and your partner are monogamous, or if you've never had anal warts, or if you're a top, or if you never have anal sex (etc. etc.) any previously sexually active gay man with HIV should have an anoscopy, preferably a high-resolution anoscopy (if you can find that).

All I know is that we don't have any  visual warts, or any dysplasia, if that is what your asking me?  we've both had  colonoscopies so there was nothing there so  what's your point dear........

Anal warts and anal dysplasia are not the same thing. A colonoscopy and an anoscopy are not the same thing. A colonoscopy does not detect anal dysplasia which is what can lead to anal cancer. As previously stated you haven't the slightest idea what you're attempting to discuss (shocking, I know...)

So, I should probably ask my doctor when I see her on Monday to schedule a anoscopy for me.... I have never had one.  Believe me, I would remember if something went up there (I might not remember the person's name - but I do remember when something goes up there  :o  )

I had anal warts back in 1987 (way way before I was poz) --- they treated with podopherine (or something sounding like that) - they went away and never had them again...

I will check with doc  and see if she will schedule it.  Wouldn't want to have any of "my parts" start failing on me.

People should not put anything in their mouth that could harm them.  We were taught that as children and NEVER put anything in your mouth that has been on the floor.

No I didn't say that what I said was we don't have anal sex (with other men)  nor do we have oral sex with any we don't sleep around if that is what your asking me, were very happy with each other  ;)

Well there is the HPV vaccine and now its recommended for boys too.
So here's hoping that generation grows up to encounter less of these troubles.

Oral sex is probably one of the leading causes of relationship happiness! (if done well and often enough!)

People should not put anything in their mouth that could harm them.  We were taught that as children and NEVER put anything in your mouth that has been on the floor.   

You forget the three second rule :)
